"apellidos" => "del Carpio Orantes" "email" => array:1 [ 0 => "neurona23@hotmail.com" ] ] ] "afiliaciones" => array:1 [ 0 => array:2 [ "entidad" => "Departamento de Medicina Interna, Hospital General de Zona 71, Instituto Mexicano del Seguro Social, Delegación Veracruz Norte, México" "identificador" => "aff0005" ] ] ] ] "titulosAlternativos" => array:1 [ "en" => array:1 [ "titulo" => "Guillain-Barré syndrome associated with zika virus infection in the Americas: A bibliometric study" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Realizamos un estudio bibliométrico extenso sobre cohortes de estudio del síndrome de Guillain-Barré asociado a zika, investigando en diversos buscadores como Scopus, Medline-Pubmed, Web of Science, Science Direct, Scielo e Imbiomed, entre otros.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Se seleccionan los estudios realizados en América, se agrupan acorde al país y al número de casos reportados, se evalúa la metodología empleada respecto de búsqueda de agentes infecciosos (seleccionando aquellos que buscaron zika como punto principal y otros agentes en forma secundaria —virales y bacterianos—), así como se analiza el porcentaje de casos positivos a zika, la incidencia global o promedio y el patrón de neuroconducción reportado. Se excluyen los reportes de casos y las series de casos menores a 5 pacientes y aquellas cohortes que no reportaron serologías para zika.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Se encontraron 22 estudios que reportan cohortes de síndrome de Guillain-Barré asociado a zika entre los años 2014 a 2017, con número de casos variables. Brasil y Colombia destacan con 4 y 6 estudios respectivamente, Puerto Rico y México con 3 estudios cada uno, Venezuela, Jamaica, Granada y Martinica con un estudio cada uno. Existen 2 estudios multicéntricos con participación de países de Centroamérica y Sudamérica (<a class="elsevierStyleCrossRef" href="#tbl0005">tabla 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">De los estudios que reportan positividad a zika en todos los casos de Guillain-Barré uno es de Brasil y otro de Colombia, en contraparte otro estudio brasileño reportó 0 casos positivos de 41 casos estudiados. Los demás estudios demuestran incidencias variables.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Hacia Centroamérica y el Caribe la incidencia disminuye, pero la mayoría de estudios reportan incidencia por encima del 50% y hacia Norteamérica, específicamente México, la incidencia de casos positivos decae notablemente hasta ser nula la asociación de Guillain-Barré y zika.</p><p id="par0030" class="elsevierStylePara elsevierViewall">La incidencia promedio de infección por zika en las cohortes del síndrome Guillain-Barré es del 42%.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Se encontró un metaanálisis que incluyó 3 grandes estudios en Sudamérica, con los cuales estimó la prevalencia del síndrome de Guillain-Barré asociado a zika en el 1.23%<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a>.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Respecto del patrón neurofisiológico estudiado destaca que la mayoría, 12 estudios, son polineuropatías inflamatorias desmielinizantes agudas (AIDP). el patrón axonal motor (AMAN) fue reportado en dos estudios (Brasil y Colombia) y un estudio realizado en Venezuela reportó el patrón axonal sensitivo-motor (AMSAN) como el de mayor predominio.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Los estudios serológicos varían, existen cohortes a las que solo se les estudio zika (7 estudios), 5 cohortes estudiaron los 3 principales arbovirus (zika, dengue y chikungunya), 3 cohortes solo estudiaron dengue y zika, otras cohortes buscaron agentes virales y bacterianos diversos, destacando en estos últimos la búsqueda de <span class="elsevierStyleItalic">Campylobacter</span> y TORCH.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Concluimos que la relación causal directa de zika y los casos de Guillain-Barré es variable y con resultados dispares incluso en la misma región geográfica lo que no permite establecer una relación causal fehaciente. Se observa que conforme la oleada de zika fue colonizando territorios hacia Centroamérica y el Caribe, la incidencia de zika en los casos de Guillain-Barré disminuía, hasta ser casi nula al llegar a Norteamérica.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Respecto del patrón neurofisiológico predomina AIDP, el cual es el principal implicado en procesos infecciosos y de mayor reporte en Europa y Norteamérica, sin embargo en algunas regiones como México, Centroamérica y Sudamérica, el patrón previamente reportado era AMAN, el cual no fue encontrado en estas cohortes específicas.
