Resumen
Introducción

Entre las complicaciones más frecuentes postictus se encuentra el hombro doloroso, y su incidencia se estima hasta en el 72 % de los casos. La etiología más frecuente es el dolor complejo regional de tipo I, aunque en general se suelen asociar varias causas.

Material y métodos

Se han recogido los casos de 126 pacientes que sufrieron un ictus en el año 2000, y se han valorado las características del accidente cerebrovascular (ACV), su situación basal, complicaciones, factores pronósticos, evolución durante el primer año mediante la escala de Medida de la Independencia Funcional (MIF) y la presencia de hombro doloroso.

Se ha efectuado un análisis estadístico mediante la t de Student y se ha comprobado su significación mediante el test de la chi cuadrado.

Resultados

De los pacientes analizados, 50 han sufrido dolor de hombro y, de ellos, el 36 % era de tipo neuropático, el 34 % eran espásticos y el 30 % tenían una distrofia simpaticorrefleja. La probabilidad de presentar este cuadro aumenta con la edad (p < 0,000003), en el tipo hemorrágico (p < 0,0002) y con tener una puntuación inicial del MIF menor de 40 (p < 0,001) o menor de 80 a los 3 meses (p < 0,001).

Introduction

Among the most frequent post-stroke complications, painful shoulder is found and its incidence is estimated in up to 72 % of the cases. The most frequent etiology is type I regional complex pain, although several causes are generally associated.

Material and methods

We have collected 126 patients who suffered stroke in the year 2000, and the cerebrovascular accident (CVA) characteristics, its baseline situation, complications, prognostic factors, course during the first year have been assessed with the Functional Independence Measurement (FIM) scale and the presence of painful shoulder.

A statistical analysis was performed with the Student’s t test and its significance was verified with the Chi-squared test.

Results

Of the patients analyzed, 50 had suffered shoulder pain, and 36 % of them were neuropathic type, 34 % spastic and 30 % had reflex sympathetic dystrophy. The likelihood of presenting this picture increases with age (p < 0.000003), in the hemorrhagic type (p < 0.0002) and with an initial FIM score of 40 (p < 0.001) or less than 80 at three months (p < 0.001).
