La trombosis de la vena ovárica es una complicación rara del posparto (0,1%).

Es potencialmente letal, ya que puede ser origen de sepsis, tromboembolismo pulmonar y trombosis de la vena cava inferior, o de las venas renales.

La tomografía computarizada (TC) y la resonancia magnética (RM) son las técnicas de elección, para su diagnóstico, teniendo los ultrasonidos un valor limitado, dada su baja sensibilidad y especificidad.

Presentamos el caso de una mujer que en el puerperio tuvo como complicación una trombosis de la vena ovárica derecha, cuya clínica, imagen ecográfica y por TC simulaban una apendicitis. Descubrimos los signos radiológicos y subrayamos la necesidad de sospechar su diagnóstico en pacientes puérperas.

Ovarian vein thrombosis is a rare postpartum complication (0.1%).

It can be fatal, since it can lead to sepsis, pulmonary thromboembolism and inferior vena cava or renal vein thrombosis.

Computed tomography and magnetic resonance imaging are the techniques of choice for its diagnosis, while the value of ultrasound is limited due to its low sensitivity and specificity.

We report the case of a woman who, during puerperium, developed thrombosis of right ovarian vein that presented clinical, ultrasonographic and computed tomographic features of appendicitis. We describe the radiological signs and stress the fact that this diagnosis should be suspected in puerperal women.
