He did a <span class="elsevierStyleSup">68</span>Ga-PSMA PET/CT due to BCR [PSA 4.3<span class="elsevierStyleHsp" style=""></span>ng/mL; PSA doubling time (PSADT) 5.5 months]. Maximum intensity projection (MIP) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A) revealed abnormally high <span class="elsevierStyleSup">68</span>Ga-PSMA expression in a thyroid nodule located in the left lobe with maximum standardized uptake value (SUVmax) 14 (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>C) and in several cervical and mediastinal lymph nodes with SUVmax 14 (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>B and D). Thyroid nodule and cervical lymph nodes were submitted to ultrasonography guided fine needle aspiration cytology. Histology demonstrated adenocarcinoma with morphology and immunochemistry compatible with PCa. Immunochemistry analysis revealed cells with diffuse and intense androgen receptors, PSMA and PSA expression (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), but without thyroglobulin (TG), thyroid transcription factor 1 (TTF-1) and cytokeratin 7 (CK7) expression. The disease was classified as stage IV and systemic therapy with abiraterone and prednisolone was added.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Patient 2 (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>) was 58 years old, he had PCa Gleason 7 (4<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3) that had been submitted to surgery (T3bN0R1) 3 years before and salvage stereotactic body radiation therapy (SBRT) 5 month later. <span class="elsevierStyleSup">68</span>Ga-PSMA PET/CT was done because of BCR (PSA 1.82<span class="elsevierStyleHsp" style=""></span>ng/mL; PSADT 6 months). MIP (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>A) showed mild abnormal <span class="elsevierStyleSup">68</span>Ga-PSMA uptake in a left thyroid lobe nodule (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>B) with SUVmax 6 and some milimetric mediastinal and hilar lymph nodes (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>C and D) with SUVmax 4. Thyroid cytology presented erythrocytes and colloid, without neoplastic cells. Immunochemistry showed follicular cells with CK7, TG and TTF-1 expression, but without PSA or PSMA expression.
