Malignant findings are indicated with dashed yellow arrows for peritoneal localizations (d,e,h) and hollow yellow arrow for the right iliac bone metastases (e,l).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Peritoneal carcinomatosis is a very rare occurrence of metastasis in prostate cancer (PCa), with only few case reports documented in literature.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–3</span></a> One of the main determinants for this unusual PCa manifestation is the peritoneal disruption during radical prostatectomy. Due to its very subtle nature, this sort of cancer infiltration can be very difficult to detect and frequently the diagnosis is possible for higher levels of PSA. The advent of Prostate Specific Membrane Antigen (PSMA) PET/CT and the outstanding performance in documenting PCa foci could allow for the prompt detection of peritoneal localizations, commonly invisible for other modalities.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Herein are illustrated the imaging findings of a 52-year-old male affected by PCa (Gleason Score 5 + 5; TNM pT4N1) diagnosed in 2016 and initially treated with radical prostatectomy plus androgen deprivation therapy. After several lines of therapy, including chemotherapy with docetaxel, the patient presented in November 2019 with biochemical progression during leuprolelin acetate therapy. Consequently, he was referred to restaging [<span class="elsevierStyleSup">11</span>C]Choline PET/CT (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>: a,b,f,I), performed with a trigger PSA of 1.45 ng/mL and a PSA doubling time (PSAdt) of 2 months. The examination showed no clear signs of disease localization. Given the high suspicion of occult metastases, a [<span class="elsevierStyleSup">68</span>Ga]PSMA PET/TC was required and performed two weeks later (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>: d,h,l,e). Notwithstanding, the scan showed a diffuse malignant tracer accumulation consistent with omental infiltration (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>: d,e,h) associated with a high focal uptake at the right iliac bone (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>: e,l). If we exclude the mild omental thickening present on localization CT images (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>: c,g,k), no other signs of metastases could be retrieved. A posteriori, in the previous [<span class="elsevierStyleSup">11</span>C]Choline PET/CT scan, a very subtle uptake at the level of the peritoneum in the right flank (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>: f) and hypochondriac region (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>: b) could be noticed. Following the examination, the patient started complaining abdominal discomfort mainly referred at the upper quadrants, which clinically confirmed the peritoneal disease infiltration. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
