In recent years, several studies have shown that regular physical activity, especially aerobic exercise training could improve sleep quality and daytime function in patients with chronic insomnia.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">5,6</span></a> Physical activity through walks is effective in the prevention of disorders of quality and duration of sleep.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> High diet quality reduces risk of several chronic diseases and mortality.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">8</span></a> In particular, the intake of whole grains and fiber, and fruit and vegetables reduce chronic disease risk, while the intake of saturated fat, sodium, and sugar-sweetened may increase disease risk.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">9</span></a> It has also been found that a diet quality with higher consumption of certain foods, improves sleep quality.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">10</span></a> However, the majority of these studies focus fundamentally on sleep duration, with neither insomnia nor the diurnal consequences it may have coming specifically into play. We hypothesized that increased physical activity by walking measured with pedometers and diet quality, are associated with less insomnia. The present work aims to evaluate the associations of the presence of insomnia with physical activity measured with pedometers and diet quality evaluated with the diet quality index (DQI) in an adult population.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Design</span><p id="par0010" class="elsevierStylePara elsevierViewall">The findings shown here are a subanalysis of the TRIPLE A study (Advanced Approach to Arterial Stiffness), which was intended to evaluate structure and vascular function, in a large European population. The TRIPLE A is an multicentre prospective longitudinal study from 18 European countries, with three scheduled visits at baseline and after 2 and 5 years of follow-up. First results for the TRIPLE A study can be found in a previously published.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">11</span></a> The TRIPLE A study is registered at <a id="intr0005" class="elsevierStyleInterRef" href="http://www.clinicaltrials.gov/">www.clinicaltrials.gov</a> with the ID number: <a id="intr0010" class="elsevierStyleInterRef" href="https://clinicaltrials.gov/NCT02318628">NCT02318628</a>. In the present report, we present the analysis of the baseline data in Salamanca, Spain.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Study population</span><p id="par0015" class="elsevierStylePara elsevierViewall">A total of 2348 subjects were enrolled in 32 centers for the TRIPLE A study. In Salamanca (Spain) a total of 454 subjects were selected by consecutive sampling from the practices of family doctors to evaluate cardiovascular risk. Inclusion criteria: Patients from 20 to 80 years of age, free of cardiovascular disease diagnosed. Exclusion criteria: Patients with peripheral arterial disease, body mass index (BMI) >40<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>, atrial fibrillation, and pregnancy were not selected. The recruitment and data collection period for the study was January of 2014 to June of 2015 (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Sample size</span><p id="par0020" class="elsevierStylePara elsevierViewall">Accepting an alpha risk of 0.05 and a common standard deviation of 3312 steps in a two-sided test, with 332 subjects in the first group (no insomnia) and 122 in the second (insomnia), the statistical power was >95% to recognize as statistically significant the difference of means between groups of 1685 steps (primary outcome measure). Therefore, we considered enough the 454 subjects included in the study. However, regarding to diet quality index, accepting an alpha risk of 0.05 and a common standard deviation of 3, the statistical power was 60% to recognize as statistically significant the difference of means between groups of 0.7 points. The study was approved by the clinical research ethics committee (CEIC) of the health care area of Salamanca (“CEIC of Area de salud de Salamanca”, 9 June 2014). Informed written consent was collected in all included patients, according to the recommendations of the Declaration of Helsinki.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Measurements</span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Sleep quality</span><p id="par0025" class="elsevierStylePara elsevierViewall">Patient sleep quality was evaluated with the Atenas scale, a self-administered instrument with eight items developed by Soldatos,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">12</span></a> that was validated for use in the Spanish population.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">13</span></a> The first five items evaluate “sleep disturbances” (i.e., induction of sleep, nocturnal awakenings, early arising, total duration of sleep, and general quality of sleep) and correspond to criterion “A” of the insomnia diagnostic according to the classification of mental and behavioral disorders (ICD-10). The last three items evaluate “daytime consequences” (i.e., sensation of well-being by day, physical and mental functioning by day, and somnolence during the day), and correspond to criterion “C” of the ICD-10. Each item is answered with a Likert-type scale, which ranges from 0 “no problem” to 3 “significant problem”, with a minimum total score of 0 and a maximum of 24. Higher ratings indicate greater severity of deterioration. A score ≥6 indicates the presence of insomnia.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">14</span></a></p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Physical activity</span><p id="par0030" class="elsevierStylePara elsevierViewall">Physical activity was evaluated through the use of digital pedometers (Omron HJ-321, Omron Healthcare Co., Ltd., Kyoto, Japan) over seven days, which has been previously validated.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">15</span></a> Patients wore the pedometer on the right side of the waist. Piezoelectric sensors use multiple-position sensing technology. The manufacturer of the pedometer describes that in the model used in this study, an acceleration waveform is generated from both sensors, and based on the size and shape of the waveforms, an algorithm selects the sensor that is used to count steps. The pedometer features and displays total steps, aerobic steps, the distance traveled, and calories consumed, and it stores results for the previous seven days. Calories burned were estimated by measuring the intensity of the activity. Aerobic steps were counted separately when walking more than 60 steps per minute and more than 10<span class="elsevierStyleHsp" style=""></span>min continuously. If a rest of less than 1<span class="elsevierStyleHsp" style=""></span>min was taken after a continuous walk of more than 10<span class="elsevierStyleHsp" style=""></span>min, it was regarded as part of “a continuous walk”. To score the physical activity that the patients performed as measured with pedometers, the classification established by Tudor-Locke and Bassett<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">16</span></a> was used, in which it was established that a person is active if they walk 10,000 or more steps per day. For the analysis, data from five of the seven recorded days were taken into account, discarding the information from the first and last days.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Diet quality</span><p id="par0035" class="elsevierStylePara elsevierViewall">Diet was evaluated using the diet quality index (DQI),<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">17</span></a> which includes three food group categories: daily intake of one portion of food in the first food group category was scored 2; lower and higher intakes were scored 1 and 3, respectively. Daily consumption of one alcoholic drink (one bottle of beer, one glass of wine, or one serving of liquor equivalent to approximately 12<span class="elsevierStyleHsp" style=""></span>g of alcohol) was scored 3; lower and higher intakes were scored 1. Consumption of foods considered detrimental in the second food group category were scored 2 if reported as 4–6 times per week; more and less frequent consumption rates were scored 1 and 3, respectively. High consumption (four or more times per week) of food items considered beneficial in the third food group category was scored 3. Intakes of 2–3 times and less than twice a week were scored 2 and 1, respectively. All food item scores were summed. The total possible score ranged from 18 to 54.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Smoking status</span><p id="par0040" class="elsevierStylePara elsevierViewall">Smoking history was assessed through questions about the participant's smoking status (smoker/non-smoker). We considered smokers to be subjects who currently smoked or who stopped smoking less than one year ago.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Definition of hypertension, type 2 diabetes mellitus and dyslipidemia</span><p id="par0045" class="elsevierStylePara elsevierViewall">The 2013 European Society of Hypertension and European Society of Cardiology guidelines defined hypertension as values ≥140<span class="elsevierStyleHsp" style=""></span>mmHg SBP and/or ≥90<span class="elsevierStyleHsp" style=""></span>mmHg DBP or the presence of antihypertensive treatment.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">18</span></a> The guidelines of the American Diabetes Association defined diabetes mellitus type 2 as the presence of HbA1c ≥6.5% or fasting plasma glucose ≥126<span class="elsevierStyleHsp" style=""></span>mg/dl or 2<span class="elsevierStyleHsp" style=""></span>h plasma glucose ≥200<span class="elsevierStyleHsp" style=""></span>mg/dl during an oral glucose tolerance test or in a patient with classic symptoms of hyperglycemia or hyperglycemic crisis, a random plasma glucose ≥200<span class="elsevierStyleHsp" style=""></span>mg/dl or the presence of antidiabetic treatment.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">19</span></a> The American Association of Clinical Endocrinologists’ guidelines for management of dyslipidemia and prevention of atherosclerosis, defined dyslipidemia as the presence of total colesterol ≥240<span class="elsevierStyleHsp" style=""></span>mg/dl or triglycerides ≥200<span class="elsevierStyleHsp" style=""></span>mg/dl or the presence of lipid-lowering drugs.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">20</span></a></p></span></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Other measurements</span><p id="par0050" class="elsevierStylePara elsevierViewall">Age, sex, occupation, drug use, and family and personal history of risk factors (arterial hypertension, dyslipidemia, and diabetes) for cardiovascular diseases were recorded. Sleep Apnea syndrome was recorded according to the medical history of the participants. Data input was performed using the Teleform system (Autonomy Cardiff Vista, California, USA). The study was performed by a trained research group.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Anthropometric measurements</span><p id="par0055" class="elsevierStylePara elsevierViewall">Body weight was measured twice using a homologated electronic scale (Scale 7830; Soehnle Professional GmbH& Co, Backnang, Germany) after calibration (precision ±0.1<span class="elsevierStyleHsp" style=""></span>kg). Height was measured using a portable system (Seca 222; Medical scale and measurement systems, Birmingham, UK). The mean of two readings taken in the standing position was recorded. Values were rounded to the nearest centimeter. BMI was calculated as weight (kg) divided by height squared (m<span class="elsevierStyleSup">2</span>). A value >30<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span> was defined as obese. The anthropometric variables were evaluated with the patient wearing light clothing and no shoes.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Office blood pressure</span><p id="par0060" class="elsevierStylePara elsevierViewall">Office blood pressure (BP) was calculated as the average of the last two of three measurements of systolic BP (SBP) and diastolic BP (DBP), made with a validated sphygmomanometer (OMRON Model M10-IT, Omron Healthcare Co., Ltd., Kyoto, Japan). Measurements were made on the dominant arm after participants had been in a seated position for at least 5<span class="elsevierStyleHsp" style=""></span>min, with a cuff of appropriate size as determined by measurement of the upper arm circumference and following the recommendations of the European Society of Hypertension.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">21</span></a></p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Laboratory determinations</span><p id="par0065" class="elsevierStylePara elsevierViewall">Venous blood sampling was performed between 08:00 and 09:00, after the individuals had fasted and abstained from smoking, alcohol, and caffeinated beverages for the previous 12<span class="elsevierStyleHsp" style=""></span>h.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Statistical analysis</span><p id="par0070" class="elsevierStylePara elsevierViewall">The continuous variables are expressed as the means (standard deviations). Frequency distributions were used for categorical data. Comparisons between qualitative variables were performed using the chi-squared test and Fisher's exact test. We used the <span class="elsevierStyleItalic">t</span>-Student–Fisher test to compare quantitative variables between two groups. The ANCOVA test was used to evaluate the differences in mean DQI, steps/day, aerobic steps/day and kcal spent/day values between (<6<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no insomnia; ≥6<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>insomnia) of the Atenas scale, with three models of adjustment. Model 1: No adjustment; Model 2: adjusted for age and sex (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>female and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>male); Model 3: adjusted for age, sex (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>female and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>male), current smoking (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>yes), BMI (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>yes), hypertension (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>yes), diabetes (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>yes), dyslipidemia (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>yes) and sleep apnea syndrome (0<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>no and 1<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>yes). The analysis has been made with all the cases, including those cases that had some missing data. The data were analyzed using SPSS Statistics for Windows version 23.0 (IBM Corp, Armonk, NY, USA). Values of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 were considered statistically significant.</p></span></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">The average patient age was 57.9 (12.0) years, and 57.3% of patients were female. A total of 33.8% of the women and 17.5% of the men presented with insomnia. The habit of smoking was more frequent in the group of people who reported having insomnia (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.016). The diet quality of the patient group not presenting with insomnia (40.4<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.9) was superior to that of the group with insomnia (39.7 (3.0); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.023). Overall, subjects with insomnia presented smaller numbers of total steps, aerobic steps, and kilocalories burned during physical activity (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001). The other descriptive variables in the sample are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">The subjects with insomnia obtained the highest percentages in all the items with the highest score of the Athens scale, both in the “sleep disturbances” section and in the “daytime consequences” section (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">An inverse correlation was obtained between the overall Atenas scale rating and the physical activity parameters kcal spent/day (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.141; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), aerobic steps/day (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.132; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), steps/day (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.129; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), and the DQI (<span class="elsevierStyleItalic">r</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.105; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05). The mean differences of total steps/day, aerobic steps/day, and kilocalories/day spent in performing physical activity, between no insomnia group and insomnia group were: 1022.5 (95% CI: 177.9–1867.0), 743.9 (95% CI: 68.3–1419.4) and 39.8 (95% CI: 5.7–73.9) respectively, after controlling for age, sex, smoking habit, BMI, hypertension, diabetes, dyslipidemia and sleep apnea syndrome. However, no association was found between the DQI and insomnia in the most complete model of adjustment, model 3 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Discussion</span><p id="par0090" class="elsevierStylePara elsevierViewall">This study's findings show that performing more physical activity, as measured with pedometers, could be correlated with less presence of insomnia. However, no relationship was observed between the DQI and insomnia. This finding suggests that physical activity may be relevant as a predictive factor for insomnia.</p><p id="par0095" class="elsevierStylePara elsevierViewall">The negative association between insomnia and performing physical activity found in this work is concordant with the study performed by Al-Eisa et al.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">22</span></a> in which pedometers were also used to quantify the total steps/day completed. Related to these findings Youngstedt,<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">23</span></a> found that performing physical activity can decrease sleep difficulties by reducing anxiety, which is associated with less insomnia symptoms. This point of view is supported by a study performed by Herring et al.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">24</span></a> which reports that the performance of physical activity can lead to stable reductions in manifestations of anxiety. In another study,<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">25</span></a> performing regular outdoor activity, such as walking, promoted light exposure, helping to synchronize the circadian rhythm of melatonin secretion necessary for maintaining sleep–wake cycles. Some studies have reported a relationship between physical activity and sleep quality; however, the majority of these studies are characterized in two ways. On the one hand, the studies analyze sleep quality overall, or perhaps one concrete characteristic, such as duration<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">7</span></a> and not insomnia, as in our study; furthermore, most cases use the Pittsburgh scale, whose Spanish-language version is less consistent and has a more complex format than the Atenas scale.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">26</span></a> Moreover, the Atenas scale includes valuations in aspects related to the act of sleeping and with the possible daytime consequences on the following day. On the other hand, in the majority of studies, accelerometers are used to evaluate physical activity. While it is true that the use of accelerometers is indicated in the objective measurement of physical activity, these devices underestimate energy expended at high intensities and when walking on inclines.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">27</span></a> In relation to these findings, in our study, we found that energy expended as measured by pedometers was also correlated with insomnia. Moreover, pedometers are light, small, easy to use, non-intrusive, easy to transport, and socially accepted and may be useful for broad segments of the population.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">28</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">In general, the different estimates point to a relationship between diet quality and sleep, although most confidence intervals include no relation, marking a non-significant result.</p><p id="par0105" class="elsevierStylePara elsevierViewall">It would be necessary to increase the sample size until a sufficient power for the variable diet quality be obtained. However, a study by Moubarac et al.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">29</span></a> demonstrated a relationship between psychological distress of patients with daytime somnolence and the consumption sugared foods. Other studies have reported an increase in brain sensitivity to rewards related to food (for example, the option of unhealthy foods) after sleep restriction.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">30</span></a> However, the majority of evidence relates to studies of sleep restriction, and restrictions sleep duration and poor quality of sleep may have different etiologies.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Some limitations may have had an influence on this study's results. The principal limitation was the cross-sectional design for data gathering, which impedes establishing a causal relationship between physical activity, diet quality, and insomnia. Some parameters of clinical interest were not taken into account in the current study: history of psychiatric disorders and use of hypnotics, sedatives, antidepressants and anti-inflammatory drugs, might act as mediators in the association between physical activity and/or diet with insomnia. Physical activity was evaluated with pedometers, and the simple fact of wearing the device may have had an influence on physical activity during the week. The observation duration was relatively short in our study. Futures longitudinal studies should help elucidate the physical activity and sleep quality relationship. The questionnaires used to evaluate insomnia and diet quality were self-administered but were questionnaires that were easy to understand and to complete on the part of the patients; additionally, both have been previously validated in the Spanish population.</p><p id="par0115" class="elsevierStylePara elsevierViewall">In conclusion, completing a higher total number of steps, aerobic steps, and energy expended by walking, could be correlated with a lower presence of insomnia, independently of age, sex, and other confounding variables. However, according to the analysis performed, no relationship was observed between diet quality and insomnia according to this study. This suggests that the results reported here allow for a clear implication for public health: measures to increase levels of physical activity by walking measured with pedometers, could usefully be added to the other tools available for insomnia management. It would be necessary to study the relationships between certain patterns of eating and other types of routinely practiced physical activity and the presence of insomnia.</p></span><span id="sec0095" sleep quality, such as insomnia. The main objective of the present study was to analyze the relationship of the presence of insomnia, with walking physical activity and diet quality in an adult population.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Patients and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A cross-sectional descriptive study was performed (subanalysis of the TRIPLE A study) between January of 2014 to June of 2015. A total of 454 subjects were selected by consecutive sampling. Insomnia was evaluated with the Atenas scale, diet quality using the diet quality index (DQI) questionnaire, and physical activity using pedometers for seven days, evaluating the total number of steps, aerobic steps, and energy expended.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Of all patients, 26.8% presented with insomnia (33.8% in women and 17.5% in men). In a adjusted model, the mean differences of total steps/day, aerobic steps/day, and kilocalories/day spent in performing physical activity, between no insomnia group and insomnia group were: 1022.5 (95% CI: 177.9–1867.0), 743.9 (95% CI: 68.3–1419.4) and 39.8 (95% CI: 5.7–73.9), respectively. Se seleccionó una muestra de 454 sujetos mediante muestreo consecutivo. El insomnio fue evaluado con la escala Atenas, la calidad de la dieta mediante el cuestionario del índice de calidad de la dieta (DQI) y la actividad física mediante podómetros durante 7 días, evaluando el número de pasos totales, pasos aeróbicos y gasto energético.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Presentaron insomnio el 26,8% (el 33,8% en mujeres y el 17,5% en varones). Las diferencias de media de los pasos totales/día, pasos aeróbicos/día y las kilocalorías/día gastadas en la realización de la actividad física, entre el grupo de sujetos sin insomnio y el grupo de sujetos con insomnio fueron: 1.022,5 (IC 95%: 177,9–1.867,0), 743,9 (IC 95%: 68,3–1.419,4) y 39,8 (IC 95%: 5,7–73,9),respectivamente.
