Robotic cholecystectomy in situs inverus totalis

The Situs inversus totalis supposes the complete transposition of the organs, with an incidence of 1:5.000–1:20.000.¹ Associated with bronchiectasias and sinusitis corresponds to Kartagener’s syndrome. Laparoscopic cholecystectomy is more difficult² given its opposite location in the left hypochondrium (Fig. 1) forcing a “mirror” dissection compared to the standard.

We present an 88-year-old patient, hypothyroid with repetitive pneumonia, studied by CT that confirms Kartagener’s syndrome. The patient reports recurrent biliary colic. The ultrasound confirms cholelithiasis without dilation of the biliary tract. Robotic cholecystectomy is indicated (Fig. 2) for the technical advantages in this case.
