Mantle cell lymphoma usually arise in lymphatic nodes while extranodal involvement is very uncommon.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">1,2</span></a> We report a case of a 74-year-old female with history of rapidly growing tumor in the palpebral portion of the right lacrimal gland displacing the eyeball ventromedially. Craneal MRI (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A) demonstrated two bilateral intraorbital solid expansive tumors with similar homogeneous radiographic appearance, located in the upper outer quadrant of the orbit in the region of the lacrimal gland. A staging <span class="elsevierStyleSup">18</span>F-FDG PET/CT demonstrated a moderately increased uptake in both lacrimal gland (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B) with measured SUVmax 6.7 and 5.5 in the right and left lacrimal gland respectively, as well as supradiaphragmatic FDG-avid adenopathies in cervical and axillary regions (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>C and D) up to 1.7<span class="elsevierStyleHsp" style=""></span>cm in greatest dimension and SUVmax values up to 6.6 (mediastinal mean SUV 1.5), corresponding to stage IV. Pathological study of lacrimal gland (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>E) showed a diffuse lymphoid infiltration of small cell rounded nucleus, with CD20+/CD43+/CD5+ phenotype, cyclin D1-positive, and IGH-CCND1 translocation, consistent with mantle cell lymphoma with low proliferative index (Ki67<5%). The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
