One year later, she presented a vaginal recurrence which showed perineural infiltration in the surgical piece.</p><p id="par0010" class="elsevierStylePara elsevierViewall">After some time, she consulted for intense low back pain with neuropathic characteristics, irradiated to the left lower limb. The pain did not respond to symptomatic treatment and progressed, associating weakness and unsteady gait. Due to this evolution, imaging tests were performed (PET/CT and MRI). The observed findings suggested tumoral infiltration of the left sciatic nerve (see <a class="elsevierStyleCrossRefs" href="#fig0005">Figs. 1 and 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Ultrasound-guided biopsy confirmed the diagnosis, and the pathological anatomy result was: fibrous connective tissue infiltrated by adenocarcinoma with foci of necrosis, consistent with clinical suspicion of gynecological origin (previous endometrioid adenocarcinoma).</p><p id="par0020" class="elsevierStylePara elsevierViewall">Isolated metastatic involvement of a peripheral nerve trunk is a rare phenomenon among the possible mechanisms of tumor dissemination, including direct extrinsic infiltration and perineural dissemination.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> The appearance of peripheral neuropathy may indicate nerve tumor invasion, radiation damage, or other inflammatory neuropathies.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">In case of high clinical suspicion of tumoral nerve involvement (especially if the MRI is inconclusive), a PET/CT should be performed. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
