A 77-year-old male reported abdominal pain and fecaloid vomiting, with no fever or other symptoms. Blood work was normal, but chest X-ray showed findings suggestive of herniation of intestinal loops into the thoracic cavity. CT scan confirmed the presence of a voluminous right diaphragmatic hernia through the Morgagni foramen that contained most of the digestive tract; only the left stomach and colon remained in the abdominal cavity (Fig. 1). A nasogastric tube was placed, followed by laparoscopic repair with closure of the hernia orifice that had a diameter of 10 cm at its largest point with mesh placement.
Diagnosis: Morgagni hernia as a cause of intestinal obstruction.
