No other active lesions suggestive of pathology were observed in the remainder of the study.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">A 63-year-old patient diagnosed with rectal cancer (T3N0M0) in 2002 underwent abdominoperineal resection, radiotherapy and chemotherapy which was completed 6 months later, achieving complete remission.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In July 2011 the patient was seen in the Medical Oncology Department for acute pain in the perianal region. Pelvic MR was performed showing occupation of the presacral space by hypodense tissue of cicatricial appearance which did not allow the presence of disease to be ruled out.</p><p id="par0015" class="elsevierStylePara elsevierViewall">On suspicion of tumor recurrence a PET/CT study was performed showing post-surgical changes in the presacral region with normal glycidic metabolism not suggestive of disease persistence or recurrence. However, a moderate patchy increase of <span class="elsevierStyleSup">18</span>F-FDG was observed in the epiphysis and proximal third of the right femoral diaphysis which was suggestive of Paget's disease in the CT study (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">Posteriorly, a scintigraphic study with <span class="elsevierStyleSup">99m</span>Tc-HDP was carried out with the aim of confirming the monostotic nature, only observing intense-moderate reinforcement of uptake in the epiphysis and proximal third of the right femoral diaphysis (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">The utility of <span class="elsevierStyleSup">18</span>F-FDG PET/CT in the detection of residual and/or tumoral recurrence (sensitivity and specificity of 76%) as well as the localization of distant metastasis (sensitivity 91% and specificity of 76%) is well established.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Malignant bone lesions generally present a greater increase in the metabolism of FDG than benign tumors, albeit with considerable metabolic overlapping between the two lesions.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Different studies in the literature have defined the cut off of the maximum SUV (<span class="elsevierStyleItalic">standardized uptake value</span>) to allow differentiation between benign and malignant bone lesions. However, the results obtained differ. The studies of Shin et al. (<span class="elsevierStyleItalic">cut-off</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3.4; sensitivity: 91% and specificity: 78%), Schulte et al. (<span class="elsevierStyleItalic">cut-off</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>3.0; sensitivity: 93% and specificity: 66.7%) and Tian et al. (<span class="elsevierStyleItalic">cut-off</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2.5; sensitivity: 96% and specificity: 44%) are of note.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Paget's disease is characterized by an alteration in bone remodeling due to an imbalance between production and reabsorption. It may affect most of the bones but usually involves the pelvis, vertebral bodies, skull, femur and tibia. Bone scintigraphy presents a greater sensitivity than plane radiography, being useful in the determination of disease extension. However, PET/CT study with <span class="elsevierStyleSup">18</span>F-FDG presents greater spatial resolution and signal noise than bisphosphonates.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,4</span></a> In addition, recent studies have described the utility of <span class="elsevierStyleSup">18</span>F-FDG PET/CT in the monitoring of treatment with bisphosphonates in patients with Paget's disease, especially in the monostotic form,<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> as well as in the diagnosis of bone malignancy and localization of extension of malignancy.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Some studies have concluded that Paget's disease is not associated with a significant increase in glycidic metabolism in the PET study. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
