One-sample Kolmogorov–Smirnov test for normal distribution accepted normality (<span class="elsevierStyleItalic">p</span>: 0.085).</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Glomerular filtration rate (GFR) is defined as the volume of plasma that is cleared of a substance by the kidney(s) per unit time and it is considered the best overall measure of the kidney's functions.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a> Thus, it is not possible to directly measure the GFR in humans, but it can be measured indirectly as the clearance of exogenous filtration markers (mGFR) or estimated from serum levels of endogenous filtration markers (eGFR). mGFR is the best overall parameter for assessing renal function but both mGFR and eGFR are associated with error in their determination. Inulin clearance is the gold standard for GFR determination, but its widespread clinical utilization is limited by the requirement for constant intravenous infusion and timed collections of urine samples.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">2</span></a> Therefore, the availability a simple and accurate method for GFR measurement remains a problem.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Various nuclear medicine techniques to measure the GFR have evolved over a period of time for the need to identify early decline in kidney function and some nuclear medicine techniques using technetium-99m (<span class="elsevierStyleSup">99m</span>Tc) labeled diethylene triamine penta-acetic acid (DTPA) are in use in clinical practice. Renal DTPA clearance can be determined from the measurement of activity in single or multiple blood samples, from the rate of removal of activity from blood or tissue, from the rate of appearance of tracer in urine, and from the rate of renal tracer uptake.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">3</span></a> Although, the plasma sample-based methods are accurate and give close results to the gold standard of inulin clearance, they are not practical in daily routine use and large-scale screening or routine monitoring of patients with chronic renal disease due to venous sampling over several hours, blood handling and gamma counting. On the other hand, gamma camera-based methods using <span class="elsevierStyleSup">99m</span>Tc-DTPA without blood or urine sampling introduced by Gates for the first time have been the most common method in the routine setting. It is commercially available in virtually all gamma camera software packages,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">4</span></a> because of technical simplicity and requirement for less time for the patients. Although, the Gates’ method is widely used, the diagnostic accuracy of the gamma camera methods is still under debate and increasingly questioned.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">5–7</span></a> Therefore, simple and accurate determination of the GFR is still a clinical problem.</p><p id="par0015" class="elsevierStylePara elsevierViewall">With this perspective, the present study was designed to develop a simple camera-based approach to measure the GFR without blood sampling and to compare our results with those of Gates’ method and other mGFR and eGFRs derived from available measurements and equations used in daily clinical practice.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study population</span><p id="par0020" class="elsevierStylePara elsevierViewall">Thirty-four patients (pts) who had creatinine clearance (CC) calculated the week before the renal perfusion scintigraphy (RPS) was done were included into this prospective study. Only one patient of the study population was a woman (64 years) and the remaining others were men (mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD: 33<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14 years). The patients in the study group had unilateral or bilateral pelvicaliceal dilation on their ultrasonographic examination and they were referred to our department for RPS. A structured interview and clinical history was done. The patients with renal fusion anomalies were excluded from the study not to be calculated split renal functions. Patients who had renal transplantation were also excluded from the study due to ethical reasons. All patients were informed about the procedure and written informed consents were obtained before the test. The hospital ethics committee approved the study protocol.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Technique, acquisition protocol and instrumentation</span><p id="par0025" class="elsevierStylePara elsevierViewall">It has been reported that appreciable <span class="elsevierStyleSup">99m</span>Tc-DTPA has already left the intravascular space even after 4<span class="elsevierStyleHsp" style=""></span>min of the administration.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">8</span></a> Actually, the <span class="elsevierStyleSup">99m</span>Tc-DTPA clearance curve from the calf after intra-arterial injection has been reported to be biexponential and the first exponential corresponded to the washout of unextracted tracer and generally lasted 2–5<span class="elsevierStyleHsp" style=""></span>min.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">9</span></a> Under the light of this data, we have infused <span class="elsevierStyleSup">99m</span>Tc-DTPA in order to simulate creatinine secretion for 5<span class="elsevierStyleHsp" style=""></span>min. Oral hydration before the study was suggested to all patients for volume expansion. A venous catheter was inserted to all patients for multiple percutaneous injections. The dynamic imaging was obtained with a posterior projection of the patient while the patient was in the supine position on the imaging table. The imaging session of the patients was divided into two parts, and thus the GFR was calculated in the first part and regular RPS was performed in the second part. For the first part, while ∼74<span class="elsevierStyleHsp" style=""></span>MBq (2<span class="elsevierStyleHsp" style=""></span>mCi) of <span class="elsevierStyleSup">99m</span>Tc-DTPA (TechneScan, Mallinckrodt, USA) in 100<span class="elsevierStyleHsp" style=""></span>cc saline solution was infused intravenously to the patient during 5<span class="elsevierStyleHsp" style=""></span>min by using a infusion pump (Alaris, SE Pump, CareFusion, UK), a dynamic acquisition (1<span class="elsevierStyleHsp" style=""></span>sec per frame/5<span class="elsevierStyleHsp" style=""></span>min) was initiated to evaluate the renal handling of the radiopharmaceutical by using a gamma camera (Philips Forte Jet STREAM, Holland) fitted with LEHR collimators when activity was detected in the abdominal aorta. A 15% window was centered over the 140-keV photo peak. Data were stored in a 64<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>64 matrix, and 1.33 zoom was applied. The infusion set including <span class="elsevierStyleSup">99m</span>Tc-DTPA was counted under the gamma camera 30<span class="elsevierStyleHsp" style=""></span>cm away from the detector before and after the administration of the radiopharmaceutical in order to calculate the injected counts to the patients. Shortly after, for the second part in the same session without patient movement, serial 60-s images were acquired for 45<span class="elsevierStyleHsp" style=""></span>min after the intravenous injection of ∼111<span class="elsevierStyleHsp" style=""></span>MBq (3<span class="elsevierStyleHsp" style=""></span>mCi) <span class="elsevierStyleSup">99m</span>Tc-DTPA to perform the regular RPS including a 1-min flow study for regular reporting the perfusion, uptake and excretion functions of the kidneys. The prediuretic and postdiuretic phases were acquired as a single dynamic study starting immediately after the injection of the radiopharmaceutical and continued for 20<span class="elsevierStyleHsp" style=""></span>min after the injection of furosemide at 25<span class="elsevierStyleHsp" style=""></span>min. This second part of the imaging session was acquired to evaluate and report the kidney functions of the patients and these data were not used for the research purpose except Gate's automatic GFR calculation.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The reason why we infused the radiopharmaceutical to the patients was to provide constant activity pass through the nephrons by simulating in vivo creatinine secretion, and therefore, the shape of the resulting time–activity curve derived from the first phase of the study will be in a linear style (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). On the other hand if you inject the radiopharmaceutical in bolus fashion, the shape of the increasing part of the time–activity curve derived from the second phase will be in an exponential style (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Image evaluation</span><p id="par0035" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">99m</span>Tc-DTPA is a hydrophilic solute with a perfusion-dependent transfer rate across endothelium and the dispersion rate as a fraction of regional plasma volume was numerically similar to <span class="elsevierStyleSup">99m</span>Tc-DTPA clearance as a fraction of regional extracellular fluid (ECF) volume.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">9</span></a> For this reason, we have focused on count rate of unit volume of ECV by using dilution principle.</p><p id="par0040" class="elsevierStylePara elsevierViewall">After the subtraction of the post-injection count rate (cpm) of the infusion set from the pre-injection count rate (cpm), the result was divided by five to estimate the injected radiopharmaceutical in a minute for each patient. The formula used to calculate the ECF (L) for each patient was the square root of the weight (kg) multiplied by the height (m).<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">10</span></a> The count rate of the unit volume of the ECF for each patient was calculated according to the dilution principle [Injected count rate (cpm)<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>20 (ml)<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>ECV (ml)<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>serum count rate (cpm)]. The region of interest (ROI) was drawn around the each kidney to calculate the count rate for the each kidney (Total kidney counts/5<span class="elsevierStyleHsp" style=""></span>min) and then the result was divided by the count rate of unit serum volume in order to calculate scintigraphic glomerular filtration rate (ScinGFR).</p><p id="par0045" class="elsevierStylePara elsevierViewall">The formula is:<elsevierMultimedia ident="eq0005"></elsevierMultimedia>where <span class="elsevierStyleItalic">A</span><span class="elsevierStyleInf"><span class="elsevierStyleItalic">r</span></span> is the renal count rate (cpm); <span class="elsevierStyleItalic">A</span><span class="elsevierStyleInf"><span class="elsevierStyleItalic">i</span></span> is the infused count rate (cpm); ECV is the extracellular fluid volume<elsevierMultimedia ident="eq0010"></elsevierMultimedia>.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">10</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Actually, the unit of count rates was counts/min, the actual unit of ScinGFR was ml/min.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">GFR estimation and measurement</span><p id="par0055" class="elsevierStylePara elsevierViewall">The Modification of Diet in Renal Disease (MDRD) Study equation<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">11</span></a> and Cockcroft–Gault equation<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">12</span></a> which are two commonly used equations which are using serum creatinine in combination with age, sex, weight, or race were used to estimate the GFR of our patients. The Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equations based on serum creatinine and Cystatine-C (CysC) were also used to estimate GFR.<a class="elsevierStyleCrossRefs" href="#bib0220"><span class="elsevierStyleSup">13,14</span></a> Unlike them, the measurement of creatinine clearance was also performed.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">15</span></a> These equations improve upon several of the limitations with the use of serum creatinine alone. The GFR measurements of our patients were also performed automatically by using software based on Gates’ formula in the gamma camera.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">16</span></a></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">Descriptive statistics were expressed as mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD. The difference between the pairwise comparisons was tested for significance by using the Paired samples-<span class="elsevierStyleItalic">t</span> test. The difference between the multiple comparisons was tested for significance by using the One-Way ANOVA test. The relationship between the quantitative continuous variables to measure of the strength of the association between the two variables was investigated by using Pearson's correlation coefficient. The Bland–Altman analysis was used to assess the degree of agreement between quantitative measurements. One-sample <span class="elsevierStyleItalic">t</span> test was used to evaluate the average of differences between the paired data whether is different from zero or not. Statistical analysis was performed by using the SPSS 22 Statistical Package Program for Windows (SPSS Inc., Chicago, IL, USA). Values of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 were considered statistically significant.</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Results</span><p id="par0065" class="elsevierStylePara elsevierViewall">Split renal GFR measurement which is the advantageous aspect of the scintigraphic formula was also calculated by our method (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). Creatinine (mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD: 1.53<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.16) levels of the patients were higher than the Cystatin-C (mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD: 1.22<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.73) levels of the patients and this difference was statistically significant (<span class="elsevierStyleItalic">p</span>: 0.008). This comparison was performed due to use of each parameter for estimating GFR separately or together. GFR results derived from different formulas and results of the statistical analysis of pairwise comparisons were presented in <a class="elsevierStyleCrossRefs" href="#tbl0010">Tables 2 and 3</a>, respectively.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Although, the eGFRs derived from MDRD formula were prominently lower and the eGFRs derived form Cockroft-Gault formula were prominently higher than those derived from CKD-EPI formulas, statistically significant difference was not detected among them (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.113). Similarly, there was not any statistically significant difference between the mGFRs of creatinine clearance and the eGFRs derived from other formulas (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.129). On the other hand, when mGFRs derived from scintigraphic method were compared to others, there was a statistically significant difference among them (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.031) due to the difference between the mGFRs derived from scintigraphic method and eGFRs derived form Cockroft–Gault formula. When the results of the ScinGFR were compared to others without the eGFRs derived form Cockroft-Gault formula, the difference among them was statistically insignificant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.164) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">The lowest mGFR values were calculated by our method and in pairwise comparisons, the mGFRs derived from our method were lower than the others and the differences between them were statistically significant except the mGFRs derived from our method and the eGFRs derived from the MDRD formula. Similarly, the mGFRs derived from Gates’ formula were also significantly higher than those derived from our formula. Correlation analysis was performed to evaluate the strength of linear relationship between the variables. Interestingly, there was a positive strong correlation between the mGFRs derived from our method and the others except those derived from CKD-EPI Cystatine-C formula, but the correlation between the mGFRs derived from our method and Gates’ formula was very weak. Since, correlation studies demonstrate the relationship between one variable and another, they are not recommended as a method for evaluating the comparability between methods. Therefore, Bland Altman analysis was performed to assess the degree of agreement between the methods.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The degree of agreement between the results of creatinine clearance and our method was evaluated by Bland–Altman plot (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>). The mean and the standard deviation of the differences between two measurements were 8.59<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1905. The line of equality was not in the 95% confidence interval. The average of the differences between the paired data was higher than the zero and this difference (+Bias) was statistically significant (<span class="elsevierStyleItalic">p</span>: 0.013). It indicates that the scintigraphic method constantly underestimates the GFR results, when it is compared to creatinine clearance. The plot demonstrates the negative trend of differences at low levels of GFR. On the other hand, >95% of the differences between the measurements were included in the range of agreement.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The agreement between the measurements of MDRD and scintigraphic method was evaluated by Bland–Altman plot (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>). The mean and the standard deviation of the differences between two measurements were 2.52<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1816. The line of equality was in the 95% confidence interval. The average of the differences between the paired data was higher than the zero but this difference (+Bias) was not statistically significant (<span class="elsevierStyleItalic">p</span>: 0.424). It indicates that scintigraphic method constantly underestimates the GFR results, when it is compared to MDRD. The plot demonstrates the negative trend of differences at low levels of GFR. On the other hand, >95% of the differences between the measurements were included in the range of agreement.</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">The agreement between the measurements of CKD-EPIcrea-CysC and scintigraphic method was evaluated by Bland–Altman plot (<a class="elsevierStyleCrossRef" href="#fig0025">Fig. 5</a>). The mean and the standard deviation of the differences between two measurements were 11.37<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>19.92. The line of equality was not in the 95% confidence interval. The average of the differences between the paired data was higher than the zero and this difference (+Bias) was statistically significant (<span class="elsevierStyleItalic">p</span>: 0.002). It means that scintigraphic method constantly underestimates the GFR results, when it is compared to CKD-EPIcrea-CysC. The plot demonstrates the negative trend of differences at low levels of GFR. On the other hand, >95% of the differences between the measurements were included in the range of agreement.</p><elsevierMultimedia ident="fig0025"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">The agreement between the measurements of Gates’ method and our method was evaluated by Bland–Altman plot (<a class="elsevierStyleCrossRef" href="#fig0030">Fig. 6</a>). The mean and the standard deviation of the differences between two measurements were −12.99<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>27.22. The line of equality was in the 95% confidence interval. The average of the differences between the paired data was lesser than the zero and this difference (+Bias) was statistically significant (<span class="elsevierStyleItalic">p</span>: 0.009). It means that our method constantly underestimates the GFR results, when it is compared to Gates’ method. On the other hand, >95% of the differences between the measurements were included in the range of agreement.</p><elsevierMultimedia ident="fig0030"></elsevierMultimedia></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">In our study, we have developed an alternative quantitative approach to measure the GFR by infusing <span class="elsevierStyleSup">99m</span>Tc-DTPA to simulate creatinine secretion, which has a stable plasma concentration due to release into the circulation at a relatively constant rate from the metabolism of creatine in skeletal muscle and from dietary meat. Our method provided similar mGFR results in narrower range as creatinine clearance did and our results positively well correlated with results derived from other equations except Gates’ method.</p><p id="par0105" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">99m</span>Tc-DTPA is a small molecule with a molecular mass of 492 Dalton.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">8</span></a> If <span class="elsevierStyleSup">99m</span>Tc-DTPA is injected intravenously as a bolus, the rising and decreasing parts of the renogram curve will be exponential due to dilution of the radiopharmaceutical in the ECF and calculating the renal functions requires complex mathematical procedures. However, if the <span class="elsevierStyleSup">99m</span>Tc-DTPA is infused in a constant manner, the rising part of the time activity curve will be linear due to partial linear uptake of the extracted and diluted part of the radiopharmaceutical. The extraction fraction of a filtration marker is a quotient GFR/renal plasma flow, and is normally 0.2 and after i.v. injection, an indicator mixes throughout its distribution volume.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a> Therefore, indicators suitable for the measurement of GFR by plasma clearance have a physiological distribution volume close to ECF<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a> or the distribution volume of small hydrophilic solutes such as <span class="elsevierStyleSup">99m</span>Tc-DTPA for GFR is the ECF volume.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">9,18</span></a> In resting skeletal muscle, the continuous capillary endothelium, which is highly permeable, allows about 50% of <span class="elsevierStyleSup">99m</span>Tc-DTPA to diffuse across in the direction plasma to interstitial fluid during a single capillary pass and thereafter <span class="elsevierStyleSup">99m</span>Tc-DTPA accumulating in the interstitial fluid begins to return to plasma within 30<span class="elsevierStyleHsp" style=""></span>s of its arrival (back-diffusion).<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">17,19</span></a> Although the plasma clearance curve of a filtration marker has been conventionally described as bi-exponential up to 4<span class="elsevierStyleHsp" style=""></span>h, actually, it consists largely of three exponentials and the first exponential has been reported to represent tracer exchange across capillary endothelium into the interstitial fluid throughout the body.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a> Besides, it has been reported that the first exponential has the largest zero-time intercept of the others and when the indicator is <span class="elsevierStyleSup">99m</span>Tc-DTPA, this generates a volume of about 10<span class="elsevierStyleHsp" style=""></span>L instead of plasma volume.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a> Therefore, if the known count rate of a unit volume is infused constantly, the count rate of the unit volume of the ECV can be calculated by considering the time. After then, the radiopharmaceutical collected in the kidneys indicates the unit volume of the ECV carrying counts to them which is an indirect measure of the GFR. This is the main idea which our formula has been based on.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Our formula is different from the original Gates’ formula in several ways. In our method, for ease of calculation, the background subtraction was not performed to neutralize the effect of attenuation which is calculated from the renal depth and linear attenuation coefficient of <span class="elsevierStyleSup">99m</span>Tc in soft tissues in original Gates’ method.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">20</span></a> While the weight and height of the patient were used to calculate the renal depth in original Gates’ formula, we have used these parameters to calculate ECV of the patients. Our calculations were simply based on dilution principle. On the contrary of the Gates’ method, our formula was not normalized to body surface area (BSA) of 1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span> according to DuBois formula. Indexing GFR to body surface area reduces variation among healthy individuals and allows comparisons to normative values, but has no physiological explanation and may underestimate renal function in very obese individuals.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">21</span></a> As an alternative, it has been suggested to scale GFR to extracellular fluid volume (ECV) as in our calculation method.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">22</span></a> Since GFR may change as a direct result of a change in ECF volume, it has been reported to be physiologically more relevant and technically very much easier, to express GFR in terms of ECF volume rather than body surface area.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">22</span></a> Besides, indexation to ECF abolishes differences that arise between children and adults when GFR is indexed to BSA.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a> In addition the difference in renal function between men and women has also been reported to be annihilated when GFR was indexed to ECF.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">18</span></a> Actually, same parameters such as the weight and the height of the patient were used for different purposes such as BSA and ECV, and therefore, our formula was inherently normalized against the biological variations among individuals. These are probable underlying reasons of statistically significant difference between the results of Gates’ method and our method.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The most accurate method for evaluating GFR is based on the plasma disappearance curve after a single bolus injection of a radioactive tracer exclusively eliminated by the glomerulus such as <span class="elsevierStyleSup">51</span>Cr-EDTA or <span class="elsevierStyleSup">99m</span>Tc-DTPA and followed by the taking of two or more blood samples 3–4<span class="elsevierStyleHsp" style=""></span>h after the injection.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">23</span></a> The plasma sample-based methods are more accurate and closely estimate the gold standard of inulin clearance but, the radionuclide clearance methods using blood samples taken at different time points post-injection are inconvenient due to time consuming and difficulties related with multiple blood sampling, compared to our method. Besides, according to the systematic review that the evidence regarding the accuracy of renal and plasma clearance of DTPA is limited or insufficient and limited evidence suggests that plasma clearance of DTPA is an inaccurate method.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">15</span></a> Therefore, we have preferred to infuse DTPA instead of performing plasma DTPA clearance method.</p><p id="par0120" class="elsevierStylePara elsevierViewall">A simple chemical parameter for a first-step gross evaluation of renal function is serum creatinine, but it is not an ideal guide to GFR because it is highly dependent on muscle mass and it is insensitive to changes in renal function until glomerular filtration is reduced substantially.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">24</span></a> Creatinine clearance is a routinely used GFR-estimating method, but it requires the inconvenience of urine collections and remains inaccurate because of many confounding factors,<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">25</span></a> such as age, sex, race and sample collection errors. Although, our results showed similar distribution pattern as creatinine clearance did, they were significantly lower than those of it and other GFR estimating methods in our study. Estimating equations for GFR are regression equations without requiring calculation of clearance and based on plasma levels of endogenous filtration markers, demographic and clinical variables.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">26</span></a> Hence, an estimating equation provides a more accurate estimate of mGFR than the plasma concentration of endogenous filtration marker alone. On the other hand, the plasma concentrations of endogenous filtration markers such as creatinine, and cystatin C are affected by other factors including generation, tubular reabsorption or secretion and extra-renal elimination in addition to GFR, and therefore, estimating equations tend to give incorrect results. Therefore, eGFR measured at the patient's bedside using methods like Cockcroft–Gault and MDRD formulae are a practical and clinically useful alternative to formal GFR measurement but a crude way of assessing renal function.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">27</span></a> The above mentioned confounding factors are probable underlying reasons for statistically significant difference between the results of creatinine based GFR estimating methods and our method except MDRD.</p><p id="par0125" class="elsevierStylePara elsevierViewall">Exogenous filtration markers are administered into the body for the purpose of performing clearance measurements, but endogenous filtration markers are generated in the body at a relatively constant rate, and the plasma levels can be used to estimate GFR. In our formula, <span class="elsevierStyleSup">99m</span>Tc-DTPA was infused as an exogenous filtration marker and its plasma levels as an endogenous filtration marker was used to measure GFR.</p><p id="par0130" class="elsevierStylePara elsevierViewall">The results of the GFR estimates calculated by CKD-EPI equations were higher than those calculated by creatinine clearance but, there was not a statistically significant difference among them in our study. The creatinine clearance is a widely used laboratory test to estimate the GFR. It has been reported that endogenous creatinine clearance consistently overestimated GFR and demonstrated relatively higher overestimation at low GFRs.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">15</span></a> Although, creatinine is freely filtered across the glomerulus there is tubular secretion by the organic cation secretory pathways in the proximal tubule that it causes progressive overestimation of the GFR with more severe disease. Hence the creatinine clearance tends to exceed the true GFR by 10–20% or more depending upon the proportion of urinary creatinine that is derived from tubular secretion.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">28</span></a> In our study, GFR results estimated from creatinine clearance were significantly higher than our results and the GFR overestimation of creatinine clearance may be a potential underlying mechanism. It has already been emphasized that when a measured GFR is required, several alternatives to renal inulin clearance exist but according to the systematic review, strong scientific evidence suggests that endogenous creatinine clearance is an inaccurate method.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">15</span></a> Even so, the creatinine clearance was thought to be a good standard to compare our results.</p><p id="par0135" class="elsevierStylePara elsevierViewall">The Cockcroft–Gault formula was the first GFR estimating equation to predict creatinine clearance and the second one was the MDRD formula. The MDRD formula was different from Cockcroft–Gault formula because it was adjusted for a body surface area of 1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span>. These two equations are limited by lack of validation in the full range of GFR and unsuitable to estimate GFR in healthy individuals. Although, they have been shown to markedly underestimate GFR above approximately 60<span class="elsevierStyleHsp" style=""></span>ml/min 1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span> and to markedly overestimate GFR below approximately 20<span class="elsevierStyleHsp" style=""></span>ml/min 1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span>,<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">2,26</span></a> the range of results derived from both formulas were wider than the others, in our study.</p><p id="par0140" class="elsevierStylePara elsevierViewall">According to our results, there was not any statistically significant difference among the estimates derived from CKD-EPI equations and creatinine clearance but the estimates derived from Cockroft–Gault equation were significantly higher and estimates derived from MDRD equation were significantly lower than the others. While the MDRD Study equation was found to be more accurate compared with the CG equation,<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">29</span></a> the CKD-EPI creatinine equation has been reported to be more accurate than the MDRD Study equation for GFR estimation.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">25</span></a> Actually, the CKD-EPI equations are the most accurate GFR estimating equations that have been evaluated in large, diverse populations at this time.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">25</span></a> Equations that use both creatinine and cystatin C are more precise than equations that use either marker alone, and it is likely that equations using multiple endogenous filtration markers can be even more precise. In our study, the results of our method were significantly lower than the others except MDRD and under the light of these data, inulin clearance as a reference method seems to reveal the potential role of the scinGFR compared to others.</p><p id="par0145" class="elsevierStylePara elsevierViewall">GFR is affected by numerous physiological and pathological conditions and varies with the time of day, dietary protein intake, exercise, age, pregnancy, obesity, hyperglycemia, use of antihypertensive drugs, surfeit or deficit of extracellular fluid, and acute and chronic kidney disease.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">25</span></a> Since, patients in our study group had creatinine values and creatinine clearance test earlier than the test day, the numerous physiological and pathological conditions mentioned above may be potential underlying reasons for the statistical significant difference between the results of creatinine based GFR calculations and our method. Besides, as with any physiological measurement, GFR has an intrinsic biological variability, and reported within-person biological coefficient of variation values for GFR is between 5 and 12%.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a> An intrinsic biological variability in GFR determination also can be an underlying mechanism for the statistically significant difference between the results of cystatin based GFR calculations and our method.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Protein binding of <span class="elsevierStyleSup">99m</span>Tc can lead to GFR underestimation and variable dissociation of <span class="elsevierStyleSup">99m</span>Tc from DTPA can cause imprecision and bias.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a> Protein binding of <span class="elsevierStyleSup">99m</span>Tc-DTPA has been reported to increase with time and to cause error in GFR calculations.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">30</span></a> Since our acquisition period for the study is short enough, protein binding of <span class="elsevierStyleSup">99m</span>Tc-DTPA was not considered a potential underlying mechanism why our results were significantly lower than the others.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Determining GFR by means of radioactive tracers can be useful in every time renal impairment is suspected, even when plasma creatinine is in the normal range. On the other hand, determination of absolute function of each kidney separately is important in conditions such as unilateral or bilateral hydronephrosis, urinary tract infections with or without associated vesico-ureteral reflux, small kidney, single kidney, duplex kidney, urethral valves, pre- and postoperative follow-up. The quantification of the split renal function by <span class="elsevierStyleSup">99m</span> Tc-DMSA can be performed by different mathematical analysis, most often from planar images with compensation for kidney depth based on the geometric mean, which can be determined using anterior and posterior views<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">30</span></a> but the results were shown to be only fairly correlated with reference techniques, even when using SPECT.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">31</span></a> Factors that may influence both the accuracy and the day-to-day reproducibility are the exact estimation of kidney depth, the timing of measurement, and the quality of the preparation injected.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">24</span></a> Therefore, estimation of individual kidney function measured by our formula seems to be helpful in the follow-up of patients with above mentioned clinical conditions.</p><p id="par0160" class="elsevierStylePara elsevierViewall">There were some limitations of the study. Although, the blood for measurement of cystatine-C was collected at the test day, serum creatinine levels were measured in previous days before the test and this time difference may have caused some differences in our results. There is a usual trade-off between clinical simplicity and accuracy in wide range of approaches to the determination of renal function in clinical practice, especially GFR.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">17</span></a> Our formula is so simple for ease of calculation and it is an empirical approach to assess the renal function as our formula based on some assumptions and not exact calculations. If the background subtraction and attenuation correction are added to our formula like Gates’ formula, we may have more correct results. Another issue about limitation of the study is that if ECV is measured correctly instead of calculating available formula, our results would be more accurate. Plasma concentration of Cystatin-C is influenced by thyroid function and there are genetic influences on plasma concentration.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">1</span></a> Actually we have not checked the functional status of the thyroid function of our study population, and therefore, this issue was another limitation of the study.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusion</span><p id="par0165" class="elsevierStylePara elsevierViewall">We have developed a simple formula to predict the split and global kidney functions. When we compared our results with the available formulas to predict the renal functions, despite some discrepancies, we have found good correlation among them. Hence the aim of the study was to design a new camera-based approach to measure the GFR and to compare our results with other measured GFR (mGFR) and estimated GFRs (eGFRs) derived from available measurements and equations used in daily clinical practice.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">34 patients were included in the study. ∼74<span class="elsevierStyleHsp" style=""></span>MBq (2<span class="elsevierStyleHsp" style=""></span>mCi) Technetium 99m diethylene-triamine-pentaacetic acid (<span class="elsevierStyleSup">99m</span>Tc-DTPA) was administered to the patients during 5<span class="elsevierStyleHsp" style=""></span>min. A simple formula based on a dilution principle was used to measure GFR (ScinGFR).</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Our formula provided similar mGFR results in narrower range as creatinine clearance did and our results correlated well with results derived from other equations. When ScinGFR values were compared to others, there was a significant difference among them (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.031) due to difference between the ScinGFR and Cockroft–Gault.
