FACS: <span class="elsevierStyleItalic">Facial Action Coding Systemgr</span> 1.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">The most widely accepted definition of pain is the one proposed by the <span class="elsevierStyleItalic">International Association for the Study of Pain</span>, which defines it as “an unpleasant subjective sensory and emotional experience associated with present or potential injury”.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">1</span></a> In the clinical setting its measurement is essential regarding treatment decisions, the appropriate use of analgesia and the assessment of treatment efficacy. However, each individual's biological reaction, age, cognitive status and, moreover, his/her own experience to suffering, emotional state or pain expectation hinder its assessment.<a class="elsevierStyleCrossRefs" href="#bib0290"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Pain assessment scales are the tools used to detect its presence. In the clinical setting there are various types of patients, and therefore, assessment scales should be appropriate for each type.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The pain measurement tools most frequently used are: the visual analogue scale,<a class="elsevierStyleCrossRefs" href="#bib0305"><span class="elsevierStyleSup">5,6</span></a> the verbal numeric scale<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">7</span></a> or verbal descriptive scale.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">5</span></a> However, these are only useful in conscious patients, able to communicate. Physiological<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">4,8,9</span></a> and/or behavioral<a class="elsevierStyleCrossRefs" href="#bib0330"><span class="elsevierStyleSup">10–12</span></a> indicators have been used for those who present loss or impairment in communication capacity, such as new-borns, patients with cognitive and intellectual disabilities, with dementia or in critical condition. The latter ones are based on body postures and facial movements, but its main limitation is the inability to quantify pain intensity.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Significant efforts have been made to identify with reliability and validity the facial indicators.<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">13</span></a> The <span class="elsevierStyleItalic">Facial Action Coding System</span> (FACS), proposed in 1978 by Ekman and Friesen<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">14</span></a> is undoubtedly the most widely used method for coding facial expressions in research.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">15</span></a> FACS was designed to provide objective descriptions of facial activity of 6 basic emotions: joy, disgust, anger, fear, surprise and sadness, minimizing the use of subjective evaluations.<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">16</span></a> The use and the criteria for detection and quantification of facial expressions are performed by a trained coder (qualified after passing a test supervised by the developers of the system requiring 70% accuracy in coding),<a class="elsevierStyleCrossRef" href="#bib0365"><span class="elsevierStyleSup">17</span></a> identifying and discriminating 44 <span class="elsevierStyleItalic">action units</span> (AU, “facial action units”) separated with the help of slow motion in video editing equipment. In FACS, both the frequency as well as the intensity of each AU are classified in a 5-point scale (A–E).</p><p id="par0025" class="elsevierStylePara elsevierViewall">The expression of pain has been described as being unique and different from the 6 basic emotions.<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">18</span></a> The typical configuration of pain on the face has been identified in infants, children and adults.<a class="elsevierStyleCrossRef" href="#bib0375"><span class="elsevierStyleSup">19</span></a> This expression is coded in AU, and the essential information about pain includes the following: the descent of the eyebrows (AU4), the raising of the cheeks and eyelid compression and/or contraction of the cheekbones (AU6/AU7), wrinkling of the nose (AU9), raising the upper lip (AU10) and eye closure (AU43). Prkachin and Solomon<a class="elsevierStyleCrossRef" href="#bib0380"><span class="elsevierStyleSup">20</span></a> defined the pain as the sum of intensities of these “fundamental AU's” contained in most expressions of pain to produce a 16-point scale.Pain=AU4+max(AU6|AU7)+max(AU9|AU10)+AU43</p><p id="par0030" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the studies in which the fundamental AU and the AU associated with the expression of pain are defined.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">The validity and reliability of the FACS system has been demonstrated repeatedly in pain research.<a class="elsevierStyleCrossRefs" href="#bib0400"><span class="elsevierStyleSup">24,25</span></a> It has also been used to differentiate the real pain from the simulated pain.<a class="elsevierStyleCrossRefs" href="#bib0385"><span class="elsevierStyleSup">21,26,27</span></a> Even specific subsystems are designed based on FACS for pain assessment, including <span class="elsevierStyleItalic">Neonatal Facial Coding System</span> (NFCS)<a class="elsevierStyleCrossRef" href="#bib0420"><span class="elsevierStyleSup">28</span></a> for new-borns to 2 months of age, or <span class="elsevierStyleItalic">Child Facial Coding System</span><a class="elsevierStyleCrossRef" href="#bib0420"><span class="elsevierStyleSup">28</span></a> for children between one and six years of age.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Based on the foregoing, in this paper we review the published scientific evidence on pain evaluation by the FACS system.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Materials and methods</span><p id="par0045" class="elsevierStylePara elsevierViewall">This systematic review was conducted in accordance with the recommendations of the Cochrane Collaboration.<a class="elsevierStyleCrossRefs" href="#bib0425"><span class="elsevierStyleSup">29,30</span></a> The studies were searched in the PubMed, Science Direct and Scopus databases. A search was conducted from 1982 to June 2014 with the keywords “facial action coding system pain” under the inclusion criteria shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>. Reference search was narrowed using the keyword “FACS” in the titles, abstracts and keywords. Two independent reviewers screened the articles and those not meeting the inclusion criteria (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) were discarded. The search strategy is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Figure 1</a>. Methodological quality was assessed using the GRADE system.<a class="elsevierStyleCrossRef" href="#bib0435"><span class="elsevierStyleSup">31</span></a></p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">The results to the search “facial action coding system pain” showed 4335 references. When the search was restricted to the use of “FACS” system in the title, abstract and key words, 40 references were obtained (after the elimination of duplicates). After reading the articles, 14 of them were discarded due to not assessing pain by the FACS system, not evaluating pain expression through it or not using this system. A conference and a study that used the FACS system on <span class="elsevierStyleItalic">Rhesus</span> macaques was also dismissed. Finally, 26 articles were used, of which the most relevant details are presented in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> and are used for this review. Some older articles that provide different angles to the discussion were also analyzed, mainly from a methodological point of view. The results of the methodological quality and strength of recommendation are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Discussion</span><p id="par0055" class="elsevierStylePara elsevierViewall">As pain is a subjective experience, self-report is traditionally considered the best access to measuring and quantifying it. However, there is no guarantee that these will accurately reflect the painful experience produced during and after a noxious stimulus.<a class="elsevierStyleCrossRefs" href="#bib0440"><span class="elsevierStyleSup">32,51</span></a> Most studies cannot show a significant correlation between self-reports and results with FACS. There are people who, because of age (infants) or having impaired cognition (mental retardation or dementia), are not able to provide valid self-reports.<a class="elsevierStyleCrossRefs" href="#bib0405"><span class="elsevierStyleSup">25,40,46,47,52</span></a> However, FACS has proven an objective tool for all cases.<a class="elsevierStyleCrossRef" href="#bib0460"><span class="elsevierStyleSup">36</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">The degree of cognitive impairment significantly affects pain behaviour. There are sensitive tools to detect pain at all levels of cognitive deficiencies, such as the <span class="elsevierStyleItalic">Non-communicating Children's Pain Checklist</span>, but FACS is more suitable for mild to moderate cognitive deficiencies.<a class="elsevierStyleCrossRef" href="#bib0480"><span class="elsevierStyleSup">40</span></a> In normal conditions,<a class="elsevierStyleCrossRefs" href="#bib0480"><span class="elsevierStyleSup">40,42</span></a> the severe-deep cognitive deficiencies show a higher facial expression, which significantly increase the descent of the eyebrows (AU4) and chin lift (AU17),<a class="elsevierStyleCrossRef" href="#bib0455"><span class="elsevierStyleSup">35</span></a> compared with mild-moderate cognitive deficiencies. However, FACS can be used with the same reliability in all patients,<a class="elsevierStyleCrossRef" href="#bib0530"><span class="elsevierStyleSup">50</span></a> regardless of the cognitive status of the same.<a class="elsevierStyleCrossRef" href="#bib0485"><span class="elsevierStyleSup">41</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Facial expressions do not only represent pain; they can coexist with other emotions.<a class="elsevierStyleCrossRef" href="#bib0465"><span class="elsevierStyleSup">37</span></a> During pain, negative emotional expressions increase, such as displeasure,<a class="elsevierStyleCrossRefs" href="#bib0400"><span class="elsevierStyleSup">24,39</span></a> disgust,<a class="elsevierStyleCrossRef" href="#bib0400"><span class="elsevierStyleSup">24</span></a> fear, sadness, anger<a class="elsevierStyleCrossRef" href="#bib0475"><span class="elsevierStyleSup">39</span></a> and, to a lesser extent, surprise.<a class="elsevierStyleCrossRefs" href="#bib0475"><span class="elsevierStyleSup">39,48</span></a> In patients with cognitive deficiencies it has been observed that pain could be confused with laugh or smile.<a class="elsevierStyleCrossRef" href="#bib0480"><span class="elsevierStyleSup">40</span></a> Therefore, one of the contexts where FACS is particularly useful is in situations in which patients are in critical or intensive care, since the number of facial expressions is much more limited.<a class="elsevierStyleCrossRef" href="#bib0520"><span class="elsevierStyleSup">48</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">There have been numerous studies on sex differences in pain perception.<a class="elsevierStyleCrossRef" href="#bib0495"><span class="elsevierStyleSup">43</span></a> Most of the findings are based on self-reports, which describe an increased sensitivity in the perception of pain in women.<a class="elsevierStyleCrossRefs" href="#bib0390"><span class="elsevierStyleSup">22,53</span></a> This difference may be due to biological and social interactions, or psychological factors.<a class="elsevierStyleCrossRef" href="#bib0550"><span class="elsevierStyleSup">54</span></a> The expression of emotions between the sexes also seems to be significantly different, this being greater in females.<a class="elsevierStyleCrossRef" href="#bib0390"><span class="elsevierStyleSup">22</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">Men and women do not differ in their subjective responses to pain according to FACS, but sex had a significant influence on the relationship between the self-report and the facial expression, women showing a stronger relationship than men.<a class="elsevierStyleCrossRef" href="#bib0390"><span class="elsevierStyleSup">22</span></a> Contrary to studies based on self-reports, men with cognitive deficiencies tend to exhibit more facial expressions than women.<a class="elsevierStyleCrossRef" href="#bib0480"><span class="elsevierStyleSup">40</span></a> In these type of patients, this difference could be due to their self-report results inaccuracy, as no significant differences were found in healthy subjects.</p><p id="par0080" class="elsevierStylePara elsevierViewall">In the medical field there are currently no methods to differentiate real from faked pain.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">15</span></a> Some studies show that the percentage of simulating the disease during painful stimuli is 10% in chronic patients, and much higher in medicolegal contexts.<a class="elsevierStyleCrossRef" href="#bib0555"><span class="elsevierStyleSup">55</span></a> The FACS system has proven to detect differences between real and simulated pain,<a class="elsevierStyleCrossRefs" href="#bib0385"><span class="elsevierStyleSup">21,26,27</span></a> up to 72%<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">15</span></a> and 88%<a class="elsevierStyleCrossRef" href="#bib0505"><span class="elsevierStyleSup">45</span></a> by automated systems.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The first limitation to the widespread use of FACS is the time required<a class="elsevierStyleCrossRef" href="#bib0365"><span class="elsevierStyleSup">17</span></a> for encoding, which was developed to be performed manually by personnel trained in the system. The training requires more than 100<span class="elsevierStyleHsp" style=""></span>h.<a class="elsevierStyleCrossRefs" href="#bib0365"><span class="elsevierStyleSup">17,34</span></a> NFCS is learned in less time, but only applies to newborns.<a class="elsevierStyleCrossRef" href="#bib0445"><span class="elsevierStyleSup">33</span></a> A study conducted by Solomon invests 30<span class="elsevierStyleHsp" style=""></span>min of training in Prkachin<a class="elsevierStyleCrossRef" href="#bib0385"><span class="elsevierStyleSup">21</span></a> research-based AU's, reporting potential improvements in pain detection.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Furthermore, facial expression coding is usually done outside the clinical environment, by means of watching a video of the patient.<a class="elsevierStyleCrossRef" href="#bib0525"><span class="elsevierStyleSup">49</span></a> The encoding method allows to evaluate pain at any given time (with each video <span class="elsevierStyleItalic">frame</span>), but it requires a huge amount of time, approximately 2<span class="elsevierStyleHsp" style=""></span>h for encoding, by trained personnel, 1<span class="elsevierStyleHsp" style=""></span>min of video.<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">15</span></a> The coding should be easier to use, efficient, real-time, and does not require much time.<a class="elsevierStyleCrossRef" href="#bib0450"><span class="elsevierStyleSup">34</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Some authors have developed methods to automate the system,<a class="elsevierStyleCrossRef" href="#bib0525"><span class="elsevierStyleSup">49</span></a> such as <span class="elsevierStyleItalic">Computer Expression Recognition Toolbox</span>,<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">15</span></a> based on <span class="elsevierStyleItalic">Active Appearance Models</span>,<a class="elsevierStyleCrossRef" href="#bib0500"><span class="elsevierStyleSup">44</span></a> trying to solve the problems of time and real-time detection. However, it has a high cost and it requires compliance with very specific requirements for the automated system to properly detect facial changes.<a class="elsevierStyleCrossRefs" href="#bib0355"><span class="elsevierStyleSup">15,44,49</span></a> Some of them are lack of facial hair, <span class="elsevierStyleItalic">piercings</span> or glasses, adequate lighting, frontal facial position and, in some cases, even three quarters of the face.<a class="elsevierStyleCrossRef" href="#bib0500"><span class="elsevierStyleSup">44</span></a></p><p id="par0100" class="elsevierStylePara elsevierViewall">Some studies have applied FACS on static images without the face being in its neutral position, for example, photographs of patients in different situations of severe acute pain,<a class="elsevierStyleCrossRef" href="#bib0440"><span class="elsevierStyleSup">32</span></a> or the study the characters that appear in “Los Caprichos”, by Spanish painter Francisco de Goya.<a class="elsevierStyleCrossRef" href="#bib0560"><span class="elsevierStyleSup">56</span></a> The implementation of the system in these situations cannot distinguish permanent facial features from those generated by contraction, neither can adequately rate the intensity of each AU. Therefore, its application in these cases may decrease its reliability.</p><p id="par0105" class="elsevierStylePara elsevierViewall">A potentially troubling finding is that observers tend to significantly underestimate the pain when forced to perform assessments based only on facial expression. A tendency to underestimate pain systematically, from 50% to 80%,<a class="elsevierStyleCrossRef" href="#bib0450"><span class="elsevierStyleSup">34</span></a> was found in the study conducted by Solomon et al. In clinical practice, the clinical assessment might be biased by irrelevant factors.<a class="elsevierStyleCrossRef" href="#bib0405"><span class="elsevierStyleSup">25</span></a> Another study gives evidence that doctors are not particularly sensitive to pain.<a class="elsevierStyleCrossRef" href="#bib0450"><span class="elsevierStyleSup">34</span></a> Moreover, repeated exposure to the same situation reduces the emotional response to the same. Jancar and Speller<a class="elsevierStyleCrossRef" href="#bib0540"><span class="elsevierStyleSup">52</span></a> reported that health professionals do not detect the presence of pain when the self-report is insufficient. It has been suggested that clinical care could be improved by encouraging health workers to learn these nonverbal pain signals.<a class="elsevierStyleCrossRefs" href="#bib0455"><span class="elsevierStyleSup">35,38</span></a> The training of doctors in the FACS system has proved useful and valid in the clinical application,<a class="elsevierStyleCrossRef" href="#bib0455"><span class="elsevierStyleSup">35</span></a> being more effective in detecting subtle facial expressions indicating low levels of pain.<a class="elsevierStyleCrossRef" href="#bib0450"><span class="elsevierStyleSup">34</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Conclusions</span><p id="par0110" class="elsevierStylePara elsevierViewall">FACS is an objective tool to detect and quantify pain in all patients. However, the greatest clinical utility focuses on pain assessment in patients unable to communicate properly: young children, people with mental disabilities, neurological or dementia patients in postoperative care, chronically ill or in critical condition requiring assisted ventilation. It is also useful in detecting real and simulated pain in the forensic field.</p><p id="par0115" class="elsevierStylePara elsevierViewall">However, much training time is needed for the use of the system, including micro-analytical coding procedures that can be difficult to use in routine clinical practice for pain assessment.
