id="sect0015">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The Joint Commission International Accreditation Standards for Hospitals encourage the hospitals to develop and implements sophisticated process for handover communication. Effective communication that is conducted timely, accurate, complete, unambiguous, and understood by the recipient, helps to reduce errors and improve the patient safety. One of the services securities in the hospital is by implementing handover communication which is a key for staff to achieve patient safety goal. The handover communication process is mentioned in Goal 2 of the International Patient Safety Goals (IPSG).<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Australian Commission on Safety and Quality in Healthcare stated that bedside handover is an information delivery method (responsibility and accountability) when transferring continuous care or exchanges between nurse shifts held at the patient's bedside to deliver information to the patients and also between the nurses, ensuring continuity of care, patients are also given an opportunity to convey inputs and problems they felt.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">2</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">A study conducted by Chaboyer, McMurray and Wallis found that the implementation of bedside handovers could improve the information delivery accuracy.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">3</span></a> The bedside handover was a strategy to improve communication between nurses, patients, and patient's families. Nurses involved patients in this process and it only spent 1.5<span class="elsevierStyleHsp" style=""></span>min.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">3</span></a> During the effort to increase the information delivery effectiveness between nurses, communication should be carried out at the patient's bedside during shift's changes (bedside handovers). Until now, the implementation of bedside handover procedure has not been explained specifically, so it makes each health service organization has various interpretations in its implementation. The absence of this policy resulted in the implementation of various bedside handovers technique, including at Public Hospitals in Aceh. A study conducted by Thaher found that the lack of knowledge and understanding of nurses about the importance of the bedside handover process for patients so that it has an impact on nurses being less active in the stage of the preparation, patient involvement and safety scans.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">4</span></a> There were 2025 observations of shift bedside handover in the inpatient room: 1762 observations (87%) were classified into bad category, 263 observations (13%) were classified into good category.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">4</span></a> The aforementioned phenomenon above clearly indicated that the comprehensive and continuous socialization is needed to apply and cultivate the implementation of bedside handover correctly and uniformly.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This research was a quasi-experimental study with one group pre and post-test design. The population in this study was 88 nurses worked in inpatient rooms. Samples comprised 64 nurses were recruited using purposive sampling technique. Inclusion criteria for samples were the nurses who were on duty in the inpatient room either the State Civil Service or on a contract basis, nurses who have a minimum educational background of Diploma III in Nursing, working period<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>1 year, willing to be a respondent as evidenced by a letter of willingness to be a respondent. Exclusion criteria were the nurses who were on leave at the time of the research (annual leave, maternity leave, leave for important reasons) and nurses who are on a learning assignment.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Program bedside handover training included information about bedside handover, hospital policies related to bedside handover, and the demonstration or role play about bedside handover. The implementation of bedside handover was carried out in five stages with the preparation bedside handover, introduction to the patients, exchange the information, patient involvement, and safety scan. The methods used were lectures, question and answer, discussion, and demonstration/role play. The implementation of this training adhered to the health protocols during COVID-19 pandemic.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Program effectiveness was assessed one week before (pre-test) and after (post-test) intervention completion using a questionnaire for knowledge aspect, and an observation list for attitude and skill aspects. The instruments were developed by researchers based on findings of literature reviews. Validity and reliability test of the questionnaire were conducted and show that the questionnaire had good internal consistence (Cronbach alpha value of 0.89 for knowledge questionnaire). The observation list assessing attitudes and skills was considered valid by three experts in its relevant field.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Data were analyzed using univariate analysis for demographic data. Normality test with Kolmogorov–Smirnov test was done and found that knowledge, attitudes, and skills data were normally distributed (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>0.05), so the paired <span class="elsevierStyleItalic">t</span>-test was used for bivariate analysis.</p><p id="par0040" class="elsevierStylePara elsevierViewall">This study was approved by the Ethics Committee of Aceh Regional Public Hospital and Faculty of Medicine, Universitas Syiah Kuala with ethical clearance number of 061/EA/FK-RSUDZA/2020. All participants provided a written informed consents and participants’ participations were voluntary.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Demographic characteristics of the respondents</span><p id="par0045" class="elsevierStylePara elsevierViewall">Based on <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, it can be inferred that 75% of the nurses were 26–35 years old and 92.2% of them were female. 60.9% of the nurses had diploma as their latest education, 68.8% of them were contract employees, and 59.4% of the respondents had 1-5 years of work experience.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Nurse's knowledge, attitude and skill about bedside handover</span><p id="par0050" class="elsevierStylePara elsevierViewall">Nurses’ knowledge, attitude and skills in implementation of bedside handover before and after training are presented in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows that there were statistically significant increase of the nurses’ knowledge, attitude and skills following the training program completion.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Discussion</span><p id="par0055" class="elsevierStylePara elsevierViewall">Training is defined as an organized method for ensuring that every person has a good knowledge and skills for a specific purpose and they have obtained the necessary knowledge to support their jobs. Moreover, knowledge also requires an increased cognitive, affective and motor skills.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">5</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">According to a study conducted by Dewi, the most successful bedside handover was depended on the nurse's knowledge, so it was recommended for the nurses to optimize the bedside handover's quality through bedside handover training.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">6</span></a> The bedside handover training in this study was an effective way to improve nurses’ attitudes in implementing bedside handover. It was also supported by a study conducted by Safitri and found that handover training which was conducted using a modern approach and also applied at patient's bedside showed a significant difference in nurses’ motivation and job satisfaction before and after receiving training.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">7</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">There were five stages of bedside handover. It was the preparation bedside handover, introduction to the patients, exchange the information, patient involvement, and safety scan. Preparation at the nurse station is the first step in implementing bedside handover. At this stage, the previous shift nurse needs to prepare and ensure the information to be delivered during bedside handover.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The preparations made by nurses before starting a bedside handover were: (1) allocated the patient, (2) reviewed the patient's status, (3) informed the patient that a bedside handover will be started, and (4) asked families and visitors to leave the room.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">3</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">The second stage is the introduction of nurse and the patient where the nurse greets the patient and the previous shift nurse introduces the next shift nurse to the patients and their family. This stage is an important stage to build a trust relationship between nurse and the patients.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The third stage is the information exchange. All information that has been recorded in the patient's medical record is delivered sequentially and concisely, so the information conveyed by nurses during bedside handover can be carried out systematically and uniformly. According to a study conducted by Radtke,<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">8</span></a> bedside handover was an effort to improve communication between nurses, patients and their families. It is also supported by a study conducted by Hagman, Oman, Kleiner and Johnson which found that bedside handover increased clinical effectiveness, patient safety, nurse efficiency, and nurse satisfaction.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">9</span></a></p><p id="par0085" class="elsevierStylePara elsevierViewall">It was also supported by a study which found that there was a significant increase in the ability to implement patient handovers and patient safety as well as 5.95 (27%) and 3.76 (47.12%) respectively after being given a handover training with an effective communication approach to the nurses at Raden Mattaher Hospital Jambi.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">10</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">The fourth stage is patient involvement. Bedside handovers can contribute to maintain patient's information confidentially and involve patients in the treatment plans. Moreover, patients also can exchange information with the nurses about their treatment activities to ensure that there were no misunderstandings between them and the nurse as service providers.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">11</span></a> Tobiano, Bucknall, Sladdin, Whitty, and Chaboyer in their study stated that one of the barriers faced by nurses in involving patients’ participation during bedside handover process was due to the concerns about sharing confidential and sensitive information.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">12</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">It also supported by a study conducted by Chaboyer, McMurray, and Wallis which found that there were several activities in the patient's engagement process, which were: asked the patients if they have questions and gave them opportunities to confirm and clarify. Understanding the patient is a way for nurses to involve patients into the health services, know the patient's needs, the desire of the patients, and their expectations.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">3</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">The last stage is a safety scan. Nurses need to conduct a safety check on the patients’ environment and equipment to identify the safety and the risk of accidents that can occur to patients. A study conducted by Mellawani, Yetti and Nuraini found that the nurses’ caring attitude in implementing bedside handover is expected to increase the caring behavior in implementing bedside handover, one of the steps is by creating a comfortable environment to support the patient's recovery process.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">13</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">The nurses’ skills in implementing bedside handovers can become a culture if they are followed by establishing a standard operating procedure for bedside handovers and continuing the implementation of socialization or training for bedside handovers on an ongoing basis for all nurses. An earlier study found that the most successful bedside handovers depend on the nurses’ knowledge, attitudes and skills. Consequently, the optimization of bedside handover's quality can be recommended by conducting a bedside handover training or simulations.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">6</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">This study had the limitations. The participants were recruited from one of public hospitals in Aceh Province, so the findings may not represent all public hospitals in the province. Also, the instruments were developed by researchers, although it had been tested for its validity and reliability, further tests are required to assess the validity and reliability of the instruments especially for the attitude and skill observation.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Findings of this study suggest that there was a statistically significant increase of the nurses’ knowledge, attitudes, and skills in implementing bedside handovers after training. Limited knowledge about bedside handover can reduce the quality of nursing care provided by nurses. The nurses’ knowledge limitation can be increased through training about bedside handover. This study aimed to determine the effectiveness of bedside handover training to improve the knowledge, attitudes, and skills among nurses. This study was a quasi-experimental study with one group pre- and post-test design. The convenience sampling method was used to collect data from 64 nurses working in a public hospital in the capital of Aceh Province. All participants were provided two-days training (three-hours a day) about bedside hand over. Program effectiveness was assessed one week before and after program training completion using a paper-based questionnaire and an observation list. Data were analysed using parametric statistical tests and paired <span class="elsevierStyleItalic">t</span>-test.
