Las osteotomías distales del primer metatarsiano han sido indicadas en el tratamiento quirúrgico del hallux rigidus moderado. La finalidad de estas osteotomías es realizar un descenso de la cabeza metatarsiana pero a expensas de un acortamiento del mismo, lo que podría ocasionar en algunas circunstancias una metatarsalgia de transferencia.
Este trabajo muestra la modificación de una osteotomía distal oblicua del primer metatarsiano, basada en las descritas por Weil-Barouk y Ronconi, con el objetivo de conseguir un descenso importante pero controlando y evitando un excesivo acortamiento del primer metatarsiano. Presentamos sus indicaciones, contraindicaciones, técnica quirúrgica y las consideraciones sobre sus ventajas y desarrollo.
Distal osteotomies of the first metatarsal have been indicated in the treatment of moderate hallux rigidus. The aim of these osteotomies is to perform a descent of the metatarsal head, but at the expense of a shortening of the first metatarsal bone, which could cause transfer metatarsalgia in some cases.
This paper shows the modification of an oblique distal osteotomy of the first metatarsal, based on Weil-Barouk and Ronconi osteotomies, with the aim of achieving a significant controlled descent and preventing excessive shortening of the first metatarsal. We present indications, contraindications, surgical technique, discuss considerations and advantages.
