Dislocations of iridociliary cysts into the vitreous cavity have been described,<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">3</span></a> which hampers differential diagnosis with UBM, with other imaging techniques of standard use in ophthalmological services being more useful, such as optical coherence tomography (OCT).</p><p id="par0015" class="elsevierStylePara elsevierViewall">We present the case of a 12-year-old female patient, without a relevant general or ophthalmological history, who underwent a routine follow-up ophthalmological evaluation during which a cystic structure was detected in her left eye as an incidental asymptomatic finding: lobulated, with a pigmented surface and transparent content in the anterior vitreous cavity, only a few millimetres away from the posterior side of the crystalline lens, with certain degree of mobility on movement of the eyeball towards different positions (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), thus being suggestive of a benign finding.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">A UBM study was carried out revealing the presence of normal iridociliary structures in both eyes, without being able to examine the cyst due to its location. Given its small size, the lesion could not be examined with a conventional ocular ultrasound. In order to thoroughly study the cyst's internal structure and characteristics, we decided to carry out an OCT by means of the Cirrus<span class="elsevierStyleSup">®</span> HD-OCT system (Carl Zeiss Meditec, Dublin, CA, U.S.), following the Anterior Segment 5-Line Raster acquisition protocol. The scan was passed over the vitreous cavity in order to achieve a correct focus and acquire tomographic images of the cyst (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). The images revealed a round lesion located in the posterior side of the crystalline lens, with 2 hemilobes (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>: tomographic slice 3), one of them being multilobal and the other one, of greater size, consisting of a single lobe with an internal hyporeflective signal (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>: tomographic slice 4). The internal and external walls of the cyst had a hyperreflective signal due to the presence of pigment.
