Resumen
Introducción

El sarcoma del estroma endometrial (SEE) es un tumor infrecuente que contabiliza menos del 10% de los sarcomas uterinos.

Pacientes y métodos

Presentamos el caso de una mujer de 44 años con masas uterinas detectadas mediante ecografía a la que se realizó una histerectomía con conservación de anejos.

Resultados

Microscópicamente, uno de dichos nódulos estaba constituido por una densa proliferación maligna de células estromales del endometrio y focos de células espumosas. El estudio inmunohistoquímico mostró positividad para vimentina, CD10, receptores hormonales y negatividad para actina y desmina.

Discusión y conclusiones

El CD10 es un marcador inmunohistoquímico del estroma endometrial normal y de los tumores del estroma endometrial, y es útil para el diagnóstico diferencial entre el SEE y el leiomioma celular o el leiomiosarcoma uterino. Revisamos la nueva clasificación de la Organización Mundial de la Salud del SEE y sus opciones terapéuticas.

Summary
Introduction

Endometrial stromal sarcoma (ESS) is an uncommon neoplasm which accounts for less than 10% of all uterine sarcomas.

Patients and methods

We report a case of a 44 year-old woman with uterine masses revealed by ultrasonography, who had undergone hysterectomy without adnexectomy.

Results

Microscopically the neoplasm was composed of malignant stromal elements and foci of foamy cells. The immunohistochemical study showed immunorreactivity for vimentin, CD10, estrogen receptor and progesterone receptor but it was negative for actin and desmin.

Discussion and conclusions

CD10 is an immunohistochemical marker of normal endometrial stroma and of endometrial stromal neoplasms. This marker is useful in the differential diagnosis of ESS versus uterine cellular leiomyoma or uterine leiomyosarcoma. The new World Health Organization classification of ESS and the more recent treatments are reviewed.
