Pacreu et al.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> in REDAR, which discusses anaesthetic considerations in Deep Brain Stimulation (DBS) electrode placement for anorexia surgery. DBS is a minimally invasive intervention that involves placing electrodes connected to a pulse generator (usually located in the subcutaneous pectoral area) in order to electrically stimulate the deep structures of the brain. After proving effective in Parkinson's disease, its use has extended to other movement disorders (tremors, tics, and dystonia), psychiatric diseases (major depression, obsessive-compulsive disorder, or anorexia), chronic pain, and refractory epilepsy. So far, more than 160,000 patients worldwide have undergone DBS surgery, and the number increases every year.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In Spain, DBS has gained widespread acceptance — the procedure is performed by 30 hospitals and around 400 patients per year are treated (Spanish Society of Functional and Stereotactic Neurosurgery, November 2021). The increase in the number of indications for DBS has increased the likelihood that a patient scheduled for a diagnostic or therapeutic intervention, not necessarily neurosurgical, in other hospitals will be carrying a DBS stimulation device. In such cases, therefore, anaesthesiologists will need to consider not only the patient’s underlying disease and specific medication, but also certain factors related to this device.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Management of DBS devices is similar to that of a cardiac pacemaker or implantable cardioverter-defibrillator. Electrical fields can cause the electrodes to over-heat or overstimulate, which can damage brain tissue or cause the device to malfunction. For this reason, diathermy, transcranial magnetic stimulation, and electrocautery have traditionally been contraindicated. Bipolar electrocautery should be used with caution and monopolar electrocautery should be avoided. If electrocautery is unavoidable, guidelines recommend using the lowest voltage possible and placing the ground connection as far as possible from the generator and its electrodes. Alternatively, a pulsed radiofrequency system, such as the Peak PlasmaBlade™ (Medtronic plc) can be used.</p><p id="par0015" class="elsevierStylePara elsevierViewall">There is scant mention in the literature of the use of external defibrillators or cardioversion in these patients, and their consequences. These procedures are known to potentially damage cardiac pacemaker generators, and it would therefore be advisable to follow the same safety measures taken in patients with pacemakers, i.e., place the patches or pads away from the generator, use the lowest possible electrical charge and, after the procedure, check that the DBS system is working correctly.</p><p id="par0020" class="elsevierStylePara elsevierViewall">With regard to electroconvulsive therapy (ECT), a review by Peroski et al.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> concludes that this therapy is feasible and safe, provided the patients have been carefully selected and all the necessary safety measures taken. Intraoperative monitoring with transcranial motor evoked potentials (TcMEP) has also been questioned. A recent case report<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> describes the successful use of TcMEP in a patient with DBS due to dystonia whose device was deactivated during surgery. After restarting DBS after surgery, the device settings and impedance of the electrode leads were found to be unchanged.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Magnetic resonance imaging (MRI), an important diagnostic tool, deserves special consideration because it is also used to monitor DBS after electrode implantation, assess disease progression, and identify potential complications. The literature includes reports of over-heating of the DBS device, induced currents, generator dysfunction, and movements induced by the magnetic field of the device. DBS systems that are compatible with standard MRI were launched in 2015. These devices incorporate capacitors that dissipate the energy emitted by the MRI and allow the DBS system to remain active during the MRI scan. Good practice guidelines for MRI in patients with DBS devices, including ultra-high field MR and functional MR, have recently been published.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Generally speaking, both general and regional anaesthesia and nerve blocks are considered safe in these patients. It is important to bear in mind that DBS does not resolve the underlying disease and, therefore, anaesthesiologists must take into consideration both the risks and side effects of the patient’s background medication and their potential interaction with anaesthetic agents and other drugs administered in the perioperative period. As far as possible, management of the DBS device should be coordinated with the neurosurgery or neurology team. The need to turn off the device and the potential consequences of this should be discussed. It is best to continue and/or adapt the patient's chronic medication for as long as possible, and to restart them as soon as possible after surgery.
