Con la denominación genérica de enfermedades autoinmunes sistémicas se intentan agrupar diversas entidades clínicas que comparten una base común como es la lesión anatomopatológica del tejido conjuntivo. La etiología es todavía desconocida, aunque probablemente es multifactorial y comprende en su proceso fisiopatológico la autoinmunidad, la patología de la inflamación, las alteraciones genéticas y los agentes infecciosos. Aunque todavía no existe un tratamiento definitivo para su curación, con la introducción de los corticoides e inmunosupresores se ha conseguido una modificación importante en su historia natural. Sin embargo, han aumentado también el número de complicaciones asociadas al tratamiento crónico con estos fármacos. Así, los tratamientos con corticoides, inmunosupresores o antiinflamatorios no esteroideos son capaces de provocar, entre otras complicaciones, una elevación de la presión arterial o el empeoramiento de una hipertensión arterial esencial ya existente, que es la enfermedad más frecuente de cuantas afectan a la humanidad. Por otra parte, la afección renal que acontece en la evolución natural de algunas enfermedades autoinmunes sistémicas también puede provocar el desarrollo de una hipertensión arterial secundaria.
En el presente artículo se revisan los principales fármacos utilizados en el tratamiento de las enfermedades autoinmunes que pueden provocar hipertensión, así como su posible tratamiento. Asimismo, se revisa el manejo de la hipertensión arterial asociada a una afección renal, hecho frecuente en las enfermedades autoinmunes sistémicas.
The generic classification of systemic autoimmune disease is actually an attempt to categorize a diverse group of clinical entities which has as its foundation an anatomopathologically diagnosed conjunctive tissue lesion. The ethiology is still unknown, but is probably multifactorial and includes in its pathophysiological process autoimmunity, inflammation pathology, genetic alterations and infectious agents. While a definitive treatment still has not been found as a cure, the introduction of corticoids and immunosuppressors has served to significantly alter its usual evolution. On the other hand, an increased number of complications are known to have been associated with chronic treatment using these drugs. Treatments with corticoids, non-steroid anti-inflammatory drugs or inmunosuppresors are able to cause, among other complications, rises in blood pressure and the worsening of already existing essential hypertension, which is the most commonly occurring disease in the general population. What is more, the renal affectation that also occurs in the natural evolution of some systemic autoimmune diseases can provoke the development of secondary hypertension. The present article offers both a review of the main drugs used in the treatment of autoimmune diseases which can cause hypertension, and discusses possible treatments for such diseases. Also reviewed is the management of hypertension associated with renal affectation, a common occurrence in systemic autoimmune diseases.
