On examination, three smooth surface erythematous violaceous plaques were observed, of firm consistency on palpation, in the back of his right hand and the second finger of the same hand (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A). Dermoscopy showed a whitish hyperkeratotic central area, on an erythematous base, adopting a white starburst-like pattern. Discretely irregular linear vessels (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B) were observed peripherally. Histological examination showed non-necrotizing epithelioid granulomas in the middle and deep dermis with presence of amastigotes of leishmania. The diagnosis was acute cutaneous leishmaniasis. Treatment was initiated by intralesional infiltration of 0.2<span class="elsevierStyleHsp" style=""></span>ml of meglumine antimoniate divided between the 3 lesions every 2 weeks, with good clinical response.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Cutaneous leishmaniasis is an endemic protozoan disease in our country, mainly caused by <span class="elsevierStyleItalic">Leishmania infantum</span> and transmitted by the bite of female mosquitoes of the <span class="elsevierStyleItalic">Phlebotomus</span> and <span class="elsevierStyleItalic">Lutzomya</span> genera. Given the clinical variability, dermoscopy is a useful tool for its suspicion, although the determination of the parasite is necessary.
