id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Sexually transmitted infections (STIs) are a major public health problem, both because of their magnitude - more than one million people worldwide contract an STI every day, contributing to a total of 376 million new cases per year<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a> — and because of their complications and sequelae without early diagnosis and treatment. Infertility, adverse pregnancy outcomes and cancer are some of the consequences that can result,<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3–8</span></a> not to mention the psychosocial consequences that people diagnosed with STIs can suffer, such as stigma or gender-based violence<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> and the close relationship with the human immunodeficiency virus (HIV).<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,7</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Most EU countries saw a progressive decline in STIs until the early 1990s, attributed, among other factors, to changes in sexual behaviour in response to the emergence of HIV.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> From 1996 onwards, several countries described a resurgence of STIs<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> with numerous outbreaks, mainly affecting young people, men who have sex with men (MSM), heterosexual contacts in prostitution, drug users<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> and the emergence of HIV co-infection. According to the latest available data<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> STIs are still on the rise, with an increase in the number of cases for all STIs in Europe. Thus, from 2015 to 2019 the increase for gonorrhoea, syphilis and lymphogranuloma venereum (LGV) has been 55.0%, 25.3% and 74.8%, respectively. Genital chlamydial infection remains stable, with an increase of 8.4%, but continues to be the most common bacterial STI, with a rate of 157 cases per 100,000 inhabitants. In Spain, according to the notifiable disease (ND) surveillance system, a decrease in the number of cases of syphilis and gonorrhoea had been observed since 1995 and continued until 2001. However, since then, and following the same European pattern, there have been continuous increases in the incidence of both diseases.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> The incidence rates for 2019 were 28.8 and 13.3 cases per 100,000 inhabitants, respectively. However, unlike in Europe, genital chlamydial infection is the STI with the highest increase in recent years in Spain, with a rate for 2019 of 44.2 cases per 100,000 inhabitants, representing 44.7% of the STIs reported for that year.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Similarly, in Catalonia, surveillance data show a significant increase in the number of STIs reported over the last 10 years, with annual average increases of 42.3% for chlamydia, 33.0% for gonorrhoea, 18.7% for syphilis and 23.4% for LGV. For 2019, a total of 17,974 cases of syphilis, gonorrhoea, chlamydia and LGV were reported, an increase of 54.9% from the previous year.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Although the increase in STIs has been attributed in part to improvements in surveillance systems and the use of more sensitive diagnostic techniques, other causes must be considered, such as socio-cultural changes, screening and the rise of risky sexual behaviour. The false perception of protection against HIV following the existence of pre-exposure prophylaxis (PrEP) -available in Spain from 2015-, especially among MSM, and the perceived invulnerability of the young population have reduced awareness of the risks of acquiring these infections. Unplanned sex, low condom use, multiple partners, sex work, recreational drug use during sex and common use of social networks to find sexual partners have been identified as factors contributing to the growing epidemic.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15–18</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Following the World Health Organisation pandemic status declaration on 11 January 2020 of the severe acute respiratory syndrome type 2 coronavirus (SARS-CoV-2),<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> a paradigm shift in interpersonal relationships, a long period of lockdown and the preventive measures taken to curb the spread of the virus, notably social distancing, led to a decrease in STIs. Thus, it was observed that bacterial STIs decreased, but once the restrictive measures were relaxed, the incidence of STIs increased to values even higher than those prior to the pandemic.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The aim of this analysis was to describe the evolution of the incidence and impact of SARS-CoV-2 on reported STI cases in Catalonia during the period 2018–2021.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">The surveillance and control of STIs in Catalonia is governed within the framework of European and national surveillance. New diagnoses of infectious syphilis, gonorrhoea and LGV reported since 2008 and genital chlamydial infection since 2015 are individually notified to the notifiable diseases register for STIs.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> The individualised notification includes the patient's diagnosis and demographic data and is accompanied by an epidemiological questionnaire that includes clinical and behavioural history data.</p><p id="par0035" class="elsevierStylePara elsevierViewall">A descriptive analysis was conducted comparing the impact of the SARS-CoV-2 pandemic on the demographic characteristics and sexual orientation of syphilis, gonorrhoea, chlamydia and LGV notifications received during the pre-pandemic period, defined as the sum of cases in the years 2018 and 2019, and the pandemic period between the years 2020 and 2021.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Study variables</span><p id="par0040" class="elsevierStylePara elsevierViewall">We included the demographic variables of sex, age and country of birth grouped according to whether born in Spain or born outside Spain, and transmission group categorised according to whether they were: MSM and bisexual men, men who have sex only with women (heterosexual men) and women who have sex with men (heterosexual women).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">Annual incidence rates per 100,000 inhabitants were calculated, stratified according to demographic characteristics. The population denominators used to calculate the rates were obtained from the Statistical Institute of Catalonia, according to the municipal census of inhabitants on 1 January of each of the years of the study period. Reported cases of SARS-CoV-2 were obtained from the same source.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Percentage declines were estimated using negative binomial and Poisson regression models. Student's <span class="elsevierStyleItalic">t</span>-test was used for comparison of means.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The influence of the number of positive SARS-CoV-2 cases on the number of positive STI cases during the pandemic months was studied using a correlation model, and the number of expected STI cases for the pandemic period (2020 and 2021) was estimated using the Holt-Wilson time series model.</p><p id="par0060" class="elsevierStylePara elsevierViewall">All data were analysed with R software.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Ethical considerations</span><p id="par0065" class="elsevierStylePara elsevierViewall">Data from the ND system was used for this analysis, governed according to Organic Law 3/2018, of 5 December, on the Protection of Personal Data and Guarantee of Digital Rights<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> and Law 33/2011, of 4 October, General Law on Public Health.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> The patients’ personal data were anonymised prior to the analysis.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">For the period 2018–2021, a total of 6308 STIs were reported. Of the total 54.9% were reported for chlamydia, 31.1% for gonorrhoea, 12.1% for syphilis and 1.9% for LGV. The average annual increase for the period studied was 23.6% for all STIs and 25.4% for chlamydia, 28.0% gonorrhoea, 5.1% syphilis and 30.0% LGV. Incidence rates ranged from 2018 to 2021 from 83.4 to 139.8 cases for chlamydia, from 44.2 cases to 86.5 cases for gonorrhoea, from 23.2 to 25.2 cases for syphilis and from 2.4 to 4.5 cases for LGV. In 2020 chlamydia and syphilis showed a higher reduction than gonorrhoea and LGV compared to the previous year, 22.7%, 20.9%, 9.5% and 2.5%, respectively (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The reported cases of the 4 STIs for the period analysed were studied chronologically by month and a parallel behaviour was observed with increases and decreases in the same pre-pandemic time periods, with a significant decrease in 2020, and especially during the months of March to May (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). After analysing the relationship between reported STI cases and confirmed SARS-CoV-2 cases, no significant correlation was observed (p = 0.8; 95% CI: -0.356, 0.447). However, fluctuations were observed at times of sharp increases in SARS-CoV-2 that coincided with the decline in STIs, especially during the first pandemic wave (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Thus, the overall incidence rate for all STIs in 2020 decreased by 18.3% compared to 2019. During the pandemic months of April and May (2020−2021) STIs decreased by 16.6% and 11.8%, respectively, compared to the pre-pandemic years (2018−2019). If these same data are compared between 2019–2020 and for the same months, it was observed that during the months of April–July 2020 the decrease was more pronounced compared to the same months of the previous year, 72.6%, 61.5%, 36.2 and 29.5%, respectively. From June onwards the values for 2020 were higher than in 2018, but never exceeded the 2019 data for the rest of the year (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">According to the predictive model, if the increasing trend in the number of reported STI cases of previous years had continued in 2020, there would have been 44.6% (26,118) more cases than were actually reported (14,468) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Despite the fact that 2021 saw the highest incidence rates of the period studied, with the exception of syphilis, which showed lower values than in 2019 (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), if we compare the actual cases reported in the 2 periods, pre-pandemic and pandemic, we still observe a reduction in incidence of 13.8% and, in fact, estimates for 2021 placed the number of STIs at 43.9% (35,333) more than those actually reported (19,817).</p><p id="par0085" class="elsevierStylePara elsevierViewall">The proportions by sex, country of birth and transmission group changed significantly (p = 0.000) in chlamydia and gonorrhoea from the pre-pandemic (2018−2019) to the pandemic (2020−2021) period, while no significant changes were observed in the remaining STIs.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The proportion of men diagnosed with chlamydia and gonorrhoea in the pandemic years increased significantly by 15.3% and 5.1%, respectively, compared to the pre-pandemic period, while women decreased by 9.9% for chlamydia and 18.1% for gonorrhoea (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">According to country of birth, the proportion of Spaniards decreased by 9.2% for chlamydia and 3.1% for gonorrhoea, while those born outside Spain increased by 29.4% and 14.6%, respectively (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><p id="par0100" class="elsevierStylePara elsevierViewall">The mean age of people diagnosed with chlamydia and gonorrhoea increased significantly (p = 0.000) by one year with 28 years and 33 years, respectively, in the pandemic period. The mean age at diagnosis of syphilis and LGV remained at 39 and 37 years, respectively.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The proportion of cases by sexual orientation was significantly reduced for chlamydia in HT men by 21.5%, with MSM increasing by 26.6%. For gonorrhoea, the proportion of female and male HT was significantly reduced by 11.3% and 45.6%, respectively, with MSM increasing by 31.0% (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">STIs continue to be a major public health problem, with sustained increases in incidence in recent years. During the months prior to the implementation of preventive measures to reduce the spread of SARS-CoV-2, the number of reported STIs was higher compared to the same months in 2019. The adopted measures of social restriction, restructuring of health services and resources, reduced detection and possible fear of contagion may have led to an initial decrease in STI cases, which probably resulted in a decrease in transmission in a specific period, particularly higher during the initial lockdown compared to the later reopening phases, which in turn led to an under-reporting of cases. However, after some of these measures were lifted, STIs increased, following the trend observed in recent years, and with incidences in 2021 higher than the pre-pandemic values, a trend in line with that described in other autonomous communities and in other European countries or in the USA,<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24,25</span></a> despite the fact that, according to estimates, 43.9% fewer-than-expected STIs had been reported in 2021. The release of social restrictions and the gradual increase in medical consultations that had been delayed or suspended due to the pandemic would explain this trend.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Although the pandemic period saw initial declines in all STIs, deficits were more significant for chlamydia, as described in Italy, the USA or Australia.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24–26</span></a> The mostly asymptomatic course and reduced testing of this infection during the pandemic probably meant that it was affected disproportionately to the rest, resulting in undiagnosed infections. Thus, our results also show a significant reduction in the proportion of women diagnosed with chlamydia and gonorrhoea in the pandemic years compared to men, as observed in a study conducted in Valencia,<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> confirming that the decrease in routine gynaecological visits, with a reduction in STI screening in the female population, and the increased symptomatology of chlamydial or gonorrhoeal urethritis in men may have been part of the reason for this change. Consistent with other data, STIs with significant symptoms, such as gonococcal urethritis in men or LGV, showed reductions of less than 10%, suggesting that people presenting with more severe clinical conditions sought medical care during the pandemic,<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">24–26</span></a> and indeed, according to our data, people diagnosed during the pandemic period had 18.9% more symptomatology than during the pre-pandemic period.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Contrary to what happened in other countries, where STIs occurred in younger populations, probably due to a sense of invulnerability to SARS-CoV-2, given that the concentration of morbidity and mortality occurred in older people,<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> our data show significant increases in the mean age for chlamydia and gonorrhoea in the pandemic period, presumably attributed in part to the increase in cases involving HT men and MSM, who in our setting are older than the female population diagnosed with an STI.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">Therefore, the apparent reduction in STIs was due to an epidemiological shift as a result of social distancing, restrictions on mobility, a decrease in the number of patients attending specialised clinics and a decrease in screening tests, which reduced the likelihood of contracting these infections.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27–29</span></a> Despite the restrictions posed by the pandemic, some populations such as MSM maintained high-risk sexual behaviours, and in line with other studies<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a> were diagnosed with these STIs during the pandemic period perhaps because of previous sexual relationships, or because they felt at lower risk of complications if they acquired SARS-CoV-2 during close contact.</p><p id="par0130" class="elsevierStylePara elsevierViewall">After a year of pandemic and severe restrictions, risky sexual behaviour is likely to have increased and, consequently, there has been an upturn in STIs with sustained increases in incidence following the pandemic. The influence of the number of positive cases of SARS-CoV-2 with the number of positive cases of STIs during the months of the pandemic was studied using a correlation model. Using the Holt-Wilson time series model, an estimate was made of the number of STI cases expected for the pandemic period.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">The global incidence rate for all STIs in 2020 decreased by 18.3% compared to 2019.
