Changes that occur in adolescence require children to adapt to these changes. This response to changes can also be carried out as a form of adaptation to deal with stressful changes. Adolescent responses can be either adaptive or maladaptive in nature. Maladaptive responses may take forms as risky sexual activity, mood and bipolar disorders, suicidal thoughts, behavioral disorders, physical abuses, violent behaviors, problems with body image, abuse of drugs and addictive substances, and self-inflicted injuries.<a class="elsevierStyleCrossRefs" href="#bib0100"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Self-injurious behavior constitutes as one of the most prevalent adolescent's maladaptive responses to changes, and its prevalence continues to increase. BBC reported that between 2005 and 2010, there had been an increase in the number of individuals under the age of 25 who were admitted to hospital due to self-injury, namely around 50% of 1758 people in 2004–2005 and 2727 people in 2008–2009 in England.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> Adolescents who injure themselves are at risk of suicide.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">1</span></a> WHO states that as many as 62,000 adolescents died from self-injury in 2016, whereas suicide was the second largest cause of death at the age of 15–29 years old.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">4</span></a> Research in Indonesia found that 38% of students had injured themselves, and such behavior started on average at the age of 14 years.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> The prevalence of self-injurious behavior tends to increase in adolescence years.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Emotional and behavioral problems are mental problems that are common among children and adolescents. These emotional problems include symptoms of anxiety and depression, such as sadness, loneliness, worry, fear, uselessness, and anxiety, which are risk factors for self-injurious behavior in adolescents.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">6</span></a> Behavioral problems that become risk factors to injure themselves in adolescents are hyperactivity, aggressiveness, impulsivity, substance abuse, threshold and antisocial personality disorders, and hostility.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">1</span></a> In addition, adolescents experiencing poor quality relationships, such as lack of social support, negative interactions, and abuse, are more at risk of injuring themselves.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">7</span></a> EBP often experienced by adolescents and become the most influential risk factors for adolescents in carrying out self-injurious behavior.</p><p id="par0020" class="elsevierStylePara elsevierViewall">The researchers conducted a preliminary study at SHS 112 Jakarta through interviews with students and the Counseling Teacher. The teacher said that student behaviors tended to be good, and there had been no severe violation of school discipline. However, there were risk factors for emotional problems, such as anxiety problems among students related to academic, economic, and family problems. Through interviews, it was found that there had been some cases of physical abuse, quarrels, and abuse of seniority system among students, as well as suicidal thoughts and self-injurious behaviors, such as grabbing hairs and banging heads. These conditions indicate that some of the adolescents had emotional problems and self-injurious behaviors. Based on this, the researchers conducted this research to explore the emotional and behavioral problems, self-injurious behavior, and the relationship between emotional and behavioral problems with self-injurious behavior. It is hoped that this research can be used as an additional reference to prevent risk factors for self-injurious behavior among adolescents.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Method</span><p id="par0025" class="elsevierStylePara elsevierViewall">This study used a descriptive-correlative with a cross-sectional approach. The variables in this study consisted of emotional and behavioral problems as independent variables and self-injury as a dependent variable. The sampling technique used was purposive sampling and stratified-cluster sampling with a total sample of 263 high school adolescents in West Jakarta. The samples used in this study were adolescents who were the students of class XI of SHS 112 Jakarta and SHS 33 Jakarta. The inclusion criteria are those who held an active student status at the respective schools and agreed to participate in the research as respondents. The exclusion criteria were adolescents who were younger than 15 years old and older than 18 years old, and were not attending the class when the data were being collected. This study was conducted for six months, from January to June 2019.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Data were collected using three instruments, including a demographic questionnaire, the Strength Difficulties Questionnaire to measure emotional and behavioral problems, and the Deliberate Self-Harm Inventory to measure self-injury. All questionnaires are valid and reliable. Data were analyzed using univariate and bivariate analysis. The univariate analysis used central tendency and frequency distribution. Meanwhile, the bivariate analysis used the Gamma Correlation Test. This study was conducted under the Ethical Permission of the Faculty of Nursing, Universitas Indonesia, No. 74/UN2.F12.D/HKP.02.04/2019.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Characteristics of adolescents</span><p id="par0035" class="elsevierStylePara elsevierViewall">The characteristics of adolescents consisted of age, sex, and residence status. Most respondents in class XI SHS in West Jakarta were 16 years old, with the youngest being 15 years old and the oldest being 18 years old. This study involved more female adolescents than their male counterparts, with a percentage of 65%. The majority (84.4%) of high school class XI adolescents in West Jakarta live with their biological fathers and mothers (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Adolescents self-injury</span><p id="par0040" class="elsevierStylePara elsevierViewall">Most adolescents (52.5%) in class XI SHS in West Jakarta had a clear level of self-injury (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Adolescent emotional and behavioral problems</span><p id="par0045" class="elsevierStylePara elsevierViewall">The emotional and behavioral problems of adolescents in class XI SHS in West Jakarta were mostly within the normal range of 69% (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Relationship between emotional problems and self-injury in adolescents</span><p id="par0050" class="elsevierStylePara elsevierViewall">The researchers used the gamma correlation test to see the relationship between emotional and behavioral problems and self-injury in adolescents. Adolescent emotional and behavioral problems showed a significance value of <span class="elsevierStyleItalic">p</span> 0.000 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05). Therefore, it can be seen that there is a significant relationship between emotional and behavioral problems and self-injury in adolescents, with positive direction (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Discussion</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Self-injury in adolescents</span><p id="par0055" class="elsevierStylePara elsevierViewall">Most grade XI SHS adolescents in West Jakarta showed no self-injurious behavior. This finding is in line with several previous studies which found that most adolescents were non self-injurer (clean category), but there were some differences in the number of adolescents who had inflicted self-injury to their bodies (47.5%). A research on high school adolescents in Turkey found that 44.8% of students in the country had engaged in self-injurious behaviors.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> A comparative study comparing 11 studies in Europe found an average prevalence data of 27.6% in adolescents, ranging from 17.1% in Hungary to 38.6% in France.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a> This difference could be due to differences in questionnaires and research methods used, but the number of prevalence in this current study can be considered high. The high prevalence of self-injury is due to adolescents having maladaptive coping strategies and experiencing more symptoms of psychopathology.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">10</span></a> Lack of social support and sensitivity of parents and teachers in seeing adolescent problems also the constituted factors. Self-injurers tend to cover their scars because they are afraid of being mocked and being rejected and criticized.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">11</span></a> Therefore, this behavior cannot be clearly seen and monitored by adults, which in turns may make adolescents to continue such behavior.</p><p id="par0060" class="elsevierStylePara elsevierViewall">There are more adolescents in this research who do not have self-injurious behavior and need to maintain their conditions so that they do not want to try to harm themselves. This can be done by providing mental health promotion regarding self-injurious behavior and stress management techniques.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">1</span></a> In addition, because of the prevalence of self-injury that continues to increase in adolescents, regular screening needs to be done in family settings. Psycho-education in the family needs to be taught about the behavior of self-injury in adolescents in order to be able to monitor and provide appropriate treatment.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">12</span></a> However, adolescents with self-injurious behavior need some efforts to prevent the behavior to become more severe. Coping-Skills Training can be held to improves the ability of adolescents to overcome stress by developing the coping skills that focus on positive emotions and ways to minimize avoidance coping.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a> Cognitive Behavior Therapy (CBT) also can be done to reduce self-injurious thought and behavior by changing the patterns of deviate thinking and strengthening the skills to overcome, communicate, and solve problems adaptively.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a></p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Emotional and behavioral problems in adolescents</span><p id="par0065" class="elsevierStylePara elsevierViewall">Emotional and behavioral problems in most high school adolescents in West Jakarta can be observed in the normal category, followed by borderline and abnormal categories. The results of this study are in line with the research in adolescents in the RSCM psychiatric children unit, most of whom showed the adolescent EBP within the normal category.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">15</span></a> Research on adolescents aged 14–19 years in Kenya also showed similar results, where 78% of adolescents had EBP within normal category.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">16</span></a> This could be caused by the similarity of the characteristics of the respondents resulting in similar data. Even so, adolescents who have EBP within borderline and abnormal categories also become problems that require attention. EBP in adolescents can be caused by mental health conditions, poor relationships in the family, poor economy, bridging development, and cognitive problems.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">17</span></a> EBP in adolescents needs to be watched out for and dealt with to avoid the adolescents from doing risky behavior.</p><p id="par0070" class="elsevierStylePara elsevierViewall">An effort is needed to maintain and improve the well-being of adolescents with EBP within the normal category. Better emotional and behavioral health in adolescents can be achieved through the improvement of family function and the ability of adolescents to build self-regulation and to increase social support through education in schools. Psycho-education can be given to families to increase their knowledge about the symptoms, to improve positive emotions and family interactions.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">18</span></a> School-based interventions can be done by providing educators with a curriculum to teach and support youth in building self-control and social competence. However, an adolescent with borderline and abnormal EBP categories also need to be dealt with by interventions. The intervention treatment that can be done is Cognitive Behavioral Therapy (CBT). CBT is a psychotherapy that emphasizes the important role of thinking in influencing individual feelings and actions. The idea behind CBT is that it is thoughts that cause feelings and behavior, not external things, such as people, situations, and events.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">19</span></a></p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Relationship between emotional and behavioral problems and self-injury in adolescents</span><p id="par0075" class="elsevierStylePara elsevierViewall">Emotional and behavioral problems are significantly related to self-injurious behavior. EBP constitute two factors with the highest level of relationship significance with self-injurious behavior compared to other variables. This shows that the more severe the emotional problems and behaviors the adolescents have, the more likely they would commit a higher degree of self-injury. Previous research also yielded similar results, in which self-injurious behavior was largely caused by emotional and behavioral problems, such as anxiety, depression, low self-esteem, aggressive and impulsive attitudes, eating disorders, and borderline personality disorders. These EBP cause adolescents to injure themselves due to the vulnerability of having unstable emotions and immature decision-making attitudes. Therefore, an effort is needed to prevent and overcome such problems.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Most adolescents with EBP within the normal category do not exhibit self-injurious behavior. This is because adolescents possess a greater ability to master their thoughts and behavior, so that they can act rationally and think about the consequences before taking any certain action. This condition needs to be maintained by improving their mental health conditions. Mental health promotion helps adolescents to build resilience so that they can handle difficult situations. This can be done through activities in the family and school circles, such as creating a safe and positive psychological environment, educating about mental health issues, and teaching life-skills.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">4</span></a> However, adolescents with EBP in borderline and abnormal categories are more involved in self-injurious behavior compared to adolescents in the normal category. Cognitive Behavioral Therapy (CBT) can be taken to improves emotional, cognitive, and behavioral skills by reducing negative thoughts and suicide, and increasing problem-solving practice.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The research limitations faced by the researchers include the data collection period that was close to the time of school exams and national examinations so that there is a school that did not grant research permission. The researchers then looked for another school and collected data at a much longer time than originally planned. Future researchers are expected to be able to plan better research time and to negotiate data collection time.</p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Conclusion</span><p id="par0090" class="elsevierStylePara elsevierViewall">To conclude, there is a significant relationship between emotional and behavioral problems and self-injury in adolescents. Prevention and awareness promotional efforts regarding adolescent self-injury can be taken, among others, through mental health promotion, stress management techniques, regular screening, family psycho-education, Coping-Skills Training, and CBT. Meanwhile, the prevention of and awareness promotion on adolescent EBP can be catered through family psycho-education and school-based interventions to improve adolescent self-regulation ability. This research applied purposive and stratified-cluster sampling techniques involving 263 Senior High School (SHS) adolescents in West Jakarta. Data were obtained by using Strength-and-Difficulties Questionnaire and Deliberate Self-Harm Inventory questionnaire. The majority of adolescents have a normal level of self-injurious behavior of 52.5%. The EBP that the adolescents have been in the normal range of 62.4%.
