High doses of biotin interfere with numerous laboratory tests, leading to misdiagnosis and inappropriate patient management.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Biotin (also known as vitamin B7, vitamin H, or coenzyme R) is a water-soluble vitamin used therapeutically for various medical conditions including multiple sclerosis, inherited metabolic diseases, mitochondrial disorders, and nutritional deficiency. It is also used widely for nonmedical applications, especially for hair, skin, and nail growth.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Biotin binds avidin or streptavidin with powerful non-covalent interactions. This characteristic is often applied in immunoassays for the quantification of a wide range of analytes. In patients treated with high-dose biotin, immunoassays subject to biotin interference can potentially lead to two types of analytical errors: in competitive immunoassays, excess biotin competes with the biotinylated analog resulting in falsely elevated values; in sandwich immunoassays, excess biotin displaces biotinylated antibodies resulting in falsely depressed values.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The daily recommended intake of biotin is 0.03<span class="elsevierStyleHsp" style=""></span>mg. Physicians may prescribe high levels of biotin (≥300<span class="elsevierStyleHsp" style=""></span>mg) in certain pathologic conditions, but there are also many cases of self-medication with vitamin supplements rich in biotin; some of them contain biotin levels up to 20<span class="elsevierStyleHsp" style=""></span>mg – 650 times the recommended daily allowance. Also, patients are not always aware that some of the vitamin supplements contain biotin.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Immunoassays using biotin-labeled antibodies or biotin-streptavidin labeled complexes are susceptible to significant interferences for a wide array of laboratory tests in patients who receive high doses of biotin (<a class="elsevierStyleCrossRef" href="#tbl0005">Table I</a>).<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">1–5</span></a> Biotin concentrations above 400<span class="elsevierStyleHsp" style=""></span>μg/L may cause interference for thyroid hormones, TSH, troponin, digoxin, N-terminal prohormone of brain natriuretic peptide (NT-proBNP), progesterone, and estradiol assays; in case of free triiodothyronine (fT3) and troponin, significant interferences may be observed even with biotin concentrations of 300<span class="elsevierStyleHsp" style=""></span>μg/L.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">4</span></a> Also, potential biotin interferences have been proposed for luteinizing hormone, ferritin, and various markers like prostate-specific antigen, CA15-3, and CA19-9, but such interferences have not been detected by all authors.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">2,4</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">Both false positive and false negative results may occur for the same patient, with combinations that may lead to erroneous diagnosis and, consequently, improper management. Examples of such combinations could include elevated free thyroid hormones with a low TSH, elevated 25-hydroxyvitamin D with a low parathyroid hormone (PTH), or elevated cortisol with a low adrenocorticotropic hormone. Analytes with a low diagnostic threshold (like troponins) may require particular caution to avoid biotin interferences and severe clinical implications, including exitus letalis.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Biotin interference needs to be communicated to clinicians and healthcare professionals. If high doses of biotin intake are known, clinicians should communicate it to the laboratory.
