Physical examination revealed a pulmonary systolic murmur. A chest X-ray showed an enlarged cardiac silhouette (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Thoracic CT angiography showed a mediastinal mass of 142 × 156 × 158 mm (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>, arrow), deforming and infiltrating the left atrium (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>, asterisk), producing mild stenosis of the pulmonary trunk and pulmonary veins.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Crude needle biopsy was compatible with synovial sarcoma with presence of SYT gene translocation (18q11.2).
