The green line represents the absolute numbers of OASIS for each year.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Pelvic floor dysfunction is a major health issue that affects quality of life of many women. It comprises a wide range of clinical scenarios such as lower urinary tract excretory and defecation disorders, overactive bladder, pelvic organ prolapses and sexual disorders.<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">1,2</span></a> Its prevalence increase with parity and age.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">3</span></a> Pregnant women experiment changes in pelvic floor structures due to mechanical effects, hormonal alterations that increase laxity of connective tissue, and possible damage to the anal sphincter complex and levator ani muscles during delivery, which play a crucial role in the development of prolapse and stress urinary incontinence.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">1</span></a> In fact, up to 23.7% of women with vaginal parity will suffer from one of these pathologies.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">According to RCOG Guidelines based on Sultan's description, perineal tears are classified in four degrees; the anal sphincter injury is involved in third and fourth degree.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The incidence of obstetric anal sphincter injuries (OASIS) presents a significant variability in the literature (0.6–10.2%).<a class="elsevierStyleCrossRefs" href="#bib0120"><span class="elsevierStyleSup">1,3</span></a> OASIS may cause several functional consequences, such as perineal pain, acute retention of urine, dyschesia and abscess in the short term, and fistulas, dyspareunia, neuropathy and anal incontinence in the long term, with a range of 15–61% that increases over the years and with perineal tear grade.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">5,6</span></a> It also causes important physical and psychological morbidity that affects quality of life.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">7</span></a> For this reason, accurately diagnosis and classification of OASIS is crucial in order to perform the appropriate repair.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Many risk factors of OASIS have been studied and some of them have demonstrated a strong association, such as nulliparity, operative vaginal delivery and birth weight higher than 4000<span class="elsevierStyleHsp" style=""></span>g.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">8</span></a> The relationship between OASIS and episiotomy, length of pregnancy, epidural analgesia or oxytocin induction is conflicting.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">5,9</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The means of prevention are mainly represented by the control of the descent of the fetal head by two possible techniques that have been demonstrated as preventive for OASIS: Finnish and Viennese manoeuvres. In both techniques, the non-dominant hand controls the speed of fetal head expulsion and facilitates fetal head extension. They differ regarding the part of the dominant hand used to apply pressure against the perineal body.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">5</span></a> Other demonstrated preventive factors are perineal support and massage, limitation of the expulsion efforts during the expulsion phase, and mediolateral or lateral episiotomy in primiparous women and instrumental delivery compared to a midline episiotomy or no episiotomy.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">6</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">The knowledge of these risk and prevention factors is important to prevent OASIS. From this point of view, some studies were done in order to evaluate if an interventional programme could modify the OASIS overall rate.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">10,11</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Hospital Parc Taulí is an accredited training centre for specialist physicians and midwives. Spontaneous deliveries can be attended either by obstetricians, residents or midwives following a common protocol which includes hands-on perineal protection and a restrictive use of mediolateral episiotomy. Instrumental vaginal deliveries are only performed by physicians or supervised residents and the indication of each type of instrument is done by the attending physician depending on the obstetric situation. Urinary bladder is emptied by catheterisation and adequate pain relief is recommended before the use of the instrument. Mediolateral episiotomy is performed using Mayo scissors when the head is crowning if the physician considers it necessary. Local anaesthesia in the episiotomy side is not routinely used.</p><p id="par0040" class="elsevierStylePara elsevierViewall">A bidigital rectal/vaginal examination is performed in order to rule out the presence of OASIS. If it is diagnosed, primary repair is carried out by specialist obstetrician according to local protocol, based on the established clinical guidelines.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">12</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Perineal trauma detection and repair workshops were implemented in our hospital in 2006, being done every two years since then. The program consists of a 2-day workshop: the first day is a theoretical session about anatomy, classification, repair method and prevention techniques; the second day consists in practicing repair on pig animal models.</p><p id="par0050" class="elsevierStylePara elsevierViewall">The aim of this study was to describe obstetric anal sphincter injuries (OASIS) in our population and its evolution in a 25-year period. Our secondary objective was to identify risk factors related to obstetric trauma that could have had an effect on its evolution.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Study design, procedure and clinical data</span><p id="par0055" class="elsevierStylePara elsevierViewall">A retrospective observational study was carried out to describe the incidence of obstetric trauma in our population. Data were obtained from official computerized obstetric data record.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Women who delivered in Hospital Parc Taulí from January 1998 until December 2021 were included. The exclusion criteria were caesarean section or delivery before 22 weeks of gestation.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The primary endpoint of the study was to determine the presence of OASIS according to Sultan's description:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0070" class="elsevierStylePara elsevierViewall">Third-degree tear: injury involving the anal sphincter complex (EAS).<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">∘</span><p id="par0075" class="elsevierStylePara elsevierViewall">Third-degree type A: involving less than 50% of EAS thickness torn.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">∘</span><p id="par0080" class="elsevierStylePara elsevierViewall">Third-degree type B: involving more than 50% of EAS.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">∘</span><p id="par0085" class="elsevierStylePara elsevierViewall">Third-degree type C: involving external and internal sphincters.</p></li></ul></p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">-</span><p id="par0090" class="elsevierStylePara elsevierViewall">Fourth-degree tear: injury involving the anorectal mucosa.</p></li></ul></p><p id="par0095" class="elsevierStylePara elsevierViewall">Some exposure variables proved to be associated with the primary endpoint were collected. All of them were also obtained from the official obstetric data or hospitalization record. Definitions of the exposure variables were as follows:<ul class="elsevierStyleList" id="lis0015"><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">-</span><p id="par0100" class="elsevierStylePara elsevierViewall">Type of delivery (eutocic, forceps, spatula, vacuum, caesarean section).</p></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">-</span><p id="par0105" class="elsevierStylePara elsevierViewall">Use of loco regional anaesthesia (yes/no).</p></li><li class="elsevierStyleListItem" id="lsti0040"><span class="elsevierStyleLabel">-</span><p id="par0110" class="elsevierStylePara elsevierViewall">Episiotomy (yes/no): surgical mediolateral incision of the perineum and the posterior vaginal wall.</p></li><li class="elsevierStyleListItem" id="lsti0045"><span class="elsevierStyleLabel">-</span><p id="par0115" class="elsevierStylePara elsevierViewall">Perineal 2nd degree trauma (yes/no): perineum injury involving perineal muscles but not the anal sphincter.</p></li></ul></p><p id="par0120" class="elsevierStylePara elsevierViewall">The performance of other protective methods, such as manual perineal protection or perineal massage, was not collected in the obstetric record.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical analysis</span><p id="par0125" class="elsevierStylePara elsevierViewall">Data processing and analysis were performed using Excel (Microsoft Office 16.0).</p><p id="par0130" class="elsevierStylePara elsevierViewall">The descriptive results by years were expressed as percentage of frequency (%) for categorical variables and mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>SD for continuous variables.</p><p id="par0135" class="elsevierStylePara elsevierViewall">The statistical analysis for categorical variables was done with Mantel–Haenszel Chi-Square using SPSS Statistics 17.0 (SPSS Inc., Chicago, IL). The statistical significance was stablished at <span class="elsevierStyleItalic">p</span> value <0.05.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Ethical consideration</span><p id="par0140" class="elsevierStylePara elsevierViewall">Ethical committee of our hospital approved the study protocol. Personal identification details were omitted to ensure anonymity and data was collected in a dissociated anonymous way. Informed consent was not required.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0145" class="elsevierStylePara elsevierViewall">The descriptive study included 60,145 women who delivered between January 1998 and December 2021 in our centre. From these, 50,022 (83.17%) had a vaginal delivery. The birth type was distributed in 68.3% (41,080) eutocic, which corresponds to 82.12% of total vaginal deliveries, and 14.3% (8612) operative vaginal deliveries, which corresponds to 17.22% of total vaginal deliveries. 16.83% (10,123) of the missing deliveries were caesarean section and the rest correspond to a minority of breech and tween deliveries.</p><p id="par0150" class="elsevierStylePara elsevierViewall">From all vaginal births, loco regional anaesthesia was performed in 81.3% (48,922) of cases and the episiotomy rate was 59.7% (29,882). The evolution of this data through that time period is shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> and <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0155" class="elsevierStylePara elsevierViewall">The evolution is displayed in a five-year interval and total data of the 25-year period at the end. The results are expressed in absolute numbers followed by the corresponding frequency in percentages (%) over the total number of deliveries in eutocic, operative vaginal delivery and caesarean section variables, and over the total number of vaginal deliveries for locoregional anaesthesia, episiotomy, 2nd degree perineal trauma and OASIS.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Due to the introduction of vacuum in our hospital in 2018, a detailed description of different types of deliveries was also carried out from 2018 to 2021, in order to see the evolution in the use of vacuum with respect to the rest of instrumental deliveries. This information is shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> and <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0165" class="elsevierStylePara elsevierViewall">Regarding the different types of perineal tears, second degree perineal trauma was detected in 15.2% (7605) of patients who had a vaginal delivery. OASIS were reported in 369 women, constituting 0.74% of all vaginal deliveries. The major part of OASIS was detected either in Kjelland forceps deliveries (46.8%, <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>168) and eutocic deliveries (41.5%, <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>149). The rest of them were distributed as follows: 6.1% spatula deliveries, 5.4% Naegele forceps deliveries and 0.2% vacuum-assisted deliveries. The results are shown in <a class="elsevierStyleCrossRefs" href="#fig0015">Figs. 3 and 4</a>.</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="fig0020"></elsevierMultimedia><p id="par0170" class="elsevierStylePara elsevierViewall">Due to differences seen in the tendency of OASIS rate since 2018 with the introduction of vacuum in our hospital, we decided to analyse data from 2018 to 2021 in more detail.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Mantel–Haenszel Chi-Square was the test used to determine if there existed statistically significant differences between the rate of OASIS in total vaginal deliveries per year compared to 2018 (results are shown in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). We also analysed whether there were significant differences between the rate of OASIS in operative vaginal deliveries vs. eutocic deliveries and between Kjelland forceps vs. vacuum deliveries in each of these years (results are shown in <a class="elsevierStyleCrossRefs" href="#tbl0020">Tables 4 and 5</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0180" class="elsevierStylePara elsevierViewall">Although it was shown an important reduction in the number of total OASIS from 2018 to 2021, the difference is only statistically significant once is compared to 2019 and 2020, but not 2021. Regarding the differences between operative and eutocic deliveries, the frequency of OASIS is statistical significantly higher than in operative deliveries (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05), supporting the previous evidence of operative delivery being a risk factor for OASIS. Operative vaginal deliveries appeared to have between 7 and 9.4 times more risk to present a 3rd–4th degree perineal tears than eutocic deliveries. However, it was not demonstrated statistical difference in OASIS rate between Kjelland forceps and vacuum (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.05), probably due to the low number of OASIS in each group. This is the reason why it was not possible to compare the rates in 2018 and 2019, as there was not documented any OASIS in vacuum deliveries. Nevertheless, we should take into account that although it was not statistically significant, vacuum appeared to be a protective factor for OASIS compared to Kjelland forceps (OR<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>1).</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0185" class="elsevierStylePara elsevierViewall">The incidence of OASIS in the present study is according to the incidence described in the literature (0.4–10.2%),<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">4,7–9,11,13–16</span></a> remaining in the lower part of the interval although it presents some variations over the years studied.</p><p id="par0190" class="elsevierStylePara elsevierViewall">In the analysis of OASIS evolution during 25 years, it was possible to identify some risk factors that could have contributed to changes in its prevalence at specific points. These factors, that are discussed below, are as follows: implantation of training programs, indication of episiotomy, epidural anaesthesia, type of delivery and parity.</p><p id="par0195" class="elsevierStylePara elsevierViewall">The initial incidence of OASIS was low, but there was an increment in 2006. From that point, it continued increasing, reaching the maximum peak in 2010, representing 1.5% of total vaginal deliveries of that year. After then, the incidence decreased to figure similar numbers to those observed at the beginning of the study. The reduction was especially noticeable from 2018, when the rate of OASIS descended by half over 2021, from 1.1% in 2018 to 0.6% of the total vaginal deliveries in 2021.</p><p id="par0200" class="elsevierStylePara elsevierViewall">The increment of OASIS rate in 2006 coincides with the start of perineal trauma detection and repair programs in our hospital, what could have had an impact in the rate of OASIS detected by the attendants. In fact, it has been shown in several studies that formal training in the recognition of OASIS improves its detection.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">8,16</span></a> In 2015, a review by Harvey et al., mentioned an increase in the detection of OASIS from 11% to 24.5% when the assessment was repeated by a trained fellow.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">16</span></a></p><p id="par0205" class="elsevierStylePara elsevierViewall">With the opposite effect, another topic studied in recent years is the magnitude that a training program in perineal protection can have in reducing the incidence of OASIS. Laine et al. performed a cohort study in Norway in 2012 comparing the incidence of OASIS before and after the implementation of a training program. The results were encouraging, as OASIS incidence was reduced by 50% (from 4% to 1.9%) and it was consistent across subgroups of women.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">11</span></a> Gurol-Urganci et al. performed a multicentre study in the UK in which it was also demonstrated a significant decrease in OASIS rate after the implementation of a care bundle.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">10</span></a></p><p id="par0210" class="elsevierStylePara elsevierViewall">Another fact that could have contributed to the increase in OASIS from 2006 was the decline in episiotomies. It is noticeable that while the practice of episiotomies decreased throughout the years (from 96.2% of vaginal deliveries in 1998 to 20.93% in 2021), the number of second-degree tears simultaneously increased (from 9.9% to 27.53%). The relationship of episiotomy and OASIS has been studied in depth over the years. The use of episiotomy in Western Europe and Nordic countries is generally restricted since liberal was reported to cause more perineal trauma, suturing, and healing complications.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">9</span></a> However, it has been demonstrated several times as one of the most important modifiable factors for OASIS. For instance, Bodner-Adler et al. found that frequency and severity of perineal tears were significantly lower in forceps deliveries when using episiotomy.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">17</span></a> In the same way, several retrospective studies shew that nulliparous women who received episiotomy at vacuum-assisted deliveries had a reduced prevalence of OASIS compared with women without episiotomy. Lund et al. compiled in a meta-analysis published in 2016 that mediolateral or lateral episiotomy significantly reduced the risk of OASIS at vacuum deliveries in nulliparous women [OR 0.53 (95% CI 0.37–0.77)].<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">18</span></a> In 2014, Jangö et al. had already found an interaction between vacuum extraction and mediolateral episiotomy, having a protective function against OASIS but no significant effect in spontaneous deliveries.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">9</span></a></p><p id="par0215" class="elsevierStylePara elsevierViewall">The meta-analysis performed by Okeahialam et al. in 2022, showed a significant reduction in OASI rate with a mediolateral or lateral episiotomy compared to deliveries without [OR 0.60 (95% CI 0.45–0.79)]. Similar to the previous literature revised, while nulliparous women had a significantly reduced rate of OASIS when episiotomy was performed during ventouse of forceps, the reduction in multiparous women did not reach statistical significance.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">7</span></a></p><p id="par0220" class="elsevierStylePara elsevierViewall">In the same line of the results of our study, Jangö et al. reported that while the incidence of OASIS increased over time, the episiotomy rate decreased. This increase in OASIS was still seen after adjustment of possible risk factors in the multivariable analysis.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">9</span></a> There is strong evidence against midline episiotomy, showing a clear association with increased risk of OASIS compared to mediolateral.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">8,9,19</span></a></p><p id="par0225" class="elsevierStylePara elsevierViewall">The influence of epidural is contradictory between studies; although in some was demonstrated a significant protective factor for OASIS<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">9</span></a> in others its rate was incremented.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">2,20</span></a> However, in our study the use of locoregional anaesthesia remained stable over the research period. This fact suggests that it probably had no influence on the incidence of OASIS.</p><p id="par0230" class="elsevierStylePara elsevierViewall">There are some other facts that could have contributed to the variable incidence of OASIS over the years. On the one hand, the reduction of operative vaginal deliveries. On the other hand, the substitution of Kjelland forceps to vacuum in a significant number of cases from 2018, which has been demonstrated to be less traumatic for pelvic musculature compared to Kjelland forceps. It should be taken into account that, in our study, the OASIS rates were not differentiated according to parity, while it has already been demonstrated that the probability of OASIS in operative vaginal deliveries is higher in primiparous women compared to multiparous.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">10,16</span></a></p><p id="par0235" class="elsevierStylePara elsevierViewall">When an instrumental delivery is indicated, vacuum extraction carries less risk to the anal sphincter than forceps.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">6,8,16,21</span></a> Leeuw et al. described OASIS in 3% of vacuum extractions compared to 4.7% in forceps.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">22</span></a> Also, early removal of forceps, rotation of the head of the baby to an occiput anterior position and using minimal necessary maternal expulsive efforts at time of expulsion may limit OASIS.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">16</span></a></p><p id="par0240" class="elsevierStylePara elsevierViewall">Simó González et al. carried out an observational retrospective study in Hospital Sant Pau in Barcelona between 2006 and 2009, where the population studied was similar to ours. They showed that spontaneous delivery had the lowest incidence of injury (1.1%), followed by vacuum extraction (2.7%), forceps (4.5%) and Thierry's spatulas (4.9%).<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">13</span></a></p><p id="par0245" class="elsevierStylePara elsevierViewall">Bergendahl et al. show in an observational study of a historical cohort that Swedish OASIS rate was higher in nulliparous women with vacuum extraction (12–15%) than nulliparous women with spontaneous vaginal delivery (5%). The use of forceps in Sweden is very restricted. However, among them, the rate of OASIS was even higher, making operative vaginal delivery the single most important risk factor for OASIS.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">14</span></a></p><p id="par0250" class="elsevierStylePara elsevierViewall">When analyzing OASIS in particular, it is remarkable that the majority of them were detected in forceps and eutocic deliveries, whereas only 0.28% of them were observed in vacuum-assisted deliveries. There are several facts that should be taken into account. First of all, the absolute number of eutocic deliveries was much greater than operative vaginal deliveries. This is why it is logical that a higher percentage of the total OASIS was detected in eutocic births. Otherwise, it is important to consider that the study includes data from 1998, when the use of forceps was predominant, and that the use of vacuum was generalized in detriment of forceps only since 2018, so the absolute number of vacuums persists considerably inferior in the total data.</p><p id="par0255" class="elsevierStylePara elsevierViewall">When analysing the data from 2018, the OASIS rate reduction is remarkable specially the first year after the introduction of vacuum, and it remains low while the use of vacuum continues increasing until it exceeded the use of forceps, becoming the predominant instrumental type of delivery in 2021.</p><p id="par0260" class="elsevierStylePara elsevierViewall">Some of the limitations of our study are the inherent ones to retrospective studies and, also, that the database was not designed specifically for the study (obstetric registry). This last factor might have contributed to the loss of an undetermined number of OASIS due to the lack of coding or incorrect coding of perineal tear. It would be also interesting to describe the incidence of each grade of OASIS in particular, which was not possible due to the missing information of the specific grade of OASIS before 2018.</p><p id="par0265" class="elsevierStylePara elsevierViewall">However, we consider it an important review of the experience in our hospital considering the main strength the large database over 25 years. Although speculations about the factors related to the changes in OASIS incidence over the years could be made from the extracted data, analytic studies should be done in order the confirm its risk and protective factors.</p><p id="par0270" class="elsevierStylePara elsevierViewall">The study could be the starting point for the analysis and confection of predictive models for obstetrical lesions that could change our clinical practice and, therefore, diminish the incidence of OASIS and its long-term consequences, which have an enormous impact in women's quality of life.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Conclusion</span><p id="par0275" class="elsevierStylePara elsevierViewall">The present descriptive study agrees with the literature on the predominance of OASIS in forceps deliveries with respect to other kind of vaginal deliveries. It also shows a decrease of operative vaginal deliveries and episiotomy in recent years, parallel to the decline of OASIS from 2010 and the increment of vacuum-assisted deliveries from 2018. Operative delivery was demonstrated a risk factor for OASIS, although the differences between forceps and vacuum were not significant. Obstetric anal sphincter injuries (OASIS) are related to vaginal parity and can lead to anal incontinence in 60% of women in the long term. Its identification is crucial to perform optimal surgical repair. The aim of the study was to describe obstetric trauma in our population and evolution in a 25-year period.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Observational retrospective study of woman who delivered in Hospital Parc Taulí between 1998 and 2021. Patients without vaginal delivery or delivery before 22nd gestational week were excluded. Data processing and analysis were performed using Excel.2016 and IBM SPSS Statistics 17.0.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Sixty thousand one hundred forty-five women who delivered in our hospital over the study period were included. Fifty thousand twenty-two (83.17%) had a vaginal delivery: 82.12% were eutocic and 17.22% operative assisted. Loco regional anaesthesia was performed in 81% and the overall episiotomy rate was 59.7%. OASIS were reported in 369 women, constituting 0.74% of all vaginal deliveries, the major part detected in Kjelland forceps deliveries (representing 46.8% of OASIS) and eutocic deliveries (41.5%). Analysing the evolution of OASIS, an important reduction from 2018 to 2021 was noticeable (1.1 vs. 0.6%) and also a statistically significant difference in OASIS rate between operative and eutocic deliveries (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05).</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">The study agrees with the literature on the predominance of OASIS in forceps deliveries. Cincuenta mil veintidós mujeres (83,17%) tuvieron un parto vaginal: 82,12% partos eutócicos y 17,22% partos instrumentados. En el 81% se practicó anestesia locorregional y en el 59,7% fue la tasa global de episiotomía. Trescientas sesenta y nueve mujeres sufrieron OASIS, constituyendo el 0,74% del total de partos vaginales; la mayoría detectados en fórceps de Kjelland (46,8%) y partos eutócicos (41,5%).
