The popularisation of the “keg parties” phenomenon is a habit that causes concern. The last State Survey on Drug Use in Secondary Schools (Encuesta Estatal sobre Uso de Drogas en Enseñanzas Secundarias, ESTUDES) 2012/20131 shows that the prevalence of the consumption of alcoholic beverages took an upturn in 2012 and, in spite of the ascending global tendency of boozing observed in previous years, the percentage of young people getting drunk is lower; however, this percentage is still high (3 out of 10 during the last month). Likewise, we confirm alcohol consumption is changing in our society, from a pattern characterised by frequent consumption in low quantities, to a consumption pattern of elevated doses in a few hours (binge drinking). In spite of the alarm raised in the media during the last year based on the draft bill for the prevention of alcohol consumption in minors, in which economic fines are proposed for parents of patients assisted due to acute alcohol intoxication (AAI) in health centres,2 this phenomenon is not new.3 Some authors alert that repeatedly considering drinking teenagers as an increasing problem bears the risk of “exaggerating” the problem.4 Therefore, it is important to know what the real repercussions are of consultations for AAI in the Emergency Department to weigh its real implications. For such purpose, and with the aim of assessing changes in the incidence and profile of AAIs, a new retrospective review of all teenager (12–18 years old) consultations was carried out in an emergency department during a period of 6 years (2007–2012). A total of 513 AAIs were included, and Fig. 1 contains the incidence rate and data related to the profile of patients for each of the analysed years. In the figure, we note that at the study centre, the annual rate of AAI consultations tends to decrease, and this must be interpreted with caution given the short period of time analysed; however, it could indicate a change in tendency regarding the conclusions drawn by Azkunaga et al.5 in the publication on epidemiological changes in intoxications treated in Spanish paediatric emergency departments, which alerts on a significant increase of AAIs. Part of the differences may be related to the periods of time under analysis. Azkunaga et al.5 compared the two-year period 2001–2002 with the period from October, 2008 to September, 2010, whereas the study in question analysed the evolution from 2007 to 2012, that is to say, a subsequent and more recent period, during which the decrease in AAI is observed. Regarding the results of the ESTUDES survey 2012/2013,1 we note a parallel evolution of the changes detected in “boozing” in the last 30 days for 14 to 18-year old students and consultations for AAI in the emergency department, detecting a decrease from 2010 to 2012. If we analyse the profile of those intoxicated, we confirm what we already knew: they are teenagers, around 16 years old, with no history of psychiatric disorders, arriving in an ambulance due to isolated moderate AAI during the night on weekends, where gender is not a determinant factor.6–8 When assessing the different characteristics, the profile is quite stable during the 6-year period, though there are specific differences among isolated years. Only significant year-on-year differences are detected in the 2 following aspects: the number of night consultations, but with no clear tendency, as observed since the last 2 analysed years are those with the lowest and highest proportion of AAI; and in age of patients, who are a little bit younger as years go by, although the difference is, at this time, of little clinical relevance. On the other hand, and although it is not significant, an increase in diagnosed cases in teenagers with history of psychiatric disease stands out, as well as the cases of consumption of other substances, mainly cannabis, and the relation between both facts has been confirmed.9,10 It will be interesting to assess the evolution of these consultations during the next few years in order to clarify if there is a change in the type of treated cases, which are increasingly complex, or simply small year-on-year changes with no major repercussions. The main limitations of the study in question are due to the fact that it was carried out in only one health centre, with high level assistance and in a certain geographical area, and also due to the brief period of time analysed. However, it is a starting point for future works in which it would be interesting to involve health centres with a different complexity level. Monitoring the problem of alcohol abuse in teenagers is essential to plan for effective interventions, and emergency departments are a key element to do that.
<elsevierMultimedia ident="fig0005"></elsevierMultimedia>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "
Please cite this article as: Trenchs Sainz de la Maza V, Curcoy Barcenilla AI, Arias Constantí V, Luaces Cubells C.
