"textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Antisynthetase syndrome (ASS) is characterized by the presence of fever, interstitial lung disease, arthritis, "mechanic's hands" and Raynaud's phenomenon in more than 30% of patients with dermato- or polymyositis. Immunologically positive for antibodies against aminoacyl-tRNA synthetase (aaRS), including anti-Jo-1, PL-7, PL-12, EJ, OJ, KS, YRS, Zo, being mutually exclusive. However, not all patients present these symptoms, with the diagnosis being considered acceptable if they have, in addition to the antibodies, 2 of the following characteristics: interstitial lung disease, inflammatory myopathy and polyarthritis. The most common antibody is anti-Jo-1, which appears in up to 20% of cases. The existence of clinical differences associated with the various autoantibodies has been proposed.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Given the great variability in its presentation, we have carried out a retrospective descriptive study of patients with positive aaRS determination by immunoblot registered from January 2000 to June 2017, collecting the main epidemiological variables, as well as the clinical manifestations usually associated with ASS and the associated neoplasm diagnosis.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The cases of 17 aaRS-positive patients were collected, occurring more commonly in women, 12 (70.6%), with a mean age of 56 (age range between 25 and 73) when it was first detected. Anti-Jo-1 was the most common according to the literature. The coexistence of anti-Ro is remarkable (in 10 of the 17 patients, 58.8%), which was also more commonly associated with the occurrence of interstitial lung disease (in 7 of the 8 patients with ILD, 87.5%). This was more common in men (62.55%), while muscle weakness was more common in women (55.8%). As for other clinical manifestations, the most common was fever, in one out of every 3 patients, followed by muscle weakness. Dyspnoea, 8 (47.1%), dysphagia, 8 (47.1%) and Raynaud's phenomenon, 6 (35.5%) were also present.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Among patients with aaRS, 13 (76.4%) could be classed as ASS as they met 2 of the 3 main clinical characteristics (DM/PM, interstitial lung disease and arthritis). It should be noted that, although "mechanic's hands" are considered a typical symptom, in our series, their existence was only recorded in the medical history of one patient.</p><p id="par0025" class="elsevierStylePara elsevierViewall">In this study, we have observed 2 cases of neoplasm (11.7%), with both being diagnosed before the positive determination of aaRS: a lung adenocarcinoma associated with anti-EJ and an invasive ductal breast cancer associated with anti-PL7. Both were women, with a mean ASS diagnosis age of 60.5 years. Furthermore, in both cases, muscle weakness, weight loss, and dysphagia were found, and none of them had any respiratory symptoms.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The association between dermatomyositis and the development of cancer is known.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> According to Hill et al., 13%–23% of dermatomyositis cases are associated with various types of malignancies, particularly in the ovary and lung.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> The Pinal-Fernández et al. cohort, with 169 patients, showed that the frequency of cancer associated with aaRS does not seem higher than that of the general population, and found no association with anti-Ro co-positive results.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> The Chinese cohort of Shi et al., with 124 patients, found that 6.5% developed malignancies, with a mean time between neoplasm and ASS of 3 years, with the majority also developing interstitial lung disease.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">On the other hand, in the review carried out by Boleto et al. of the cases of cancer associated with aaRS, it seems that being male, age over 60 and the coexistence with anti-SSA/Ro favour the development of the neoplasm.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> However, there are many limitations, since the study involved small cohorts and retrospective studies.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In the present study we can conclude that not all patients with aaRS develop the manifestations of ASS, in addition, according to the literature, the coexistence of anti-Ro antibodies was associated with the development of interstitial lung disease.
