We administered combined spinal-epidural anaesthesia using the needle through needle technique. The epidural catheter was inserted using a Tuohy needle and the loss of resistance to air technique. Initially, 3ml of air was injected, with a further 2ml in a second attempt. The puncture was performed at the level of L4–L5, with the patient in sedestation. In the immediate postoperative period, the patient presented an episode of sudden dyspnoea and hypotension. We confirmed our suspicion of pulmonary thromboembolism with a CT scan. Revista Española de Anestesiología y Reanimación (REDAR) -the Spanish Journal of Anesthesiology and Resuscitation- is the scientific body of the Spanish Society of Anesthesiology, Resuscitation and Pain Therapy. This is a monthly journal (10 issues) that publishes scientific articles of all areas within its specialty: clinical anesthesia, resuscitation-intensive medicine and critical care, diagnosis and treatment of acute and chronic pain, urgent care and emergencies and studies on basic science and related fields. The journal accepts work in both Spanish and English.
