The images of MRI (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>) demonstrated a mass of low signal on T1WI and high signal on T2WI with homogeneous enhancement in corpus callosum and the left temporal lobe, and a cavity of long T1 and T2 signal without enhancement in the left parietal lobe (it was considered as postoperative change), the recurrence of central nervous system (CNS) lymphoma was suspected. Then the patient underwent PET/CT to reveal whether there were any other lesions. The <span class="elsevierStyleSup">18</span>F-FDG PET/CT (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A–G) images revealed a mass in the in corpus callosum and the left temporal lobe with the SUVmax of 34.73, while the SUVmax of background parenchymal is 5.94, the tumour-to-background ratio (TBR) was 5.85, a low-density shadow in the left parietal lobe without FDG uptake. Two days later, the Al<span class="elsevierStyleSup">18</span>F-NOTA-FAPI-04 PET/CT (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>H–N) was performed. The images showed a mass with the SUVmax of 2.4 in the same location mentioned above, while the SUVmax of background parenchymal was 0.33, the TBR was 7.27, similar to the FDG.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Primary central nervous system lymphoma is a rare disease. FDG PET/CT is widely used to diagnose and evaluate the response of lymphomas<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. The newly developed radionuclide imaging agent Al<span class="elsevierStyleSup">18</span>F-NOTA-FAPI-04 specifically targets fibroblast activation protein, which is over-expressed in cancer-associated fibroblasts<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>. Several common tumor entities, such as esophageal cancer, breast cancer, lung cancer, present with a remarkably high uptake<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>. There is nearly no uptake of Al<span class="elsevierStyleSup">18</span>F-NOTA-FAPI-04 in normal cerebrum. Our case demonstrated that lymphoma also was detected by Al<span class="elsevierStyleSup">18</span>F-NOTA-FAPI-04 PET/CT with low intracranial background. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
