Series de pacientes sin neuromonitorización.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introducción</span><p id="par0005" class="elsevierStylePara elsevierViewall">La parálisis laríngea unilateral es una complicación asociada a la tiroidectomía que globalmente se sitúa entre el 5-8% (transitoria) y el 1-3% (permanente)<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. Tras tiroidectomía total, la incidencia de lesión del nervio laríngeo recurrente (NLR), uni o bilateral, se presenta en torno al 4,3% (2,4% transitoria, 1,3% definitiva)<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">La incidencia de parálisis laríngea bilateral en tiroidectomía es rara<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a>, del 0,2 al 0,6%<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,5,6</span></a>, y es la causa más frecuente de parálisis bilateral<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>, a menudo por mecanismo de tracción<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">La parálisis laríngea bilateral tras la tiroidectomía es una de las preocupaciones mayores del cirujano ante un procedimiento bilateral y un incidente de seguridad grave que puede comprometer la vía aérea, precisar reintubación o traqueotomía y que puede conducir a secuelas graves para el paciente o la muerte<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,10</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">La incidencia de parálisis se relaciona con ciertas circunstancias como son la reintervención, el cáncer, la extensión de la cirugía, la experiencia del cirujano, la lateralidad (derecha versus izquierda), el bocio retrosternal, la enfermedad de Graves, la visualización y disección intraoperatoria del NLR, el empleo de la neuromonitorización intraoperatoria (NM) y la evaluación perioperatoria de la motilidad laríngea<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>. El riesgo de parálisis bilateral se incrementa en caso de parálisis contralateral previa<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>, la no identificación del NLR y la menor experiencia del cirujano<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Los criterios de Riddell para la prevención de la parálisis laríngea desde las primeras publicaciones sobre NM en tiroidectomía siguen vigentes: identificación visual del NLR en su relación con la arteria tiroidea inferior, comprobación funcional tras finalizar la intervención mediante estímulo eléctrico y laringoscopia de rutina pre y postoperatoria<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Los primeros estudios sobre la precisión de la NM aparecieron a comienzos de la década de los 90 del siglo <span class="elsevierStyleSmallCaps">XX</span> y las primeras referencias sobre su utilidad en la prevención de la parálisis bilateral a finales de la década<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a>.</p><p id="par0035" class="elsevierStylePara elsevierViewall">La NM ofrece información sobre la función del NLR al concluir la cirugía con una precisión superior al 95%<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16–18</span></a>. Esto depende de factores como el tipo de registro (con electrodo adherido al tubo endotraqueal, electrodos de aguja), el punto de aplicación del electrodo de estimulación (sobre el nervio o en su proximidad), tipo de estímulo aplicado (intermitente, continuo, intensidad), del nervio estimulado (NLR o vago), de la secuencia de los registros, de la experiencia del cirujano, de la hemostasia durante la cirugía<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> y de la laringoscopia pre y postoperatoria<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,20</span></a>.</p><p id="par0040" class="elsevierStylePara elsevierViewall">No hay resultados definitivos sobre la influencia de la NM en la prevención de la parálisis laríngea y parece no existir ventaja real respecto a la sola identificación visual del NLR aunque sí pudiera relacionarse con una menor incidencia de parálisis transitoria<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,21</span></a>.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Debido a su elevada precisión, la NM puede ayudar en la gestión de la vía aérea y evitar una parálisis laríngea bilateral al modificar la estrategia en el caso de pérdida de la señal tras la primera lobectomía en una tiroidectomía total programada<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,16</span></a>.</p><p id="par0050" class="elsevierStylePara elsevierViewall">El objetivo de este estudio está en estimar el riesgo de parálisis bilateral del NLR en tiroidectomía total realizada con y sin NM mediante revisión sistemática de la literatura y metaanálisis.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material y método</span><p id="par0055" class="elsevierStylePara elsevierViewall">Se realizó búsqueda bibliográfica en las bases de datos PubMed, Scopus (PubMed y Embase) y Cochrane Library de artículos publicados en el periodo 2000-2014, sin restricción de idioma. Se completó la búsqueda mediante revisión de las citas bibliográficas de los estudios identificados.</p><p id="par0060" class="elsevierStylePara elsevierViewall">La estrategia empleada en PubMed fue: («Thyroidectomy» [Mesh] OR «Thyroidectomy» OR «Thyroid Diseases/surgery» [Mesh]) AND («Recurrent Laryngeal Nerve/abnormalities» [Mesh] OR «Vocal Cord Paralysis» [Mesh] OR «Diagnostic Techniques, Neurological» [Mesh] OR «Vagus Nerve Stimulation» [Mesh] OR «Monitoring, Intraoperative/methods» [Mesh] OR ((«paralysis» OR «palsy» OR «paresis») AND («recurrent» OR «laryngeal»)))</p><p id="par0065" class="elsevierStylePara elsevierViewall">En Scopus y Cochrane se emplearon búsquedas similares simplificadas.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Se obtuvieron 2.932 artículos de los que finalmente se seleccionaron 40 para metaanálisis que incluyen 54 series de pacientes (25 sin NM, 29 con NM). En la <a class="elsevierStyleCrossRef" href="#fig0005">figura 1</a> se resume la selección de artículos mediante diagrama de flujo PRISMA<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Criterios de inclusión: publicaciones con series de casos, estudios retrospectivos de casos y control o ensayos clínicos con resultados sobre incidencia de parálisis laríngea tras tiroidectomía total realizadas en un solo tiempo por cervicotomía abierta sin o con neuromonitorización intermitente y evaluación de la motilidad laríngea con laringoscopia pre y postoperatoria.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Criterios de exclusión: casos clínicos, revisiones, artículos en los que no se especifica la técnica realizada, utilización exclusiva de NM continua, parálisis laríngea prequirúrgica, parálisis laríngeas por otras causas no relacionadas con la cirugía (cicatricial, radioterapia, radioyodo), por cirugía asociada (vascular), reintervenciones, totalizaciones, tiroidectomía parcial, abordaje torácico o tiroidectomía endoscópica, vídeoasistida o robótica.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Extracción de datos. De cada documento se extrajeron por duplicado las siguientes variables: autor, año, país investigador, revista, <span class="elsevierStyleItalic">impact factor</span>/SJR en 2012/H-index, editorial de la revista, departamento investigador, tipo de estudio (series no comparativas con solo identificación visual o solo NM o series comparativas con y sin NM), número de tiroidectomías totales y número de parálisis bilaterales.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Metodología estadística. Se realizaron estimaciones agrupadas de los resultados de los estudios con o sin NM y en función de la especialidad de los investigadores (Otorrinolaringología o Cirugía) y área geográfica (Unión Europea, Estados Unidos de América y otros). Se realizó un análisis restringido a los estudios que incluían series con y sin NM del mismo ámbito asistencial. Se estimaron proporciones asumiendo modelos de efectos aleatorios calculando I<span class="elsevierStyleSup">2</span> como estimador de heterogeneidad. Para el análisis estadístico, se utilizó el software MetaXL (EpiGear Internacional Ltd, Queensland, Australia. <a id="intr0005" class="elsevierStyleInterRef" href="http://www.epigear.com/">http://www.epigear.com/</a>). Los resultados se expresan con su intervalo de confianza con una precisión del 95% (IC 95%).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Resultados</span><p id="par0095" class="elsevierStylePara elsevierViewall">Se seleccionaron 40 artículos que contienen 54 series con 30.922 pacientes a los que se realizó tiroidectomía total (25 con solo identificación visual del NLR sin NM y 29 con empleo de NM). Quince estudios incluyen series de pacientes con y sin NM, solo 3 de ellos aleatorizados y en 24 estudios series solo con NM (14) o sin NM (10).</p><p id="par0100" class="elsevierStylePara elsevierViewall">En las <a class="elsevierStyleCrossRefs" href="#tbl0005">tablas 1 y 2</a> se presentan los recuentos y características de las series con y sin NM respectivamente. En la <a class="elsevierStyleCrossRef" href="#tbl0015">tabla 3</a> se presentan los análisis agrupados con y sin NM y por subgrupos: área geográfica, especialidad (ORL o cirugía) y existencia o no de comparación de series con y sin NM. Las series con NM resultaron homogéneas entre sí pero no las series sin NM. La ausencia de casos de parálisis bilateral en los estudios aleatorizados no permitió hacer comparaciones directas.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">La incidencia de parálisis laríngea bilateral global fue de 137 casos (4,4‰).</p><p id="par0110" class="elsevierStylePara elsevierViewall">La incidencia de parálisis bilateral en la serie de pacientes con NM resultó ser inferior respecto al grupo sin NM (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>27, 2,43‰, [1,55-3,5‰] versus n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>110, 5,18‰, [2,53-8,7‰] respectivamente). La diferencia observada equivale a una reducción absoluta del riesgo (RRA) de 2,75‰, que supone un número necesario de pacientes a tratar (NNT) de 364,13.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Aunque hay diferencias en las estimaciones por subgrupos (más riesgo en estudios en EEUU, ORL, no comparativos y no prospectivos), los intervalos de confianza son muy amplios, por lo que las diferencias no serían significativas.</p><p id="par0120" class="elsevierStylePara elsevierViewall">En las <a class="elsevierStyleCrossRefs" href="#fig0010">figuras 2 y 3</a> se presentan los <span class="elsevierStyleItalic">forest plot</span> de los metaanálisis con y sin NM respectivamente.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discusión</span><p id="par0125" class="elsevierStylePara elsevierViewall">La NM informa sobre el estado funcional del NLR al concluir la cirugía y es útil para evaluar el riesgo de lesión<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,23,24</span></a> pero la controversia de la utilidad de la NM en la prevención de la parálisis del NLR no está resuelta<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a><span class="elsevierStyleItalic">.</span> La identificación visual del NLR sigue siendo el patrón oro de la gestión del NLR durante la tiroidectomía y la NM no parece haber reducido la tasa de lesiones aunque sí puede ser de utilidad en la identificación del NLR especialmente en cirugía compleja<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,21,26,27</span></a> y sobre todo en caso de reintervención<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Hay sesgo de partida respecto a la correcta evaluación de la parálisis de NLR tras la tiroidectomía ya que no se realiza laringoscopia sistemáticamente en todos los centros, la sola evaluación de la clínica no es suficiente, los resultados de la laringoscopia pueden verse afectados por el método de laringoscopia con diferente sensibilidad y especificidad y no suelen publicarse los resultados desfavorables<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Tampoco está claro el nivel de utilización de la NM en la práctica. Francis indica que la tendencia es a un mayor uso entre los otorrinolaringólogos y cirujanos generales pero los resultados no son concluyentes, no se está registrando sistemáticamente su uso o no y no se sabe si la técnica está bien o mal aplicada<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a>. Parte de la diferencia observada en la incidencia de parálisis bilateral de NLR entre los departamentos o las diferencias geográficas pueden explicarse por la diferencia en la implantación de la técnica.</p><p id="par0140" class="elsevierStylePara elsevierViewall">La incidencia de parálisis laríngea bilateral es muy baja tanto en series con o sin NM (4,4‰ de las tiroidectomías totales de las series incluidas en este estudio) por lo que para obtener resultados estadísticamente significativos sería necesario realizar un ensayo con grandes muestras<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,23</span></a>. La revisión sistemática y metaanálisis pueden paliar en parte esta dificultad<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Los resultados obtenidos en nuestro estudio permiten deducir que se comunican menos casos de parálisis laríngea bilateral en las series de tiroidectomías en las que se utilizó NM junto a la identificación visual del NLR que en aquellas en las que solo se aplicó identificación visual. La diferencia observada equivale a una RRA de 2,75‰, lo que podría justificar la utilización de la NM. Parte de la diferencia observada podría deberse al uso de la NM, aunque al no haberse asignado aleatoriamente en la mayoría de las series y ante la ausencia de más información sobre las características de los pacientes y procedimientos, no podemos descartar que las diferencias observadas se deban a la inclusión de pacientes con diferente riesgo, diferencias en la experiencia de los cirujanos o a otros factores relacionados con los centros donde se usa o no la NM. También podría interpretarse que al ser el riesgo tan bajo con los casos disponibles hay imprecisión. Así ocurre en los 3 estudios aleatorizados en los que no se observan diferencias por no haber ninguna parálisis.</p><p id="par0150" class="elsevierStylePara elsevierViewall">¿A qué puede deberse la reducción de incidencia de parálisis bilateral en las series con NM intermitente? La NM permite establecer <span class="elsevierStyleItalic">feed back</span> entre el cirujano y la función del NLR evaluada<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> e incita cambios en la estrategia y en los hábitos de los cirujanos<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,30</span></a>.</p><p id="par0155" class="elsevierStylePara elsevierViewall">La pérdida de la señal de NM tras la primera lobectomía induce a tomar medidas de prevención en la lobectomía contralateral ante una pérdida de señal y posible parálisis en la primera que dependerán de la patología y la experiencia del cirujano<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> y se han propuesto varias opciones como posponer la segunda lobectomía hasta haber comprobado la motilidad laríngea con certeza, evidenciar si hubo recuperación intraoperatoria de la señal mediante comprobaciones sucesivas en el NLR y en el vago, comprobar mediante laringoscopia intraoperatoria la motilidad laríngea o completar la tiroidectomía haciendo una segunda lobectomía más conservadora<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6,17,32–36</span></a>. Probablemente la NM continua ofrezca mejores resultados al poder prevenir la lesión del NLR sobre todo por mecanismo de tracción<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Es necesario interpretar correctamente la señal de NM<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a>. En 1.274 tiroidectomías totales, Melin et al.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">36</span></a> comunican 6 parálisis bilaterales, de ellas 2 fueron debidas a falsos negativos (FN) durante la NM y en 4 hubo pérdida de señal en el primer lado operado. Deducimos que se hubieran evitado 4/6 de las parálisis (66,66%) y que la probabilidad de parálisis bilateral se incrementó 2 veces más cuando hubo pérdida de señal en la primera lobectomía (4/2). El riesgo de parálisis bilateral se incrementa si se produce parálisis en el primer lado operado<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23,36</span></a>.</p><p id="par0165" class="elsevierStylePara elsevierViewall">La parálisis bilateral puede asociarse a diversas situaciones de interpretación de la señal de NM: con FN bilateral, con FN en la primera lobectomía y verdadero positivo (VP) en la segunda, con VP en la primera lobectomía y continuación de la técnica (y resultado FN o VP en la segunda lobectomía)<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,29,31,33,37,38</span></a>.</p><p id="par0170" class="elsevierStylePara elsevierViewall">La situación FN es infrecuente, difícil de prever y catastrófica si es bilateral<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">36</span></a>. La obtención de señal final sobre el nervio vago (V2) puede reducir los FN<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,32</span></a>.</p><p id="par0175" class="elsevierStylePara elsevierViewall">La utilidad real de la NM está en la información que ofrece sobre la funcionalidad del NLR al concluir la cirugía<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,38</span></a>. No se ha demostrado que la neuromonitorización (NM) como herramienta diagnóstica precoz de lesión del nervio laríngeo recurrente (NLR) reduzca el riesgo. Objetivo: estimar el riesgo de PLB con y sin NM en TT mediante revisión sistemática y metaanálisis.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Método</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Revisión sistemática de ensayos clínicos, estudios de cohortes y series de casos de TT con y sin NM publicados en el periodo 2000-2014 en PubMed, Scopus (EMBASE) y Cochrane Library. Neuromonitoring (NM), as an early diagnostic tool for the existence of injury to the recurrent laryngeal nerve (RLN), has not been shown to have reduced the risk, even though published series show lower incidences. Our objective was to estimate the risk of bilateral RLN paralysis with and without NM TT by systematic review and meta-analysis.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Method</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">We performed a systematic review of clinical trials, cohort studies and case series with total thyroidectomy without NM published in the period 2000-2014. A database search was performed using PubMed, Scopus (EMBASE) and the Cochrane Library. Heterogeneity between studies was explored and weighted risks grouped according to random effects models were estimated.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">We selected 40 articles and estimates of risk were identified in 54 case series (without NM, 25; with NM, 29) with 30,922 patients.</p><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">The prevalence of BLP in the series with NM was lower compared to that without NM (2.43‰, [1.55 to 3.5‰] versus 5.18‰ [2.53 to 8.7‰]). This difference is equivalent to an absolute risk reduction of 2.75‰ with a number needed to treat of 364.13. The NM group was more homogeneous (I2<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>7.52%) than those without NM (I2<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>79.32%).
