She had a 6-month history of KRAS-mutant rectal adenocarcinoma treated with neoadjuvant chemoradiotherapy and colostomy. Physical examination revealed a painless, fixed and rubbery 4<span class="elsevierStyleHsp" style=""></span>í<span class="elsevierStyleHsp" style=""></span>4<span class="elsevierStyleHsp" style=""></span>cm mass over the right frontal area. Pre- and post-gadolinium enhanced sagittal T2-weighted MR imaging demonstrated a soft-tissue mass with intracranial extent, dural involvement and permeative osteolytic lesion of the frontal bone (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A and B).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">18</span>F-fluorodeoxyglucose (FDG) PET/CT was requested for staging. Encephalic PET/CT revealed intense doughnut-shaped increased FDG activity in the frontal region (SUVmax 19.76) in correspondence to the MR imaging (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>A•C). Whole-body PET/CT images demonstrated increased FDG activity in the rectal cancer (SUVmax 9.94) and in a segment VI liver metastasis (SUVmax 4.18) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>D•F). No other bone metastases were observed. A carefully review of the CT component of the FDG PET/CT revealed also 2 small nodules in the lower left lung with no perceptible FDG uptake. At surgery the frontal lesion was only partially resected. Pathological examination reported bone and dura mater metastases from rectal adenocarcinoma. Unfortunately, the patient passed away one month later.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">The differential diagnosis for a tissue mass on the skull includes primary and metastatic neoplasms, inflammation, congenital origin and secondary to previous trauma.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> The most frequent primary tumors metastasizing to skull are breast, lung, prostate, kidney, thyroid, and melanoma.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> Other tumors include liver, stomach, colon, rectum, lymphoma, multiple myeloma, and neuroblastoma. The reported incidence of bone metastases from colorectal cancer ranges between 8.6% and 23.7% in autopsy cases.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a> Different mechanisms for colorectal cancer spread to the bone have been described, including hematogenous extension through Batson's venous plexus, lymphatic route, tumor emboli from secondary lung tumor deposition, and embolism from liver metastasis and subsequent lung metastasis.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">2</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Solitary bone metastasis from colorectal cancer is an uncommon event.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a> This clinical condition has been also reported in follicular thyroid carcinoma, non-small cell lung cancer, signet-ring cell gastric carcinoma, hepatocellular carcinoma, and clear cell renal carcinoma. MR imaging and CT findings in skull metastases are well described in the literature.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">1</span></a> Usually, skull metastases are multiple and always associated to metastases in other skeletal sites or in lung and liver. When local clinical symptoms are not presented, calvarium is usually excluded in FDG PET/CT procedure. Nevertheless, in our patient the major symptom was a soft-tissue mass in the calvarium. Thus, cranial FDG PET/CT, added to conventional whole-body scan, demonstrated an impressive image of the solitary frontal bone metastasis, mimicking a miner's cap. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
