The patient is a 48-year-old man who had been stabbed in the neck in the left lateral region, resulting in blood loss and air leakage. Chest X-ray showed right pneumothorax; a pleural drain tube was inserted and a significant amount of air was evacuated. Tomography showed a lack of lung expansion, as well as an interruption in the middle third of the trachea at the right cartilage–membrane junction (Fig. 1). Cervicotomy confirmed the trajectory of the wound from the left cervical region to the right cartilage–membrane junction. The J-shaped wound at the 5th–6th tracheal rings was sutured (Fig. 2), and thoracotomy was conducted to suture the 4-cm laceration in the middle third of the right trachea as well as a lung tear in the lobe of the azygos vein.
