Evidence has shown that the most common medical errors are associated with human factors, and can impact patient morbidity and mortality<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>. Uramatsu et al. found that 46% of fatal medical errors can be attributed to poor NTSs, and only 5% to substandard TSs<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>. According to the 4th UK national audit project (NAP 4), 40% of complications associated with advanced airway management during anaesthesia are due to poor NTSs<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Various instruments for evaluating NTSs are now available. The Ottawa Crisis Resource Management (CRM) Global Rating Scale (Ottawa GRS), developed in English by Dr. John Kim at the University of Ottawa, assesses competencies in crisis management skills in critically ill patients. The rating scale is divided into 5 categories of essential CRM skills based on recognized CRM literature—problem solving, situation awareness, leadership, resource utilization, and communication, each of which is rated on a numerical scale of 1–7, with 7 being the highest score and 1 the lowest. The Ottawa GRS also includes an overall CRM performance score that uses the same numerical scale. The scale does not standardize the numerical values found in qualitative categories and does not stipulate the length of training required. However, studies in other languages have shown its construct validity in multiple domains, inter-rater reliability and discriminatory capacity, and the instrument has been used for longitudinal evaluation of NTSs<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> and in academic programmes to evaluate the basic skills of trainees<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>. The Ottawa GRS is not limited to specific areas of medicine, and can be extrapolated to different clinical settings. It provides descriptions of the behaviours and actions associated with each category, and its simplicity, ease and rapidity of use, and greater flexibility in allocating scores for CRM performance make it the scale of choice for most raters<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">In other countries, Repo et al. validated the translation, cross-cultural adaptation, inter-rater reliability, and internal consistency of NOTECHS scale for trauma in Finnish and tested its construct validity using factor analysis<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>. Pereira et al.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> validated the internal consistency and structural validity of their NTS scale in nursing.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In Colombia, as there are no validated scales in Spanish for healthcare personnel in emergencies, we validated the Ottawa GRS scale in Spanish as a tool for rating NTSs.</p><p id="par0025" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Materials and methods</span>. We performed a cross-sectional study to validate the cross-cultural adaptation of the Ottawa GRS scale in a Spanish-speaking population aged over 18 years of age. The subjects recruited were fourth-year undergraduates, general practitioners, medical and surgical speciality residents and qualified specialists, leaders of clinical scenarios in charge of making critical health decisions.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Sociodemographic variables were analysed using descriptive statistics, continuous variables were summarized as medians and interquartile ranges, and qualitative variables as frequencies and percentages.</p><p id="par0035" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Procedure</span> We contacted the group that developed the scale at the University of Ottawa, led by Dr. John Kim, and obtained permission to use it for this purpose. The research was approved by the Ethics Committee and the Simulation Centre of the Faculty of Medicine of the University of Antioquia, Medellín-Colombia.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The Ottawa GRS was translated and adapted to Spanish by a review committee. The scale was translated from its original language into Spanish independently by a simulation specialist and a native English translator. A third translator, expert in NTSs in simulation scenarios, synthesized both translations into a single Spanish version. Following this, a reverse translation was carried, in other words, the scale was translated into the original language by a certified translator blinded to the content of the original scale (Appendix B see annex 1 and annex 2).</p><p id="par0045" class="elsevierStylePara elsevierViewall">Participants were given access to the moodle platform where they took a pre-test and post-test on the medical emergency used (anaphylaxis, Appendix 3) that required a score of 100% in TSs. They later attended a course at the simulation centre where they were first shown 3 videos we had made to show both the emergency and the performance (excellent, medium and poor) of NTSs. The content of the videos was similar to that used in the initial study conducted by Dr. Kim's group, the aim being to standardise the training of NTS raters and train the participants in the simulation scenarios. Then, in the simulation room, the participants were filmed leading the management of a medical emergency. These videos were then used to apply the scale. Next, 3 totally independent experts rated each team leader on the basis of the 3 aforementioned videos.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Evaluation of psychometric properties</span><p id="par0050" class="elsevierStylePara elsevierViewall">The sample size was too small to allow us to perform principal component analysis.</p><p id="par0055" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Internal consistency:</span> degree to which items are correlated within the scale<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>. With a type I error of 0.05, an expected Cronbach's alpha correlation coefficient score of 0.7, and a confidence interval of 0.2, we estimated that a sample size of 90 participants would be needed to evaluate 6 dimensions of the NTS scale, following Bonett’s two-stage formula.</p><p id="par0060" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Inter-rater reliability:</span> the degree of agreement among independent observers who rate the same phenomenon<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>. Three raters independently applied the scale to each of team leaders in the simulated scenarios. None of the raters participated in the simulation scenarios. We assumed a type I error of 0.05, a type II error of 0.20 to test a null hypothesis intraclass correlation coefficient (ICC) of 0.5, and an alternate hypothesis correlation coefficient of 0.7, the sample for 3 raters was 67 participants.</p><p id="par0065" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Test–retest reliability:</span> correlation of the scores obtained after administering the same scale twice at different times<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>. Assuming a type I error of 0.05 and a power of 0.80, a high test-retest ICC of 0.7, and an accuracy of 0.2, a sample size of 41 participants was required. One month after the simulation exercises had finalised, a subsample of 41videos was chosen and re-evaluated by 2 of the initial raters.</p><p id="par0070" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Convergent and divergent construct validity:</span> degree to which the scale measures the construct it intends to measure<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a>. Two hypotheses were tested:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0075" class="elsevierStylePara elsevierViewall">Convergent: scores correlate with those of other instruments that measure the same construct<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>. The Non-Technical Skills scale (NOTECHS) was administered to 15 of 91 participants. This scale assesses the NTSs of teamwork in the operating room, measuring the approach to teamwork and the rate of surgical errors.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0080" class="elsevierStylePara elsevierViewall">Divergent: scores do not correlate, or correlate poorly, with those obtained by instruments that evaluate different constructs<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a>. Correlation with the Airway Management Skills in Anesthesia Trainees (AMSAT) scale tested in 15 of 91 participants. The aim in both constructs was to evaluate 65 participants; however, due to protocol violation, evaluations were only obtained from 15 participants.</p></li></ul></p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0085" class="elsevierStylePara elsevierViewall">A total of 97 participants were recruited over a period of 10 months. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the recruitment flow chart. A total of 91 individuals were included in the study after 6 had been excluded for logistical reasons (poor audio or unsatisfactory video resolution). Fifteen videos were evaluated using the NOTECHS and the AMSAT scales, and not 65 as stated in the protocol, due to incorrect protocol violation. Once we detected the error, the independent raters were no longer able to able to continue the evaluation. Only 2 raters re-evaluated the videos.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">Participant characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Over half (57%) were men, 61.5% were final-year undergraduates, 28.6% were anaesthesiology residents, and only 2.2% were specialists; the average age was 27 years, and 85% of participants had 1–5 year’s experience in their specialty area.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the raw scores and T scores for each domain of the scale. <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the percentage of scores awarded to each domain of the scale by the 3 raters. None of the domains received scores of between 1 and 2; therefore, there was no floor effect. Most performances scored between 3 and 7, the most frequent for the 2 rates being scores of 4 and 5; therefore, there was no ceiling effect (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Internal consistency</span>. Internal consistency achieved a Cronbach's alpha of 0.954, showing an excellent correlation between all 6 domains of the scale.</p><p id="par0105" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Inter-rater reliability.</span> The ratings from all videos by 3 independent experts achieved an ICC of between 0.5 and 0.7 for all domains; reliability, therefore, was moderate. The domain with the lowest ICC was communication skills (0.587) and the one with the highest was resource utilisation in an emergency situation (0.696). <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows all ICCs.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Test-retest reliability</span>, performed by two raters, achieved an ICC of over 0.7 (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>). Bland and Altman plots to test the limits of agreement between 2 raters showed high agreement (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Construct validity. Convergent construct</span>. As shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>, convergent construct was acceptable for all domains, being greater than 0.5 for resource utilisation and situation awareness, and >0.7 for the other domains. <span class="elsevierStyleItalic">Divergent construct</span> was found to be between 0.3 to 0.48, suggesting a correlation in the constructs of the TS scale for orotracheal intubation and the NTS scale adapted to Spanish.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">Interest in NTSs and their impact on patient safety has grown in recent years. Rogers et al. showed that poor NTSs contributed to 82% of surgery-related malpractice claims<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a>. Despite this, these is still no objective instrument in Spanish for evaluating NTSs; hence the importance of this study, which is the first evaluation of the translation, cultural adaptation and validation of the Ottawa GRS scale in Spanish.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The translation into Spanish and the cultural adaptation were based on the translation protocol recommended for the Thai<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> and Italian<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> translations of the same scale, thus allowing our study to be compared with other similar publications. Our sample size (n = 91) is greater than that used in the original validation (n = 60)<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> and also in subsequent studies<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,13</span></a>. The original study included only residents, while our study included fourth-year medical undergraduates, general practitioners, residents, and specialists, thus showing the validity of the scale for different levels of training. It is also important to note that the validation of instruments to measure NTSs worldwide is mainly based on surgeons and anaesthetists, and was subsequently extended to include endoscopic procedures and emergency medicine<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,16,17</span></a>.</p><p id="par0130" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the distribution of performance scores, which were all intermediate or high. This is probably because most of the participants were fourth-year medical undergraduates who were familiar with simulation scenarios and emergency resource management both in their place of work and in the simulation centre of the Faculty of Medicine of the University of Antioquia, which indirectly and indirectly trains clinicians in NTSs. All participants had also received training in TSs and NTSs in an anaphylaxis crisis, and had seen the videos made by the researchers before the simulation exercise. We believe that the intermediate or high performance scores achieved by all participants (the absence of scores of 1 and 2) is due to their high level of training and not to poor calibration of the instrument. However, in subsequent studies it may be advisable to explore the use of this scale in totally untrained personnel, and thus evaluate its ability to discriminate between different groups. In the original study, however, there were no statistically significant differences between the scores achieved by first and third year residents<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>. Further studies are also needed to evaluate the scale in different populations, since the inclusion of a wider range of clinicians will optimise comparisons between groups. The samples used in the original study and others performed thereafter have been limited to residents<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,15</span></a>.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Internal consistency of our scale was adequate, with a Cronbach's alpha of 0.954, similar to the 0.96 reported by Jirativanont et al.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">The mean inter-rater intraclass correlation coefficient was 0.637, showing moderate reliability. This was the same value reported by Jirativanont et al.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>, but higher than that reported by Nunnink et al.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> and in the original study, where the values were 0.59 for first-year residents and 0.61 for third-year residents, respectively<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a>. The inter-rater reliability score for communication and resource utilisation was also lower than that observed in our series<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>. This correlation can be explained by the complexity of evaluating NTSs in a team dealing with an emergency, as this affects the rater’s ability to evaluate the team’s performance. For this reason, Dr. Heimreich recommended that training in NTSs should last 2 days<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a>, but this recommendation has not been followed in any NTS validation studies. Another reason for this finding is that each domain of the scale has 2 possible numerical scores that cannot be differentiated; for example, the same statement can give a rating of 5 or 6, without no clear separation between them.</p><p id="par0145" class="elsevierStylePara elsevierViewall">When evaluating the test-retest reliability we made sure that the time interval between both evaluations was sufficient to avoid fluctuations due to chance. In other validation studies, performance was evaluated by only 2 raters<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>. The test–retest correlation in this study is moderate, and is comparable to the results of other publications. As the Blan–Altman plots show, reliability was adequate, with no significant deviations in any of the subscores. However, given the dichotomy of the scores in each domain of the original scale, reliability could in fact be higher. For example, in both evaluations it was possible to award the same score on the basis of the textual statement, but this score could be 5 initially, and 6 the following month. Similarly, in line with Kim et al.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>, we observed the "dove/hawk" effect in the test-retest reliability results, in other words, one rater is consistently more indulgent (rater 2) while the other is consistently stricter (rater1). This suggests that the test-retest variability is mainly due to the rater’s training.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The intraclass correlation coefficient is used because it uses a random effects model that takes into account the variance in the retest or inter-rater evaluation, thus avoiding the systematic error of other Pearson correlation tests.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Our results correlate with those of Jirativanont et al., in which the ICC for all domains was greater than 0.8. According to the foregoing authors, the Ottawa GRS scale is easy to use, practical, and more reliable than the Anesthesiologists’ Non-Technical Skill (ANTS) scale<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The convergent construct validity showed good correlation, which means that the construct is similar in both scales. The divergent construct values range from 0.3 to 0.48, showing that the constructs are correlated. According to the foregoing authors, it not possible to separate knowledge and technical skills completely from the assessment of non-technical skills, as it is difficult to appreciate the TS when evaluating the non-technical performance; therefore, a different comparative scale should have been used to assess the divergent construct. Furthermore, prior TS training in the simulated emergency could undermine the external validity of the study, because this eliminated the surprise factor and with it the possibility of evaluating the participants’ skill in coping with an unknown situation. In this regard, Brunckhorst et al.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> concluded that all individual NTS components were strongly correlated with all TS parameters, regardless of the level of surgical training. Krage et al.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> showed that both NTSs and TSs are affected when external stressors are present.</p><p id="par0165" class="elsevierStylePara elsevierViewall">Our study has several strengths: it is the first to validate the Spanish translation of the Ottawa GRS scale; the sample size is greater than that used in other studies; participants received prior training in the simulated emergency; each participants technical knowledge was tested before the simulation; performance was evaluated on the basis of video recordings; the scale was translated by 3 experts, who also validated its cross-cultural adaptation. In addition, the performance of the participants was rated by anaesthesiologists working in various hospitals in Colombia. These raters were trained in NTSs but were not involved in the simulated activity and had no prior experience in rating scales. This increases the external validity of the scale without limiting its use to a particular speciality<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">21,22</span></a>. As the raters in our study were unfamiliar with the scale used, we can safely conclude that it can be used by any clinician. However, we believe that further research is needed, above all studies aimed at perfecting the strategy used to score each domain of the scale.</p><p id="par0170" class="elsevierStylePara elsevierViewall">This study also has some limitations: the convergent and divergent construct was validated on the basis of just 15 videos, and not 65 as stated in the protocol. The ANTS scale was not optimal for evaluating divergence, and only 2 raters, not 3 as initially stated in the study protocol, completed the reevaluation of the videos.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusion</span><p id="par0175" class="elsevierStylePara elsevierViewall">The translation, cultural adaptation and validation into Spanish of the Ottawa GRS scale showed adequate internal consistency, inter-rater and test–retest reliability, showing that the translated version of the scale can be used to evaluate the NTSs of Spanish-speaking health personnel, and as such can provide an objective measurement of the role played by human factors in fatal medical outcomes. Subsequently, the scale was applied to 91 participants. An internal consistency (Cronbach's alpha) greater than 0.9 was found in each dimension. The level of inter-rater reliability, evaluated by the interclass coefficient was 0.59 to 0.69, and test-retest reliability with an interclass coefficient greater than 0.7.
