Accordingly, almost one-third of MDRPI occurs in the ICU.<a class="elsevierStyleCrossRefs" href="#bib0105">4,5</a> Given the large number of reports on MDRPI incidence in hospitals, especially in the ICU, identifying factors that contribute become necessary.The characteristics of MDRPI generally similar to the form of the medical devices.<a class="elsevierStyleCrossRef" href="#bib0090">1</a> Both intervention and diagnostic tools are the risk factor of MDRPI. The most commonly reported medical devices related to PI include respiratory devices.<a class="elsevierStyleCrossRef" href="#bib0115">6</a> Another study reported the use of a nasogastric tube (NGT) and endotracheal tube (ETT).<a class="elsevierStyleCrossRef" href="#bib0120">7</a> Regarding the anatomic locations of MDRPIs and non-MDRPIs tend to differ. In addition, 70.3% of MDRPI events have been reported to occur in the head.<a class="elsevierStyleCrossRef" href="#bib0095">2</a> One study showed that 32.6% (74/227) of MDRPIs occurred in the nose, 14.1% (32/227) in the legs and 8% (20/227) in the hands and forehead.<a class="elsevierStyleCrossRef" href="#bib0125">8</a> The fact mentioned above indicates the importance of preventive strategies.The first corner of preventive strategies is early detection. Currently, skin temperature has been widely used as predictors or PI. Increasing Skin temperature associated with risk of PI.<a class="elsevierStyleCrossRef" href="#bib0130">9</a> Thus, changes in Skin temperature can be considered independent factors that contribute significantly to the incidence of PI.<a class="elsevierStyleCrossRef" href="#bib0135">10</a> Increased Skin temperature consistently decreases skin tolerance in cases of superficial pressure injury (SPI).<a class="elsevierStyleCrossRef" href="#bib0140">11</a> Our previous study concluded that temperature could be a predictor of PI.<a class="elsevierStyleCrossRef" href="#bib0145">12</a> Therefore, we postulate that skin temperature might be useful as a predictor of MDRPI. Thus, the present study sought to determine whether a relationship exists between Skin temperature and the incidence of MDRPI.MethodsThis was a prospective study with a purposive sampling technique. A repeated measurement in adult patients admitted to the Intensive Care Unit (ICU) in a regional hospital across eastern Indonesia. The inclusion criteria were participants aged≥17 years, hospitalized in the ICU for the first 24h, and attached with medical devices. Our independent variable was skin temperature. Skin temperature was measured using FLIR ONE thermal imaging devices. The picture is taken perpendicular to the skin where the device is attached. Measurements were taken seven times (first, third, fifth, seventh, ninth, eleventh, and thirteenth day) in the same position.The medical devices were categorized into intervention devices and diagnostic devices. Intervention devices include non-rebreathing masks, nasal cannulas, non-positive pressure masks, NGTs, ETTs, tracheostomies, IV lines, central venous pressure, drains, CVP, urinary catheters, oropharyngeal airway, stoma bags, and arterial lines. Diagnostic devices include bandages, blood pressure cuffs, pulse oximetry, splints, neck collars, back support, external fixation, identity bands, electrode pads, restraints, and tracheostomy straps. As the dependent variable, the incidence of MDRPI events was observed using an observation sheet.Mann- Whitney test and Friedman test was used to determine the relationship between Skin temperature and incidence of MDRPI. All statistical analyses were performed using IBM SPSS 24. Ethical approval was obtained from the ethics committee of the Faculty of Medicine, University of Hasanuddin Makassar (Number: 245/UN4.6.4.5.31/PP36/2019).ResultsOf the 50 participants, thirteen (26%) developed MDRPI. The mean age of those who developed MDRPI was 37 years and was more prevalent in females (14%, 7/13) than in males (12%, 6/13). Among those who developed MDRPI, 10% (5/13) diagnosed as traumatic brain injury (TBI), 6% (3/13) tumors, 4% (2/13) spinal trauma, digestive disorders, and 2% (1/13) had other diagnoses. Laboratory findings indicate no relationship with MDRPI events (p>0.05) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).<elsevierMultimedia ident="tbl0005"></elsevierMultimedia>Mann–Whitney test found a significant difference in mean rank temperature between MDRPI and non-MDRPI group. Among of intervention device, the difference found ad 5th day (34.92 vs 22.19°C), 7th day (38.04 vs 21.09°C), 9th day (40.23 vs 20.32°C), 11th day (33.54 vs 22.68°C), and 13th day (31.00 vs 23.57°C). The similar finding also among diagnostic device. The difference found at 5th day (33.19 vs 22.80°C), 7th day (39.08 vs 20.73°C), 9th day (40.73 vs 20.15°C), 11th day (34.00 vs 22.51°C), and 13th day (31.00 vs 23.57°C) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The Friedman test also confirmed the difference in skin temperature during the observation days for intervention devices and diagnostic devices (p<0.05) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).<elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia>DiscussionThe present study aimed to determine the relationship between skin temperature and MDRPI incidence. Studies have shown that decreased, and increased skin temperature is indirect risk factors for PI events.<a class="elsevierStyleCrossRef" href="#bib0150">13</a> Accordingly, one study found that increased skin temperature consistently decreases skin tolerance in SPI,<a class="elsevierStyleCrossRef" href="#bib0140">11</a> whereas another showed that changes in skin temperature are independent factors that contribute to the incidence of PI.<a class="elsevierStyleCrossRef" href="#bib0135">10</a> Moreover, studies have shown that temperature can be used to predict the occurrence of PI events.<a class="elsevierStyleCrossRef" href="#bib0145">12</a>Incidence of MDRPIThis study had been conducted on ICU patients who are 2.4 times more likely to develop MDRPIs.<a class="elsevierStyleCrossRef" href="#bib0105">4</a> Our findings showed that a quarter of the samples developed MDRPI. Previous studies have reported an MDRPI incidence of 27.9–40%, the majority of which occurred in the ICU.<a class="elsevierStyleCrossRefs" href="#bib0105">4,5,14</a> One study reported that 15 out of 48 PI events in the ICU are related by medical devices.<a class="elsevierStyleCrossRef" href="#bib0120">7</a> In Indonesia alone, research among children found that 8% of the PIs were due to the use of medical devices.<a class="elsevierStyleCrossRef" href="#bib0160">15</a> Previous study indicate that MDRPI events were 2.07 times greater among participants using mechanical ventilators, 2.07 times greater among those using anticoagulants, and 2.56 times greater among those who were sedated.<a class="elsevierStyleCrossRef" href="#bib0155">14</a> This suggests that nurses, especially those in charge of the ICU, should increase their awareness regarding MDRPI events.MDRPI based on the type of deviceThe incidence of MDRPI is directly proportional to the number and type of medical devices used. A quarter of all medical devices observed herein caused by MDRPI. Another study revealed that 211 medical devices caused 340 (60.2%) incidences of MDRPI.<a class="elsevierStyleCrossRef" href="#bib0155">14</a> The present study categorized medical devices into intervention, and diagnostic device. The previous study confirmed that medical devices in the ICU were the most common causes of PI.<a class="elsevierStyleCrossRef" href="#bib0155">14</a> Moreover, some medical devices occasionally prevent nurses from checking the skin. An example of this is the occurrence of pressure sores on the heel, which may be covered by compression stockings.<a class="elsevierStyleCrossRef" href="#bib0105">4</a> We found skin temperature increase both intervention and diagnostic devices.Skin temperature and MDRPIOur findings show that skin temperature contributes significantly to MDRPI events. There is a significant difference in skin temperature increase between patients who have MDRPI, and without MDRPI from both intervention and diagnostic devices on days five, seven, nine, eleven, and thirteen. Although the reported events generally occur on the seventh day, this does not indicate that previous days can be ignored where the temperature increase also occurs on the third day on the intervention device and the fifth day on the diagnostic tool. Besides, skin changes, including an increase in skin temperature, are also reported as predictors of MDRPI.<a class="elsevierStyleCrossRef" href="#bib0165">16</a> Therefore, given the inappropriate and strict use of medical devices has the potential to cause MDRPI. To date, recommendations for international guidelines for PIs related to the use of medical devices have not considered temperature as a risk factor for MDRPI events.<a class="elsevierStyleCrossRef" href="#bib0170">17</a> Therefore, the potential skin temperature for causing MDRPI events should be reviewed further, including larger samples and treatment units, besides ICU.LimitationsDuring measurements, several samples for medical devices were not measured, given that some of them, especially diagnostic tools, occasionally went unnoticed. The samples obtained were still lacking due to the limited study time. Lastly, given that the room temperature tended to be consistently lower than compared to the non-ICU setting, which should be considered to generalize this finding to non-ICU patients.ConclusionSkin temperature significantly contributed to the incidence of MDRPI of intervention and diagnostic device used in the ICU setting. has currently progressed&#44; with medical devices being considered as one of etiology&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">1</span></a> One study showed a significant correlation between PI and the use of medical devices&#44; with nearly 30&#37; of the reported PIs being related to the use of medical devices&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">2</span></a> Another study revealed that medical device-related pressure injuries &#40;MDRPIs&#41; accounted for 5&#46;48&#37; of all PI cases&#46;<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">3</span></a> Studies have shown that MDRPIs generally occur in the intensive care unit &#40;ICU&#41;&#46; Accordingly&#44; almost one-third of MDRPI occurs in the ICU&#46;<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">4&#44;5</span></a> Given the large number of reports on MDRPI incidence in hospitals&#44; especially in the ICU&#44; identifying factors that contribute become necessary&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">The characteristics of MDRPI generally similar to the form of the medical devices&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">1</span></a> Both intervention and diagnostic tools are the risk factor of MDRPI&#46; The most commonly reported medical devices related to PI include respiratory devices&#46;<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">6</span></a> Another study reported the use of a nasogastric tube &#40;NGT&#41; and endotracheal tube &#40;ETT&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">7</span></a> Regarding the anatomic locations of MDRPIs and non-MDRPIs tend to differ&#46; In addition&#44; 70&#46;3&#37; of MDRPI events have been reported to occur in the head&#46;<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">2</span></a> One study showed that 32&#46;6&#37; &#40;74&#47;227&#41; of MDRPIs occurred in the nose&#44; 14&#46;1&#37; &#40;32&#47;227&#41; in the legs and 8&#37; &#40;20&#47;227&#41; in the hands and forehead&#46;<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">8</span></a> The fact mentioned above indicates the importance of preventive strategies&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">The first corner of preventive strategies is early detection&#46; Currently&#44; skin temperature has been widely used as predictors or PI&#46; Increasing Skin temperature associated with risk of PI&#46;<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">9</span></a> Thus&#44; changes in Skin temperature can be considered independent factors that contribute significantly to the incidence of PI&#46;<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">10</span></a> Increased Skin temperature consistently decreases skin tolerance in cases of superficial pressure injury &#40;SPI&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">11</span></a> Our previous study concluded that temperature could be a predictor of PI&#46;<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">12</span></a> Therefore&#44; we postulate that skin temperature might be useful as a predictor of MDRPI&#46; Thus&#44; the present study sought to determine whether a relationship exists between Skin temperature and the incidence of MDRPI&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This was a prospective study with a purposive sampling technique&#46; A repeated measurement in adult patients admitted to the Intensive Care Unit &#40;ICU&#41; in a regional hospital across eastern Indonesia&#46; The inclusion criteria were participants aged<span class="elsevierStyleHsp" style=""></span>&#8805;<span class="elsevierStyleHsp" style=""></span>17 years&#44; hospitalized in the ICU for the first 24<span class="elsevierStyleHsp" style=""></span>h&#44; and attached with medical devices&#46; Our independent variable was skin temperature&#46; Skin temperature was measured using FLIR ONE thermal imaging devices&#46; The picture is taken perpendicular to the skin where the device is attached&#46; Measurements were taken seven times &#40;first&#44; third&#44; fifth&#44; seventh&#44; ninth&#44; eleventh&#44; and thirteenth day&#41; in the same position&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">The medical devices were categorized into intervention devices and diagnostic devices&#46; Intervention devices include non-rebreathing masks&#44; nasal cannulas&#44; non-positive pressure masks&#44; NGTs&#44; ETTs&#44; tracheostomies&#44; IV lines&#44; central venous pressure&#44; drains&#44; CVP&#44; urinary catheters&#44; oropharyngeal airway&#44; stoma bags&#44; and arterial lines&#46; Diagnostic devices include bandages&#44; blood pressure cuffs&#44; pulse oximetry&#44; splints&#44; neck collars&#44; back support&#44; external fixation&#44; identity bands&#44; electrode pads&#44; restraints&#44; and tracheostomy straps&#46; As the dependent variable&#44; the incidence of MDRPI events was observed using an observation sheet&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">Mann- Whitney test and Friedman test was used to determine the relationship between Skin temperature and incidence of MDRPI&#46; All statistical analyses were performed using IBM SPSS 24&#46; Ethical approval was obtained from the ethics committee of the Faculty of Medicine&#44; University of Hasanuddin Makassar &#40;Number&#58; 245&#47;UN4&#46;6&#46;4&#46;5&#46;31&#47;PP36&#47;2019&#41;&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">Of the 50 participants&#44; thirteen &#40;26&#37;&#41; developed MDRPI&#46; The mean age of those who developed MDRPI was 37 years and was more prevalent in females &#40;14&#37;&#44; 7&#47;13&#41; than in males &#40;12&#37;&#44; 6&#47;13&#41;&#46; Among those who developed MDRPI&#44; 10&#37; &#40;5&#47;13&#41; diagnosed as traumatic brain injury &#40;TBI&#41;&#44; 6&#37; &#40;3&#47;13&#41; tumors&#44; 4&#37; &#40;2&#47;13&#41; Studies have shown that decreased&#44; and increased skin temperature is indirect risk factors for PI events&#46;<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">13</span></a> Accordingly&#44; one study found that increased skin temperature consistently decreases skin tolerance in SPI&#44;<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">11</span></a> whereas another showed that changes in skin temperature are independent factors that contribute to the incidence of PI&#46;<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">10</span></a> Moreover&#44; studies have shown that temperature can be used to predict the occurrence of PI events&#46;<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">12</span></a></p><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Incidence of MDRPI</span><p id="par0050" class="elsevierStylePara elsevierViewall">This study had been conducted on ICU patients who are 2&#46;4 times more likely to develop MDRPIs&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">4</span></a> Our findings showed that a quarter of the samples developed MDRPI&#46; Previous studies have reported an MDRPI incidence of 27&#46;9&#8211;40&#37;&#44; the majority of which occurred in the ICU&#46;<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">4&#44;5&#44;14</span></a> One study reported that 15 out of 48 PI events in the ICU are related by medical devices&#46;<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">7</span></a> In Indonesia alone&#44; research among children found that 8&#37; of the PIs were due to the use of medical devices&#46;<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">15</span></a> Previous study indicate that MDRPI events were 2&#46;07 times greater among participants using mechanical ventilators&#44; 2&#46;07 times greater among those using anticoagulants&#44; and 2&#46;56 times greater among those who were sedated&#46;<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">14</span></a> This suggests that nurses&#44; especially those in charge of the ICU&#44; should increase their awareness regarding MDRPI events&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">MDRPI based on the type of device</span><p id="par0055" class="elsevierStylePara elsevierViewall">The incidence of MDRPI is directly proportional to the number and type of medical devices used&#46; A quarter of all medical devices observed herein caused by MDRPI&#46; Another study revealed that 211 medical devices caused 340 &#40;60&#46;2&#37;&#41; incidences of MDRPI&#46;<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">14</span></a> The present study categorized medical devices into intervention&#44; and diagnostic device&#46; The previous study confirmed that medical devices in the ICU were the most common causes of PI&#46;<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">14</span></a> Moreover&#44; some medical devices occasionally prevent nurses from checking the skin&#46; An example of this is the occurrence of pressure sores on the heel&#44; which may be covered by compression stockings&#46;<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">4</span></a> We found skin temperature increase both intervention and diagnostic devices&#46;</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Skin temperature and MDRPI</span><p id="par0060" class="elsevierStylePara elsevierViewall">Our findings show that skin temperature contributes significantly to MDRPI events&#46; There is a significant difference in skin temperature increase between patients who have MDRPI&#44; and without MDRPI from both intervention and diagnostic devices on days five&#44; seven&#44; nine&#44; eleven&#44; and thirteen&#46; Although the reported events generally occur on the seventh day&#44; this does not indicate that previous days can be ignored where the temperature increase also occurs on the third day on the intervention device and the fifth day on the diagnostic tool&#46; Besides&#44; skin changes&#44; including an increase in skin temperature&#44; are also reported as predictors of MDRPI&#46;<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">16</span></a> Therefore&#44; given the inappropriate and strict use of medical devices has the potential to cause MDRPI&#46; To date&#44; recommendations for international guidelines for PIs related to the use of medical devices have not considered temperature as a risk factor for MDRPI events&#46;<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">17</span></a> Therefore&#44; the potential skin temperature for causing MDRPI events should be reviewed further&#44; including larger samples and treatment units&#44; besides ICU&#46;</p></span></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Limitations</span><p id="par0065" class="elsevierStylePara elsevierViewall">During measurements&#44; several samples for medical devices were not measured&#44; given that some of them&#44; especially diagnostic tools&#44; occasionally went unnoticed&#46; The samples obtained were still lacking due to the limited study time&#46; Lastly&#44; given that the room temperature tended to be consistently lower than compared to the non-ICU setting&#44; which should be considered to generalize this finding to non-ICU patients&#46;</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Conclusion</span><p id="par0070" class="elsevierStylePara elsevierViewall">Skin temperature significantly contributed to the incidence of MDRPI of intervention and diagnostic device used in the ICU setting&#46;
