Objetivo

Determinar la variación de masa ósea en dos versiones del mismo modelo de vástago femoral no cementado con objeto de cuantificar las diferencias atribuibles a un rediseño del implante con anclaje eminentemente metafisario.

Material y método

Estudio prospectivo, controlado, en el que se analizan densitométricamente, durante un año, dos grupos de 36 pacientes afectos de coxartrosis primaria, con características biológicas semejantes, distribuidos según el equipo quirúrgico que llevó a cabo la intervención. Cada equipo utilizó uno de los implantes.

Resultados

Las modificaciones introducidas en el vástago ABG-II consiguieron un patrón de remodelado óseo con mayor conservación del capital óseo en todas las zonas estudiadas. Las cifras de pérdida ósea son semejantes a las obtenidas con implantes hechos a medida.

Conclusiones

El vástago ABG-II, de anclaje estrictamente metafisario, consigue un adecuado anclaje capaz de transmitir cargas al fémur proximal y minimizar la pérdida ósea por puenteo de fuerzas (stress shielding).

Objective

To determine the variation in bone mass with two versions of the same model of uncemented femoral stem, for the purpose of quantifying differences attributable to redesigning the implant with metaphyseal fixation.

Materials and methods

Prospective, controlled study with densitometric analysis over a 1-year period of two groups of 36 patients with primary osteoarthritis. The biological characteristics of the patients were similar and they were distributed according to the surgical team that carried out the operation. Each team used one of two implants.

Results

The modifications introduced in the ABG-II stem improved the bone remodelling pattern, resulting in better conservation of the bone stock in all the zones studied. The figures for bone loss were similar to those obtained with customized implants.

Conclusions

The ABG-II stem with strictly metaphyseal fixation achieved adequate stability, transmitted loads to the proximal femur, and minimized bone loss by stress shielding.
