This process takes place in the floor of the primitive pharynx, where later will be the foramen caecum. The gland starts the descent during the 5th week of gestation, from the base of the tongue to the anterior compartment of the neck, reaching its definitive location by the 7th week.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">1</span></a> The thyroid gland anlage descent does not form a “duct-like structure” but rather an island of cells during its descent. If it formed a real duct, the presence of cyst would be illogical. According to published data analyzing 78 embryos, the thyroglossal duct was not identified as a continuous duct-like structure but as a small sheet or mass of relatively large cells forming multiple lumina.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">2</span></a> Occasionally, during this course, developmental alterations occur and can lead to the formation of a thyroglossal duct cyst (TDC). TDC accounts for 75% of midline neck swellings in children. Approximately 7% of the general population has a TDC which fails to involute, with more than 50% of these containing ectopic functional thyroid tissue.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The incidence of TDC carcinoma (TDCC) is low and it represents 1–2% of all TDC.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">1,4</span></a> This rare cancer was first reported in 1911 by Bertano.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">5</span></a> Since then, the medical literature contains multiple case reports of TDCC and small case series. In most cases, the diagnosis is made on histopathological analysis of the specimen after TDC excision with conventional Sistrunk procedure. Accordingly, the need for further treatment with total thyroidectomy, neck dissection, radioactive iodine (RAI) remnant ablation or adjuvant radiotherapy remains controversial.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study is to review the clinical presentation, diagnosis, pathology and management strategies in a modern cohort of patients with TDC carcinoma. We also review the previous published data with the objective to provide evidence for best diagnostic and treatment strategies.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This is a retrospective case series conducted at the Thyroid Oncology Unit at Guy's and St Thomas’ Hospital NHS Foundation Trust between 1998 and 2021 in accordance with the Preferred Reporting Of CasE Series in Surgery (PROCESS) Guidelines. All data was routinely recorded contemporaneously and analysed retrospectively to evaluate the management and outcomes of patients with TDCC. Data recorded included: gender, age at diagnosis, clinical presentation, thyroid function, diagnostic investigations, cytological results, final histology and staging for all patients. All patients were originally investigated with thyroid function tests and ultrasound guided fine needle aspiration cytology (FNAC). Either CT or MRI were also requested in selected cases when there was either suspicion of extension to the thyroid gland or nodal metastases. We stratified the risk of malignancy in cytological analysis according to the classification proposed by the Royal College of Pathologists in United Kingdom.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">6</span></a> From 2002, all patients were discussed in the multidisciplinary thyroid oncology meeting where management recommendations were agreed. From 1998 to 2002, the 5th version of the AJCC<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">7</span></a> was used to report histological specimens, the 6th version<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">8</span></a> was used from 2003 to 2009 and the 7th edition was used thereafter.<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">9</span></a> In those patients with initial indeterminate or benign cytological diagnosis, the TDC was removed in a first stage via Sistrunk's procedure (with preservation of the thyroid gland). When the pathological report showed the presence of malignancy, a second operation was advised to remove the entirety of the thyroid gland. Neck dissection was performed according to the results of the imaging and cytological tests previously performed when those confirmed the presence of malignancy. Patients treated with total thyroidectomy received postoperative radioiodine treatment and thyroid-stimulating hormone suppression according to our institution's protocol. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the main surgical procedure steps.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">Using the NHS Health Research Authority decision tool this project was determined to be a service evaluation thereby not requiring ethical approval The project was approved with reference number 13048 at Guy's and St Thomas’ NHS Foundation Trust. Principles of Good Clinical Practice were adhered to throughout the study.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0030" class="elsevierStylePara elsevierViewall">A total of 14 patients were diagnosed with TDCC over the studied period. Two patients were excluded from further analysis; one declined any treatment (asymptomatic at last follow up 3 years after cytological TDCC diagnosis) while the other had an uncommon histopathological diagnosis previously reported.<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">10</span></a> Twelve patients with the diagnosis of TDCC were finally included. There were 6 males and 6 females with a mean age at diagnosis of 46 (31–66) years. Patient demographics, initial presenting symptoms, tumour stage, surgical management, radioactive iodine dose and TSH suppression were extracted for all patients (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Of note, the majority of patient (66.7%) presented with an isolated TDC and no other symptoms. The true incidence of TDCC could not be estimated from this sample due to the centralization of cancer treatment in the region were the study was conducted.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">All patients were first evaluated with FNAC. With only 4 patients with preoperative cytological suspicion of TDCC, the sensitivity of this test was 33.3%. One patient had suspected thyroid cancer on cytology (Thy5) of both the thyroid gland and lateral neck lymph node. At the time of presentation, thyroid function tests were also performed and all patients were euthyroid.</p><p id="par0040" class="elsevierStylePara elsevierViewall">All patients were treated surgically. A two-stage approach was carried out in 7 patients (58.3%). Following diagnosis of malignancy, a total thyroidectomy was performed in 10 patients (83.3%), while 2 (16.7%) were treated with Sistrunk's procedure only due to the small size of the TDCC (<10<span class="elsevierStyleHsp" style=""></span>mm). <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the preoperative cytological analysis and histological report of every patient. Among the 10 patients who underwent total thyroidectomy, 7 (70%) patients had proven carcinoma in the thyroid gland, 3 of them with deposits of less than 10<span class="elsevierStyleHsp" style=""></span>mm. The average size of the thyroid cancer deposits was 7.2<span class="elsevierStyleHsp" style=""></span>mm (1–20<span class="elsevierStyleHsp" style=""></span>mm). Only 4 (33%) patients had lateral neck nodal involvement, diagnosed either during neck dissection (3) or subsequently following disease recurrence in the neck (1). With a mean follow-up of is 44 months (5–120), all patients were alive and free of recurrence at the end of the study period.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Epidemiology and preoperative work up</span><p id="par0045" class="elsevierStylePara elsevierViewall">TDC is the most common congenital anomaly of the thyroid gland.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">3</span></a> However, TDCC is a rare clinical condition with papillary thyroid cancer representing over 90% of the malignancies in the TDC<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">11,12</span></a> with other malignancies anecdotally reported including adenosquamous carcinoma<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">13,14</span></a> or mucoepidermoid carcinoma.<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">10</span></a> Two theories try to explain the origin of the TDCC. The first theory suggests that PTC is a mutifocal disease - therefore justifying synchronous presence of cancer cells in the TDC and the thyroid gland.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">15</span></a> The other theory suggests the development of TDCC de novo from thyroid tissue present in the cyst.<a class="elsevierStyleCrossRef" href="#bib0013"><span class="elsevierStyleSup">16</span></a> Current literature seems to agree in defining thyroid-like TDCC as a neoplasm arising de novo in a TDC, and not as a metastasis from an underlying primary lesion of the thyroid gland.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">17</span></a> Such a ‘de novo hypothesis’ has been indirectly confirmed by Rossi et al.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">18</span></a> who demonstrated the lack of both morphological and molecular concordance between TDCC and concomitant thyroid cancers, when present. To further justify this second hypothesis, thyroid tumours identified by post mortem evaluation on the general population have been reported to be around 35%<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">19</span></a> and similar percentages have been described for patient with TDCC. However, in our series the reported incidence of thyroid malignancy was 58.3% while Hartl et al. reported 56%<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">20</span></a> and Bakkar et al., 62%.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">21</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">In this series, all patients are adults over the age of 30. Paediatric patients are uncommonly affected by TDCC.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">22</span></a> Accordingly, the development of a TDC in an adult should raise the suspicion of a malignant cyst. Other preoperative signs suspicious for malignancy are a growing, firm, solid nodule with irregular borders and attached to surrounding structures sometimes causing pain and dysphagia. Concurrent lymph node swelling can add to further clinical suspicion. However, TDCC normally presents as a midline neck lump showing no other clinical signs of malignancy with more than 75% of cases diagnosed incidentally after histological analysis.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">13</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">Current standard investigations for TDC patients include thyroid function tests (to confirm normal thyroid function) and ultrasonography (US) of the neck. US is helpful in localising the cyst and can identify suspicious solid components (aided by Doppler settings) to be targeted for FNA.<a class="elsevierStyleCrossRef" href="#bib0016"><span class="elsevierStyleSup">23</span></a>common congenital anoma It can also evaluate the thyroid to confirm a eutopic location and identify any additional intrathyroidal lesions. Finally, it is helpful in delineating nodal involvement of the lateral neck compartment.</p><p id="par0060" class="elsevierStylePara elsevierViewall">According to the data that this and previous studies provide, FNAC should be performed when the diagnosis of a TDC is made in patients over 25 years of age when surgery is advised due to the risk of malignancy being higher among adult patients, as well as the low cost and high accuracy reported for this test.<a class="elsevierStyleCrossRefs" href="#bib0340"><span class="elsevierStyleSup">24,25</span></a> Although the cytological diagnosis of cancer from a TDC in our series is low, FNAC has shown values of sensitivity and specificity up to 100% in other studies, especially when performed with the cooperation of specialist radiologists and cytologists, directing their sample to the solid part of the lesion, usually located within its wall.<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">24</span></a> When malignancy is suspected, cross sectional imaging is necessary to assess the relationship of the TDCC and surrounding structures prior to any surgical intervention. Both contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) can provide useful anatomical data and help identify associated lymphadenopathy.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">26</span></a><a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> shows imaging of a patient with aggressive features.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Although no staging system has specifically addressed TDCC, it is presumed that with the tumour arising from the thin wall of a cystic mass, the risk for extra-cystic extension is greater than when the tumour originates within the thyroid gland. This has implications for staging as it can be hypothesised that the transition from T1/2 to T3/T4 would be easier in TDCC than the thyroid gland, but such implications have not yet been studied.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">TDCC management</span><p id="par0070" class="elsevierStylePara elsevierViewall">According to the embryological development of the TDC summarised above, the minimal surgical procedure to treat TDCC is a Sistrunk procedure.<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">27</span></a> When preoperative investigations confirm the presence of multifocal disease, total thyroidectomy is necessary. However, when the diagnosis of TDCC is incidental after TDC excision or when preoperative evaluation does not show any thyroid abnormalities, the need for total thyroidectomy is still a matter of debate. Arguments pro total thyroidectomy include better staging of the disease, possibility of radioactive iodine therapy and follow up facilitated with thyroglobulin levels and US. On the other hand, thyroidectomy adds additional surgical risks such as recurrent laryngeal nerve palsy and hypocalcemia as well as the need for thyroid replacement therapy. Either way, when the diagnosis of TDCC is confirmed, the risk of thyroid gland involvement should be discussed with the patient, and therefore, the possibility of having a total thyroidectomy during the same operation. Patel et al.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">28</span></a> in a review of 62 patients with TDCC demonstrated that the extent of the original surgical procedure was the only significant survival predictor and the addition of total thyroidectomy did not significantly affect the survival outcomes. It is therefore argued that in low-risk patients with an otherwise normal thyroid gland, total thyroidectomy in addition to Sistrunk's procedure may not be necessary.<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">28</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The management of thyroid cancer is shifting towards a more conservative approach. For example, the 8th edition of the AJCC staging system introduced some changes in order to identify the minority of stage III and IV patients that are at greater risk of death. These changes (age threshold up to 55, microscopic extrathyroid, nodal disease, etc.) translate into a down staging of approximately 1/3 of patients with thyroid cancer. Accordingly, Lancini et al.<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">27</span></a> on a recent review categorise into a low-risk group patients younger than 45 years, tumour size less than 1.5<span class="elsevierStyleHsp" style=""></span>cm without extracystic extension and no evidence of locoregional or distant disease and concluded that these patients can be treated with Sistrunk procedure alone. In other scenarios, thyroid surgery<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>neck dissection are recommended.</p><p id="par0080" class="elsevierStylePara elsevierViewall">When the diagnosis of TDCC can be established pre-operatively, the most appropriate approach is to treat these patients with a single staged procedure that includes a formal Sistrunk's adding total thyroidectomy and neck dissection when its involvement is suspected or proven. If the thyroid gland does not show any abnormalities, the selected procedure should be a formal Sistrunks's procedure unless the need for postoperative radioactive ablation is considered necessary due to locally invasive features such as the one in the case presented (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The utility of mutational markers in guiding preoperative surgical decision-making is a matter of interest. BRAFV600E is the most common genetic alteration in PTC, with a specificity of almost 100%, and has been linked to more aggressive features.<a class="elsevierStyleCrossRefs" href="#bib0355"><span class="elsevierStyleSup">29,30</span></a> Results from Bakkar et al.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">21</span></a> including 15 patients with TDCC PTC showed a correlation between BRAFV600E-positive lesions and more advanced stages (T3) and extracystic cancerous extension. On another cohort of patients, Rossi et al.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">18</span></a> identified 80% of synchronous TDCC and thyroid PTC in 80% of BRAFV600E-positive patients. Accordingly, BRAFV600E expression seems to be predictive of locally advanced and more aggressive disease.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The pattern of lymphatic spread in TDCC remains unknown. It is well recognised that PTC arising from the thyroid gland is associated with a high risk of level VI nodal involvement, but the same has not been demonstrated in TDCC. Indeed, in our series, there were only 3 patients (25%) with lateral neck involvement (N1b) and one case with level Ib involvement; while none had level VI disease. Accordingly, the need for level VI neck dissection during the management of TDCC is still controversial and we recommend its dissection only when there is clinical evidence of level VI invasion.</p><p id="par0095" class="elsevierStylePara elsevierViewall">In our institution, the reported results along with other institutions results<a class="elsevierStyleCrossRefs" href="#bib0345"><span class="elsevierStyleSup">27,28</span></a> have shifted the management of these patients towards a more conservative approach summarised in <a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>.</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">In those cases where only a Sistrunk's procedure has been performed, the indication for postoperative radioactive ablation should not differ from those existent after papillary thyroid cancer in the thyroid gland. When bad prognostic factors such as a more aggressive histology, extrathyroidal extension or perinerual or perivascular invasion, or when R1 or R2 resections are identified in histological analysis, the use of radioactive iodine is justified and therefore so is the need for a total thyroidectomy, in the same way a thyroid completion is performed after lobectomy for high-risk thyroid cancer.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Overall, papillary thyroid cancer arising from a TDC does not seem to carry a higher risk than when arising from the thyroid gland. Accordingly, the management of the differentiated thyroid cancer arising from TDC can be extrapolated from the management of those cancers arising from the thyroid gland. However, we need to take into consideration the facility of these cancers to show extra cystic invasion and the possibility of differing patterns of lymphatic spread.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Conclusion</span><p id="par0110" class="elsevierStylePara elsevierViewall">All adult patients undergoing surgery for TDC should be carefully investigated with ultrasound scan of the neck<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>FNAC. When TDCC is confirmed or suspected, its management should be discussed in the multidisciplinary meeting. The treatment depends on the result of the FNAC, clinical findings and radiological investigations. TDCC bears extraordinary survival rates. Accordingly, the management of these cancers has shifted towards a more conservative approach that although increases difficulties in follow up, avoids complications associated with thyroid surgery. However, its peculiarities must be taken into account: ease of extra cystic invasion and possible different lymph node invasion.</p></span><span id="sec0040" class="elsevierStyleSection The majority of patients (66.7%) presented with an isolated thyroglossal duct cyst. Only 4 patients had preoperative cytological suspicion of carcinoma (sensitivity: 33.3%). At the time of presentation all patients were euthyroid. Following diagnosis of malignancy, a total thyroidectomy was performed in all patients, with the exception of 2, who had a thyroglossal duct cyst carcinoma of less than 10<span class="elsevierStyleHsp" style=""></span>mm. Among the 10 patients who underwent total thyroidectomy, 7 (70%) patients had proven carcinoma in the thyroid gland, 3 with deposits of less than 10<span class="elsevierStyleHsp" style=""></span>mm. The average size of the thyroid cancer deposits was 7.2<span class="elsevierStyleHsp" style=""></span>mm (1–20<span class="elsevierStyleHsp" style=""></span>mm).
