She was treated with oral antibiotics and extraction of fractured tooth by her dentist. As symptoms persisted she went to the hospital, and was diagnosed of facial cellulitis. Subsequently she started intravenous antibiotic treatment.</p><p id="par0010" class="elsevierStylePara elsevierViewall">A CT was performed showing an ulcerated lytic lesion with radiological criteria of aggressiveness in the right maxilla and pathological right cervical nodes suggestive of infiltrative neoplasia. The incisional biopsy was not diagnostic. It was repeated several times without satisfactory results. The last CT performed showed signs of progression of the lytic lesion, therefore an aggressive tumor was suspected. In the absence of diagnosis, PET/CT with <span class="elsevierStyleSup">18</span>F-FDG was performed. It showed a hypermetabolic mass in the right maxilla that was infiltrating soft tissues around. In addition a metabolic right pulmonary nodule and bilateral hilar and mediastinal lymphatic involvement were identified (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1A and 2A</a>). The area of the highest activity was indicated as an adequate biopsy site. Subsequently biopsy was repeated, with a diagnosis of myofibroblastic inflammatory tumor (MIT). Then, she started treatment with intravenous and topical corticosteroids. Two weeks later, she presented significant clinical improvement. PET/CT with <span class="elsevierStyleSup">18</span>F-FDG was performed four months later, showing a complete metabolic response (<a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1B and 2B</a>). Currently (three years later of the diagnosis of MIT) the patient is on remission.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">MIT, also called inflammatory pseudotumor, is a rare entity, considered a low grade neoplasia. It rarely affects the head and neck region, constituting 14–18% of the extrapulmonary locations,<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> the majority being located in orbits and upper airway. Although they are biologically controversial entities, most patients have a good prognosis and can be heal with resection.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> Clinical manifestations are variable including constitutional symptoms, locally aggressive growth with as well as vascular invasion, being able to recur resort locally and present malignant transformation in some cases.</p><p id="par0020" class="elsevierStylePara elsevierViewall">There is almost no information in the literature of head and neck MIT evaluated with <span class="elsevierStyleSup">18</span>F-FDG PET. MIT is an entity that must be included in the differential diagnosis of space occupying lesions (SOL), especially if it is a solitary mass. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
