El pie plano adquirido del adulto es una patología que cada día se observa con más frecuencia. La causa principal es la rotura del tendón tibial posterior. Esta entidad tiene varias características morfológicas, entre las que destacan el retropié valgo y el colapso del arco medial. La deformidad usualmente es progresiva e inicialmente reductible pero con el tiempo pasa a ser fija. El dolor puede aparecer en la fase reductible. El tratamiento está indicado cuando la deformidad es progresiva y cuando hay dolor. Dependiendo del grado y de la deformidad, se puede iniciar tratamiento conservador con ortesis. Se aconseja tratamiento quirúrgico si no hay respuesta al tratamiento conservador.

Adult acquired flatfoot is a frequent pathology. The main cause is rupture of posterior tibial tendon. The disease has clear features: heel valgus and flattening of the medial longitudinal arch. The deformity is progressive. There is no spontaneous correction with time and many patients become symptomatic. Treatment is required when there is a progression of the deformity and/or pain. Depending on the grade and deformity we can start with conservative treatment that includes supportive footwear and ankle orthosis. Surgical treatment is advised when conservative treatment is ineffective.
