La trombosis de los vasos principales hepáticos es una complicación grave y frecuente en el paciente que precisa trasplante de hígado. La mayoría de las veces es causa de retrasplante urgente. Presentamos 2 casos de trombosis vascular hepática secundaria a aspergilosis diseminada. Ambas pacientes presentaban un estado séptico y precisaron someterse a un segundo trasplante, a pesar de lo cual fallecieron a los pocos días. La autopsia demostró, además, aspergilosis diseminada con invasión del sistema venoso portal y la arteria hepática. Se discuten los factores de riesgo asociados a esta infección y los posibles mecanismos patogénicos.
