Resumen

Se han valorado 258 pacientes afectas de linfedema secundario a intervención quirúrgica por cáncer de mama, a las que se había realizado una linfadenectomía, que se trataron mediante drenaje linfático manual y otras técnicas de terapia física, con el objeto de conocer el coste y la efectividad del tratamiento. La edad media de las pacientes era 57,3 años (DE 8,5).

El linfedema se cuantificó a través de la medición de los perímetros en ambas extremidades, la homolateral a la mastectomía y la heterolateral, a siete niveles: en la mano por detrás de los metacarpianos, en la muñeca, en los tercios medio y superior del antebrazo y en los tercios inferior, medio y superior del brazo. Para el cálculo del perímetro total de cada extremidad se sumaron los perímetros obtenidos.

Se determinaron todos los gastos generados por la atención sanitaria: gastos de personal (médico, fisioterapeuta, auxiliar de clínica), gastos variables correspondientes al material fungible utilizado (material fungible, fármacos, presoterapia), gastos del uso del servicio de rehabilitación y gastos de los análisis clínicos.

Se realizó una media de 28 sesiones de drenaje linfático, con un rango que varía entre 15 y 50 sesiones, obtenién-dose una reducción porcentual del linfedema del 42,2% y un rango que variaba entre el 25,5 y 51,6%.

El coste medio del proceso resultó ser de 113.235 ptas., de las cuales 98.455 ptas. correspondían a la terapia y significaban el 85% del gasto sanitario del proceso, precisán-dose 2.333 ptas. para reducir cada cm del linfedema.

Podemos concluir, por los resultados de este estudio, que en nuestro medio tenemos un buen estándar de reducción del linfedema, del orden del 42%, con un coste inferior al de otros países.

A total of 258 patients suffering lymphedema secondary to surgical intervention due to breast cancer who had undergone a lymphodenectomy and had been treated by manual lymphatic drainage and other physical therapy techniques have been evaluated in order to know the cost and effectivity of the treatment. The mean age of the patients was 57.3 years (SD 8.5). The lymphedema was quantified by the measurement of the perimeters in both limbs – the homolateral to the mastectomy and the heterolateral on several levels: in the hand behind the metacarpals, in the wrist, in the middle and upper third of the forearm and in the lower, middle and upper thirds of the arm. To calculate the total perimeter of each limb, the perimeters obtained were summed up.

All of the costs generated by the health care were determined: cost of personnel (physician, physical therapists, clinical assistant), variable expenses corresponding to fungible material used (fungible material drugs, pressotherapy), expenses for the use of the rehabilitation service and clinical analysis costs.

A mean of 28 sessions were performed for lymphatic drainage, with a range going from 15 to 50 sessions, obtaining a 42.4% percentage reduction of the lymphedema, ranging from 25.5% to 51.6%. The mean cost of the process was 113,235 pesetas, of which 98,455 ptas. corresponded to the therapy, this being 85% of the health care cost for the disease, so that 2,333 ptas. was required to reduce each cm of the lymphedema. To conclude, based on the results of this study, in our environment, we have a good standard of reduction of the lymphedema, about 42%, with a lower cost than in other countries.
