Physical examination revealed generalized pustules with central umbilication admixed with erythematous excoriated papules (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1A</a>) located predominately in flexures (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1B</a>), with hands and penile shaft involvement (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1C</a>). He shared a room with 3 partners, none of whom had similar symptomatology, and acknowledged an insertive condomless vaginal sex intercourse one-week before symptoms onset. The patient was admitted for intravenous clindamycin treatment and dressings. Pustules’ swabs for real-time polymerase chain reaction (rt-PCR) ruled out Mpox infection. A disseminated gonococcal infection was excluded through blood and urethral cultures. Finally, a pustule culture identified <span class="elsevierStyleItalic">Streptococcus pyogenes</span>. A thorough physical examination after dermatologic consultation revealed 5–10<span class="elsevierStyleHsp" style=""></span>mm curvilinear desquamative thread-like tracks in hands web spaced, some of them with “delta-wing” sign (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1D</a>), compatible with scabies mite burrows.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Bacterial superinfection can modify scabies’ classic clinical picture.
