Utility of the hospital discharge basic minimun data set in the detection of cases of Haemophilus influenzae invasive disease

Objetivo

Investigar la validez del conjunto mínimo básicos de altas hospitalarias para la detección de los casos confirmados de enfermedad invasiva por Haemophilus influenzae (EIHI) de los hospitales de Cataluña.

Métodos

Se compararon dos fuentes de datos. Los diagnósticos del conjunto mínimo de datos básicos de altas hospitalarias correspondientes a infecciones invasivas y los aislamientos de H. influenzae detectados mediante vigilancia activa de los laboratorios. Para cada caso de EIHI se determinaron la forma clínica, la edad, la estancia hospitalaria y el tamaño del hospital. Se realizó un análisis de regresión logística para determinar la asociación entre la variable dependiente, caso confirmado de EIHI, y las restantes variables.

Resultados

La sensibilidad del conjunto mínimo de datos básicos de altas hospitalarias para la EIHI fue del 46,1% (38,5-54,3) y su valor predictivo del 13,1% (10,3-16,0), aunque se observaron variaciones importantes respecto a la forma clínica. La edad (OR = 3,1; 1,4-6,7) y el tamaño del hospital (OR = 2,7; 1,1-33,1) fueron las únicas variables asociadas con la detección de los casos confirmados de EIHI.

Conclusiones

El conjunto mínimo de datos básicos de altas hospitalarias no es un instrumento eficiente para los estudios epidemiológicos de EIHI. Para las enfermedades de infecciones que son objeto de vigilancia y para las que exista una definición de caso, la codificación de las altas hospitalarias debería hacerse utilizando dicha definición.

Objective

To investigate the validity of the hospital discharge basic minimum data set in detecting confirmed cases of Haemophilus influenzae invasive disease (HIID) from hospitals of Catalonia.

Methods

Two sources of data were compared: hospital discharge basic minimum data set diagnoses with codes corresponding to invasive infection and isolations of H. influenzae detected by means of laboratory active surveillance. For each case of HIID, the clinical form, the age, the hospital stay and the size of the hospital were determined. A logistical regression analysis was carried out to determine the association between the dependent variable confirmed case of HIID and the other variables.

Results

The sensitivity of the hospital discharge basic minimum data set for HIID was 46.1% (38.5-54.3), and its predictive value 13.1% (10.3-16.0) although important variations were observed according to the clinical form. Age (OR = 3.1; 1.4-6.7) and hospital size (OR = 2.7; 1.1-33.1) were the only variables associated to the detection of confirmed cases of HIID.

Conclusions

The hospital discharge basic minimum data set is not an efficient instrument for epidemiologic studies of HIID. For infectious diseases which are the object of surveillance, coding of the hospital discharge should be carried out using the accepted case definition.
