Material y métodos. Se realizó un estudio experimental en 30 ratas Sprague-Dawley macho, distribuidas en tres grupos. El grupo control, formado por 10 animales, fue sometido a traumatismo cólico mediante anastomosis simulada pasando la sutura por el espesor de la pared cólica. El grupo con anastomosis cólica se dividió en dos: un grupo con sección cólica y anastomosis colocólica con sutura continua, formado por 10 animales, y otro grupo con sección cólica y anastomosis colocólica con sutura discontinua, formado también por 10 animales. Se indujo carcinogénesis con 1-2 dimetilhidrazina y se valoró la incidencia de tejido neoplásico en la semana 20 del postoperatorio.
Resultados. La creación de una anastomosis colocólica ejerce mayor efecto promotor de la carcinogénesis que el traumatismo cólico, y la sutura discontinua mayor efecto que la continua. La sutura continua indujo menor porcentaje tumoral y tumores menos invasivos que la sutura discontinua.
Conclusiones. La sutura continua monoplano realizada preferentemente con monofilamento reabsorbible debería ser el ideal de anastomosis digestiva manual
Material and methods. An experimental study was carried out in 30 male Sprague-Dawley rats, divided into three groups. The control group (n = 10) was subjected to colonic injury by means of sham anastomosis in which the colonic wall was sutured. The remainder of the animals were divided into two groups of 10 animals each, one subjected to colonic transection and anastomosis with running suture, the other to colonic transection with interrupted suture for the anastomosis. Carcinogenesis was induced with 1-2-dimethylhydrazine and the presence of neoplastic tissue was assessed at postoperative week 20.
Results. Colonic anastomoses promoted carcinogenesis more than colonic injury, and interrupted sutures more than running sutures. The rate of tumor induction was lower with running sutures than with interrupted sutures, and the lesions were less invasive.
Conclusions.. The single-layer running suture, preferably with monofilament absorbable suture thread, is recommended for hand sewn bowel anastomoses
