However, it is not without potential serious complications that need to be understood. We present the case of a 37-year-old patient with no history of interest who had undergone a laparoscopic myomectomy. During pneumoperitoneum following anaesthesia induction without orotracheal intubation, the patient suddenly presented desaturation (SpO2 80%) and haemodynamic instability (BP 60/32mmHg, HR 45bpm), with rapid recovery after release. A second attempt at gas insufflation produced the same effect, so it was decided to switch to open surgery. The intervention and the immediate postoperative period were uneventful. The chest X-ray ordered in the postanaesthetic recovery unit revealed a hypodense line in the pericardium, bordered by a second hyperdense line, compatible with pneumopericardium (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). This is a monthly journal (10 issues) that publishes scientific articles of all areas within its specialty: clinical anesthesia, resuscitation-intensive medicine and critical care, diagnosis and treatment of acute and chronic pain, urgent care and emergencies and studies on basic science and related fields. The journal accepts work in both Spanish and English.
