id="par0005" class="elsevierStylePara elsevierViewall">Cardiovascular diseases are still the most frequent causes of morbidity and mortality in developed countries, producing an important economic, social, and personal burden. Last available data shows that in the European population, cardiovascular disease accounts for 2.2 million deaths in females and 1.9 million deaths in males.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">1</span></a> Moreover, cardiovascular disease has been estimated to cost the European Union €210 billion a year in 2015.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The prevalence of physical inactivity among adults is nowadays higher than that of any other modifiable cardiovascular risk factor. In Europe about one in every three adults are insufficiently active.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">2</span></a> Sedentary behavior is related to at least 3.2 million yearly deaths worldwide due to non-communicable disease.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">3</span></a> An increase in physical activity plays a key role in the prevention or delay of cardiovascular disease and increases life expectancy and quality of life.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Physical activity, in order to impact on health, has to be at least of moderate intensity.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">5</span></a> Although there is controversy about its optimal dose, the minimum amount of physical activity recommended to achieve unequivocal cardiovascular benefits is 150<span class="elsevierStyleHsp" style=""></span>min a week of moderate physical activity or 75<span class="elsevierStyleHsp" style=""></span>min of intense physical activity.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">6</span></a> However, only 10% of the general population is active enough to comply with these recommendations.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">4</span></a> Primary care centers appear as the optimal setting to implement interventions aimed at promoting physical activity as they allow interaction and monitoring of a large population. Physical training programs are one of the strategies used to promote physical activity in people at risk of cardiovascular disease. These programs, which have been included in multimodal models analogous to those used in cardiac rehabilitation, have shown their efficacy in increasing moderate–vigorous physical activity and in improving cardiovascular prognosis<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">7–9</span></a> in primary cardiovascular prevention. However, the high human and financial resources required for this type of multimodal interventions make their generalization impractical in primary care. The most commonly used setting to promote physical activity in different types of population in primary care is the exercise referral scheme model in which a member of the primary care team identifies and refers an insufficiently active individual to a third-party service (often a sports center or leisure facility) and this service prescribes and monitors an exercise program. Currently, the evidence on the efficacy of these exercise referral schemes models in promoting moderate–vigorous physical activity is scarce.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">10</span></a> Out of the studies carried out with exercise referral schemes in a population at risk of cardiovascular disease,<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">11–17</span></a> there are few which have obtained a significant short-term impact on moderate physical activity when compared with usual care.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">11,13,16</span></a> However, most studies have shown important methodological limitations such as low adherence to physical exercise sessions,<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">12,16,17</span></a> the non-inclusion of cardiopulmonary exercise tests (gold standard in the assessment and monitoring of physical fitness),<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">12,13,15–17</span></a> and the non-use of validated tools in the physical activity record.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">17</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Our working group, made up of health professionals from the Rehabilitation and Primary Care departments, designed a training program focused on sedentary patients with cardiovascular risk but without established cardiovascular disease, with the aim of promoting moderate–vigorous physical activity. The format of the physical training program had to be feasible and adequate to be applied in the primary care setting. Due to the profile of the population and to promote adherence to the practice of regular physical activity, we included the cardiopulmonary exercise test, for an individualized prescription of exercise, and program performance in small groups led by a specialized professional, to achieve close supervision.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">18</span></a> The duration of the physical training program consisted of 2 or 3 weekly sessions carried out in groups of 6 patients supervised by a physiotherapist over a period of two months. A pilot study without a control group carried out by our team demonstrated the feasibility and safety of the physical training program in the primary care setting and suggested a beneficial effect on physical activity and physical exercise in the short-term.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">19</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">The aim of the current study was to analyze in a randomized clinical trial, the efficacy of two-months/three-weekly sessions’ physical training program to promote moderate–vigorous physical activity and exercise in individuals at risk of suffering a cardiovascular event in the primary care setting.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and method</span><p id="par0030" class="elsevierStylePara elsevierViewall">The trial was registered in the Clinicaltrials.gov (registration number: <a href="ctgov:NCT03717363">NCT03717363</a>). This is a randomized, parallel group, controlled trial design. Blinding was not possible due to the characteristic of the intervention.<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">•</span><p id="par0035" class="elsevierStylePara elsevierViewall">Inclusion criteria<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0040" class="elsevierStylePara elsevierViewall">Patients of both genders aged between 35 and 70 years.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0045" class="elsevierStylePara elsevierViewall">Patients at risk of suffering cardiovascular events defined as: having type 2 diabetes mellitus, and/or metabolic syndrome and/or hypertension with at least one associated cardiovascular risk factor (smoking, dyslipidemia, or overweight/obesity). The hypertension, diabetes mellitus, dyslipidemia and overweight/obesity risk factors were diagnosed according to the European Guidelines on Cardiovascular Disease Prevention<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">20</span></a> and the metabolic syndrome was defined according to the National Cholesterol Education Program/American Heart Association criteria.<a class="elsevierStyleCrossRef" href="#bib0300"><span class="elsevierStyleSup">21</span></a></p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">-</span><p id="par0050" class="elsevierStylePara elsevierViewall">Sedentary patients, defined as having a score ≤2 (equivalent to a frequency of exercise<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>3<span class="elsevierStyleHsp" style=""></span>times/week) in the brief physical activity questionnaire for primary care,<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">22</span></a> and a result ≤1500<span class="elsevierStyleHsp" style=""></span>METs-minutes/week in the International Physical Activity Questionnaire (IPAQ)-long form.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">23</span></a></p></li></ul></p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">•</span><p id="par0055" class="elsevierStylePara elsevierViewall">Exclusion criteria: presence of established cardiovascular disease, history of chronic diseases or other physical or mental limitations that make it impossible to follow a physical training program.</p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">•</span><p id="par0060" class="elsevierStylePara elsevierViewall">Patients were consecutively selected from a primary care center belonging to the reference area of the University Hospital from October 2014 to March 2016. Potential participants were identified using the computerized clinical records of the primary care center.</p></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">•</span><p id="par0065" class="elsevierStylePara elsevierViewall">After signing the informed consent document, patients were assigned to the control group or the intervention group using an automated software to randomize the allocation to each group (Epidat vs 3.0). These variables were measured by a metabolic cart (MasterScreen CPX) on a breath-by-breath basis and averaged over 10-s intervals. Peak oxygen uptake was defined as the highest 30-s average of oxygen uptake in the last minute of exercise. Other peak values were also calculated at the same time point. Anaerobic threshold was determined by the V-slope method and was validated using other plots.</p></li><li class="elsevierStyleListItem" id="lsti0080"><span class="elsevierStyleLabel">4.</span><p id="par0110" class="elsevierStylePara elsevierViewall">Cardiovascular risk factors included changes in sitting blood pressure, waist circumference, and body mass index, as well as in metabolic blood parameters (glycosylated hemoglobin and lipid profile).</p></li></ul></p><p id="par0115" class="elsevierStylePara elsevierViewall">Blood samples were drawn after overnight fasting and were performed by the laboratory nurse at the primary care center. Analyses were performed at the clinical Chemistry Laboratory of Mutua Terrassa Hospital.</p></li><li class="elsevierStyleListItem" id="lsti0085"><span class="elsevierStyleLabel">•</span><p id="par0120" class="elsevierStylePara elsevierViewall">Telephone calls were carried out by study investigators to select patients. Two investigators (physicians specialized in medical rehabilitation) performed all cardiopulmonary exercise testing, and also provided the study questionnaires (the IPAQ-long form<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">23</span></a> and the brief physical activity questionnaire for primary care<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">22</span></a>). An experienced physiotherapist performed all the six-minute walk tests. All measurements were performed at baseline (at the time of allocation) and in the final evaluation (between 10 and 12 weeks after baseline evaluation), corresponding to 2–4 weeks after the end of the training program. The final evaluation was restrictively focused on activities performed once the training program was finished.</p></li><li class="elsevierStyleListItem" id="lsti0090"><span class="elsevierStyleLabel">•</span><p id="par0125" class="elsevierStylePara elsevierViewall">All patients from both groups received a 2-h educational talk on healthy cardiovascular habits two weeks after the baseline evaluation of the study. The intervention group underwent a physical training program which included 3 weekly sessions for 2 months. Each session lasted 60<span class="elsevierStyleHsp" style=""></span>min and included 30<span class="elsevierStyleHsp" style=""></span>min of aerobic exercise with a cycle ergometer and 15<span class="elsevierStyleHsp" style=""></span>min of strength exercises with weights and machines. The intensity of the aerobic exercise was prescribed according to the cardiac frequency registered in the anaerobic threshold in the cardiopulmonary exercise test, and the load of the strength exercise was set up to obtain muscle fatigue of 7 out of 10 according to the visual analog scale after one set of 15 repetitions. The entire physical training program was performed in the rehabilitation area of the primary care center in groups of six patients and supervised by an experienced physiotherapist. The intensity of the exercise was monitored by subjective perception of fatigue according to the visual analog scale and/or by heart rate, recorded through the heart rate monitor that each patient used during the exercise session.</p></li></ul></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0130" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Calculation of sample size:</span> Considering an alpha risk of 0.05 in a bilateral contrast and a statistical power of 80% and estimating a percentage of losses of follow-up of 15%, 73 subjects per group were required to detect a difference between groups of at least 20% (from 10% in the control group to 30% in the intervention group<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">12</span></a>) in the proportion of patients achieving a relevant response to physical activity. The analysis included only patients who completed the study and those in the Intervention Group achieving a good adherence to the physical training program (defined as a percentage of exercise sessions performed<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>70%).<a class="elsevierStyleCrossRefs" href="#bib0340"><span class="elsevierStyleSup">29,33</span></a> Possible differences between groups at baseline were assessed by means of Chi-square, Fisher's exact test, Student's <span class="elsevierStyleItalic">t</span> test, or Mann–Whitney test, when appropriate. Differences between groups in the outcome variables were assessed by multiple logistic regression or general linear models, in all cases adjusted by age, gender, and baseline values. Results are expressed by their multivariate odds ratio (OR) or mean difference with their 95% confidence interval (CI). A <span class="elsevierStyleItalic">p</span> value<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered statistically significant. The Stata S/E v.13.0 software was used.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0135" class="elsevierStylePara elsevierViewall">Two hundred-fifty patients were contacted by phone, 152 of which initially agreed to participate. Three of them were excluded because they did not meet the inclusion criteria, and 2 finally refused to enter the study. A total of 147 patients were randomized, 72 to the control group and 75 to the intervention group. The final evaluation was restricted to 132 patients who completed the study (69 (95.8%) in the control group and 63 (84%) in the intervention group), without significant differences in the rate of lost of follow-up (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.209) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows baseline demographics, cardiovascular risk factors, and physical activity and condition in both groups of patients. There were no significant differences in any of such parameters between groups. The intervention group received a median [Interquartile range 25–75%] of 22 [20–23] exercise sessions per patient with an adherence of 91% [83–95%]. There were no adverse events during the physical training program or the exercise tests.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">The proportion of patients in the intervention group who achieved a “relevant response to physical activity” was significantly higher compared to the control group (34.9% vs. 11.6%; adjusted OR [95%CI]: 4.2 [1.7–10.4]. In addition, significantly more patients in the intervention group achieved an “adequate amount of exercise”, an “adequate frequency of exercise”, and active exercise behavior (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0145" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the comparison of the observed changes in both physical activity and physical condition between groups. Compared to the control group, the intervention group significantly increased moderate activity (mean difference: 260 METs/min/week; 95%CI: 89–432; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005). There were no significant differences between groups in changes observed in the total IPAQ score, walking activity, or sitting time. In none of the two groups was vigorous intensity physical activity present neither at baseline nor in the final assessment.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0150" class="elsevierStylePara elsevierViewall">With respect to physical condition and compared to the control group, the intervention group significantly increased the six-minute walk test distance (mean difference: 28.2<span class="elsevierStyleHsp" style=""></span>m; 95%CI: 10.4–46.0; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.020), the exercise time (0.84<span class="elsevierStyleHsp" style=""></span>min; 0.22–1.46; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.003) and the anaerobic threshold time (0.89<span class="elsevierStyleHsp" style=""></span>min; 0.38–1.40; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002). There were no significant differences between groups in peak oxygen uptake, in peak pulse oxygen, or in oxygen uptake at the anaerobic threshold.</p><p id="par0155" class="elsevierStylePara elsevierViewall">We did not find differences between groups in most parameters related to cardiovascular risk factors (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), such as blood pressure, body mass index or blood metabolic parameters. Men, but not women, allocated to the intervention group significantly reduced waist circumference (mean difference 1.5<span class="elsevierStyleHsp" style=""></span>cm; 95%CI: 0.0–2.9; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.029)</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0160" class="elsevierStylePara elsevierViewall">This study shows that a supervised two-month physical training program is efficacious in promoting moderate–vigorous physical activity and exercise, in the short-term, in individuals at risk of suffering a cardiovascular event attending a primary care center. In addition, it shows the efficacy in improving physical condition. Nevertheless, at least in the short-term, it has no effect on cardiovascular risk factors.</p><p id="par0165" class="elsevierStylePara elsevierViewall">To our knowledge, this is the shortest physical training program carried out in the primary care setting aimed at the population at risk of suffering a cardiovascular event, which registered a significant increase in moderate–vigorous physical activity at 2–4 weeks after the end of the training program using a validated tool.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Some studies with exercise referral schemes carried out in populations with similar characteristics, with a duration of the exercise intervention somewhat longer than ours (from 10 to 16 weeks), have obtained contradictory results in moderate–vigorous physical activity,<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">11–13,15,16</span></a> which may be explained due to the variability of adherence to exercise sessions. Ensuring correct compliance with the exercise intervention is essential to be able to analyze its efficacy in promoting physical activity “a posteriori”.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Those studies with acceptable adherence to the exercise intervention<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">11,13</span></a> significantly impact moderate–vigorous physical activity, unlike others with low adherence.<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">12,16</span></a> A 91% adherence to exercise sessions in our physical training program resulted in a 25% increase in subjects reaching a “relevant response to physical activity”, compared to the control group. The characteristics of our physical training program, which unlike most programs used in primary care, prioritize individualization in the evaluation and prescription of exercise and close supervision, have been able to facilitate high adherence to physical exercise sessions.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">18</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">In addition to its effects on moderate–vigorous physical activity, the present study has found that the physical training program increases the practice of exercise as well as the proportion of patients who present an active behavior in favor of physical exercise. These results are similar to other studies with longer and intensive multimodal approach interventions.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">7,11,28,29</span></a></p><p id="par0185" class="elsevierStylePara elsevierViewall">Our results are aligned with the Spanish multicenter study carried out by Martín-Borràs et al.,<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">13</span></a> who also obtained in the short-term a significantly greater increase in moderate physical activity in the group that received a 3-month exercise referral scheme model compared to those who received the usual care, also recorded with the IPAQ. However, in contrast with our results, they also obtained a significant increase in both walking activity and the total score of the IPAQ. The absence of significant differences in our study in such parameters could be due to an increase in walking activity in the control group, as a result of the active counseling delivered to all participants. As in other studies on interventions aimed at promoting physical activity,<a class="elsevierStyleCrossRefs" href="#bib0365"><span class="elsevierStyleSup">34,35</span></a> in our study no significant differences were obtained in time sitting. It is known that interventions focused primarily on the promotion of physical activity have little effect on sedentary behaviors.<a class="elsevierStyleCrossRef" href="#bib0375"><span class="elsevierStyleSup">36</span></a></p><p id="par0190" class="elsevierStylePara elsevierViewall">We have found that some ergometric parameters, such as the exercise time and the anaerobic threshold time were improved in patients who were randomized to the physical training. However, the peak oxygen uptake did not significantly change. Conversely, in other studies that used the standard effort test instead of the ergometer test (the one used in the current study) have shown disparities in the results.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">8,11,12,14,15,37</span></a> Lingtenberg et al.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">14</span></a> who registered the real peak oxygen uptake through the cardio-pulmonar exercise test, found a significant increase, compared to the control group. However significant differences were based not only on a modest improvement in trained patients, but also in an unexplained worsening in the control group. Our results have shown only mild variations of this parameter which were not clearly different between groups.</p><p id="par0195" class="elsevierStylePara elsevierViewall">We did not find other studies that have performed supervised physical training programs in a population at risk of cardiovascular event in which the distance reached in the six-minute walk test was measured. Nonetheless, we think that it is a useful and simple tool in the primary care setting that allows us to measure changes in physical condition.</p><p id="par0200" class="elsevierStylePara elsevierViewall">In accordance with previous reports,<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">12,28</span></a> no significant differences were found between groups in cardiovascular risk factors. In this sense, a metaanalysis in diabetic population concluded that the physical training program had a small-moderate impact on glycemic and lipid parameters, with minimal or no effect on the remaining cardiovascular risk factors.<a class="elsevierStyleCrossRef" href="#bib0385"><span class="elsevierStyleSup">38</span></a></p><p id="par0205" class="elsevierStylePara elsevierViewall">This study has several limitations. Firstly, physical activity was subjectively reported by patients through a questionnaire. To minimize its impact, the IPAQ long-form,<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">23</span></a> a validated tool having demonstrated a good correlation with more objective measurements such as accelerometers,<a class="elsevierStyleCrossRef" href="#bib0390"><span class="elsevierStyleSup">39</span></a> was completed by all the participants. Another limitation is the short duration of the observation period, which precludes any conclusion regarding the effect in the long-term, necessary to impact on cardiovascular health. Additionally, the ideal duration of these types of exercise interventions remains unknown. In this study, participants performed a two-month intervention that have shown to be of benefit in the short-term in physical condition and physical exercise in a previous pilot study performed by our team.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">19</span></a> A shorter physical training program is important in a population with no cardiovascular disease, presumably actively working, to facilitate adherence to it and also to reduce expenses. Finally, the control group improvements in physical activity and physical condition could underestimate the real impact of the physical training program in this population.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusion</span><p id="par0210" class="elsevierStylePara elsevierViewall">A structured supervised two-month physical training program performed in group sessions is an efficacious tool in primary health care to promote moderate–vigorous physical activity and exercise, in the short-term, in a population at risk of cardiovascular events. This program has proven to be safe and feasible in the context of clinical practices in a primary care setting.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Ethical considerations</span><p id="par0215" class="elsevierStylePara elsevierViewall">This study was approved by the ethical and clinical research committee Mutua Terrassa (Date of approval 01/2014. Our objective was to determine the efficacy of a physical training program to promote moderate–vigorous physical activity and exercise in individuals at risk of suffering a cardiovascular event.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Patients and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Randomized, parallel group controlled trial performed from October 2014 to July 2016. Sedentary patients at risk of suffering a cardiovascular event were randomized to the control group (CG) (72) or the intervention group (IG) (75). Intervention consisted of a 2-month physical training program. The main outcome was the proportion of patients achieving a “relevant response to Physical Activity” (increase<span class="elsevierStyleHsp" style=""></span>≥<span class="elsevierStyleHsp" style=""></span>240<span class="elsevierStyleHsp" style=""></span>METs-min/week in moderate–vigorous Physical Activity using the International Physical Activity Questionnaire (IPAQ)). Secondary outcomes included the proportion of patients performing an “adequate amount of exercise” (≥360<span class="elsevierStyleHsp" style=""></span>METs-min/week in moderate–vigorous exercise using the IPAQ).</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Of the 147 patients randomized, 132 (90%) completed the follow-up. Adherence to exercise performance was 91% and no adverse effects were recorded. The proportion of patients in the IG versus CG achieving the main outcome was 34.9% vs 11.6%; OR [95%CI]: 4.2 [1.7–10.4].
