Resumen

Se revisan los conceptos más modernos sobre la patogenia de las tendinopatías y cómo están cambiando el abordaje terapéutico. Se analizan en detalle las bases teóricas, los ensayos clínicos y los aspectos técnicos sobre el tratamiento que actualmente se considera con mayor potencial: los programas de ejercicios activos. Se describen los protocolos de tratamiento que han demostrado su eficacia en ensayos clínicos controlados y aleatorizados en las principales tendinopatías intrínsecas: tendinitis aquílea, tendinitis patelar, tendinitis de aductores y epicondilitis.

Los datos más recientes sugieren que, además de mejorar los síntomas clínicos a medio-largo plazo, los programas de ejercicios bien diseñados pueden llegar a reestructurar y reparar, en algunos pacientes, la microarquitectura tendinosa.

Summary

The most modern concepts on the pathogeny of tendinopathies and how the therapeutic approach is changing are reviewed. The theoretical bases, clinical trials and technical aspects on the treatment that are presently considered to have greater potential- active exercise programs, are analyzed in detail. Treatment protocols that have demonstrated their efficacy in controlled and randomized clinical trials in the main intrinsic tendinopathies – Achilles tendinitis, patellar tendinitis, adductor tendinitis and epicondylitis – are described.

The most recent data suggest that besides improving the clinical symptoms in the middle-long term, well-designed exercise programs may restructure and repair the tendinous microarchitecture in some patients.
