The patient was evaluated for staging by means of F-18 fluoro-2-deoxyglucose (<span class="elsevierStyleSup">18</span>F-FDG) positron emission tomography and computed tomography (PET/CT). PET/CT showed hypermetabolic giant mass lesions (SUVmax: 17.56), of which the largest was 120<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>145<span class="elsevierStyleHsp" style=""></span>mm<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>85<span class="elsevierStyleHsp" style=""></span>mm in size, filling the right supraclavicular area and posterior triangle of the neck as well as hypermetabolic lymph nodes in the bilateral cervical chain and under the skin of the right chest wall (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A, B). SUVmax values of the lesions supported malignancy. The patient received chemotherapy as the results of histopathological examination of the surgical excision material were consistent with malignant pilomatrixoma (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). PET/CT after chemotherapy revealed hypermetabolic lesions suggestive of residue/relapse (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>C, D), whereas PET/CT after radiotherapy revealed progression (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>E, F). Hypermetabolic lesions detected on PET/CT after chemotherapy and radiotherapy were histopathologically consistent with malignant pilomatrixoma, thus the diagnosis was confirmed.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">While PET/CT is widely used in head and neck cancers, there is limited number of studies reporting the use of <span class="elsevierStyleSup">18</span>F-FDG PET/CT in pilomatrixoma and malignant pilomatrixoma, which is quite uncommon. The diagnosis of pilomatrixoma and discrimination between benign and malignant lesions are made histopathologically. <span class="elsevierStyleSup">18</span>F-FDG is not specific for malignant pathologies as <span class="elsevierStyleSup">18</span>F-FDG uptake is also seen in benign pathologies. For this reason, it may not be reasonable to use <span class="elsevierStyleSup">18</span>F-FDG PET/CT in making the diagnosis of pilomatrixoma as well as in making discrimination between benign and malignant lesions. However, <span class="elsevierStyleSup">18</span>F-FDG PET/CT plays a critical role in determining the biopsy site, in staging, and in detecting relapse/residual disease. Contribution of <span class="elsevierStyleSup">18</span>F-FDG PET/CT to the staging, patient follow-up and determination of biopsy site in the cases with suspicious relapse/residual disease has been demonstrated in cases with malignant pilomatrixoma, which is an uncommon condition and whose aggressive forms have been reported as in the present case, in contrast to pilomatrixoma not showing local recurrence after total excision.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Pilomatrixoma is an uncommon, superficial tumor of the skin arising from the hair follicle matrix. It is also called as calcified epithelioma of Malherbe. It may either be benign or malignant.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> Malignant pilomatrixoma is a very rare condition but may be fatal.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> Malignant pilomatrixoma as well as pilomatrixoma is localized in the head and neck region particularly in the posterior triangle of the neck, upper back and preauricular area. It displays tendency to grow through the deep planes, aggressive progression and local recurrence. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
