Summary
Objective

To investigate the incidence and clinical and epidemiological characteristics of Haemophilus influenzae invasive disease (HIID) in the population older than 5 years of age in Catalonia.

Methods

A prospective study was carried out during 1996 and 1997. All microbiology laboratories were contacted monthly in order to detect isolates from normally sterile sites.

The serotype, age, sex, clinical form and evolution of each case detected were obtained.

Results

A total of 154 cases in all age groups were detected, of which 106 (68.8%) corresponded to the population older than 5 years; 29 cases (27.3%) were caused by capsulate strains (24 were serotype b, 4 serotype f and 1 serotype e) and 77 (72.7%) by non-capsulate ones. In this population, the incidence rate of H. influenceinvasive disease was 0.9 per 100,000 persons-year and the case fatality rate 14.2%, with pneumonia being the most frequent clinical manifestation (45.3%). In subjects ? 65 years, the incidence rate was 2.7 per 100,000, with the rate for non-capsulate strains being 2.2 per 100,000.

Conclusions

The incidence of H. influenzaeinvasive disease in the population older than 5 years of age in Catalonia was low, although in the population older than 65 years this incidence was almost three times higher and mainly caused by non-capsulate strains.

Resumen
Objetivo

Investigar la incidencia y las características clínicas y epidemiológicas de enfermedad invasiva por Haemophilus influenzaeen la población mayor de 5 años de edad en Cataluña.

Métodos

Se realizó un estudio prospectivo durante los años 1996-1997. Mensualmente se contactó con todos los laboratorios para detectar aislados de territorios normalmente estériles.

Resultados

Se detectaron un total de 154 casos en todos los grupos de edad, de los que 106 (68,8%) correspondieron a población mayor de 5 años; 29 casos (27,3%) estuvieron causados por cepas capsuladas (24 fueron del serotipo b, 4 del serotipo f y una del serotipo e) y 77 (72,7%) por cepas no capsuladas. En esta población la tasa de incidencia de enfermedad invasiva por H. influenzaefue de 0,9 por 100.000 personas-año y la mortalidad del 14,2%, siendo la pneumonía la manifestación clínica más frecuente (45,3%). En sujetos ? 65 años, la tasa de incidencia fue de 2,7 por 100.000 personas-año, siendo la tasa para las cepas no capsuladas de 2,2 por 100.000.

Conclusiones

La incidencia de enfermedad invasiva por H. influenzaeen la población mayor de 5 años en Cataluña fue baja, aunque en la población mayor de 65 años la incidencia fue casi 3 veces superior y estuvo causada principalmente por cepas no capsuladas.
