During 2005–2010, there were 526,186 incidents of medication error in the United Kingdom, with the rate of damage amounted to 16% (86.821) and 822 people with severe damage or death. Economic loss in 2017 was reported amounted to € 770 Billions.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">1</span></a> In Southeast Asia, it was revealed that 7%–35% of medication error due to prescribing error, 15%–88% due to administration error, and 14%–35% because of distribution error.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Errors in the administration phase (35%–70%) occurred due to wrong time, wrong patient, wrong dosage, and wrong route.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">3</span></a> Therefore, to improve patient safety in medication administration process, a procedure known as five rights of medication administration is developed. It includes the right drug, right patient, right dosage, right time, and right route.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">One of the factors contributing to medication errors in the administration phase is a nurse.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">1</span></a> Based on the Theory of Reasoned Behavior,<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">5</span></a> the accuracy of medication administration by nurses preceded by attitudes, in which the attitudes begin with knowledge and motivation. Good knowledge of patient safety and medication errors affects the professional attitude of health workers before and after medication errors.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">6,7</span></a> One of the required knowledge is about standard operating procedures in the medication administration process. Besides knowledge and attitudes, extrinsic and intrinsic motivations provide a positive effect on the application of patient safety.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> Attitudes in the implementation of medication administration act as predictors for an individual to do something consistently.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Several training methods can be used to improve nurses’ knowledge, attitudes, and motivations, one of which is a workshop. Training in the standard operating procedures of five rights of medication administration is required to ensure consistent implementation.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">10</span></a> The workshop is a form of training conducted by classical discussion, learning and practical work.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">11</span></a> The workshop provides more insight and increases knowledge retention<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">12</span></a> because it encourages a person to behave as trained.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">This study is conducted at a type B accredited hospital. It has 329 beds and 402 nurses and midwives. Data show that there were 15 incidents, 12 of which were categorized in medication errors, and 3 of which were the case of patients who accidentally fell. The medication errors included 10 cases of wrong drug and 2 cases of the wrong patient. A preliminary study observing the implementation of medication administration was conducted in two inpatient wards. It revealed that almost all procedures carried out were to ensure the time of injection and drug route.nurse Confirma of dosage and drug with the doctor's instructions mostly (68–79%), while confirmation of the patient's identity was only performed 11% of observations, Oral drug administration was not monitored. The data showed the importance of efforts to consistently improve the behavior of correct medication administration. The workshop method with its advantages is chosen in this study. Workshop would be tested to improve and maintain nurses’ knowledge, motivations and attitudes toward five rights of medication administration.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Method</span><p id="par0030" class="elsevierStylePara elsevierViewall">This research was carried out through quasi-experiment, pre, and post without control group design. It was conducted in a hospital from April to May 2019. The samples of this study were 140 nurses from 217 populations of nurses who were selected purposively. The samples were selected based on these following criteria (1) graduated from Diploma III, (2) have worked at least one year, (3) were related to patients daily. The workshop was performed by competent nurses who followed by videos presentation about five rights of medication administration, followed by discussion. Nurses’ knowledge, attitudes, and motivations were measured before, one, and four weeks after the workshop. The instrument was a closed-ended questionnaire with five-point Likert scales, which consisted of 10 items (knowledge), 17 items (extrinsic and intrinsic motivations), and 20 items (evaluative attitudes and beliefs).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">In this study, the majority of respondents were female (67%), aged between 26 and 35 years (48%), had been working for more than five years (55%) and belonged to contract employees (53%). The results showed from <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, that the mean of knowledge before and after the workshops increased even though the mean score was in moderate criteria (4.71–5.59). The highest mean of knowledge was four weeks after the workshop (5.59). The mean of motivations and attitudes also increased one week after the workshop as well as four weeks after the workshop. Overall, the workshop could increase and maintain knowledge, attitude, and motivation for up to four weeks. A greater increase was obtained one week after training. After four weeks, there was still an increase but small.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Discussion</span><p id="par0040" class="elsevierStylePara elsevierViewall">The finding of this study concurred that the workshop effectively increases nurses’ knowledge, motivations and attitudes toward five rights of medication administration. Nurses’ knowledge about five rights of medication administration is still in the range of low and moderate categories even though nurses’ motivation and attitude about medication administration are high.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The results of this study are by previous research results, which indicate that the workshop can increase knowledge.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a> The workshop provides participants with more insight, and the discussion session was a knowledge-gathering session on practice.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">15</span></a> The workshop can also maintain knowledge retention for up to 4 weeks after learning and decrease after one year.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">16</span></a> The results also show that the range of knowledge value is at low and moderate categories. Since good knowledge can enhance professionalism, it requires repetition of training to increase knowledge and maintain knowledge retention.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">The mean score of nurses’ motivation is high before, one, and four weeks after the workshop. The motivation which remains high before and after the training is by Caroline's research in 2010.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">17</span></a> High motivation is needed by nurses to administer drugs properly and still maintain the behavior.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a> The workshop influences motivation because it clarifies the objectives of the training, increases interest and displays values that belong to motivational factors.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">18</span></a> Intellectual motivation also improves learning performance and understanding. The workshop is one way to create organizational culture as trained and together with an environmental factor as extrinsic motivations.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">8</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The high mean score of attitude is a depiction of respondents’ behavior toward five rights of medication administration if the attitude is consistent.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a> Consistent is a picture of one's behavior.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">19</span></a> Even though this study does not directly measure behavior, but previous studies have stated that there is a relationship between attitude and behavior. A consistent attitude is expected to form behavior.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a> Also, the behavioral assessment will require closed observation and consumed more time.</p><p id="par0060" class="elsevierStylePara elsevierViewall">After the workshop, nurses’ attitudes, both evaluative and belief attitude toward five rights of medication administration, significantly increased which remain consistent four weeks after the workshop. The skills improvement as a result of the workshop encourages a positive attitude of the participants.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">14</span></a> Attitudes produce beliefs that can come from outside of oneself. External beliefs that can be manipulated are knowledge.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">9</span></a> which can be improved through workshop.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">13</span></a> Simplifying the language can be well understood, organize workgroups and discussions, action plans at the end of the workshop and provide support during implementation are some of the strategies to improve attitude after training.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">11</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Conclusion</span><p id="par0065" class="elsevierStylePara elsevierViewall">The results of this research support the evidence that the workshop can increase nurses’ knowledge, motivations and attitudes about five rights of medication administration and maintain for up to 4 weeks.
