Estudio ALADIN" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">It is estimated that atrial fibrillation (AF) affects approximately 2% of the general population, a figure that increases with age, the presence of other comorbidities, and health studies compared with populations.<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">1,2</span></a> In the OFRECE study, conducted to determine the prevalence of AF in the <span class="elsevierStyleMonospace">></span>40-year-old population in Spain, this figure was 4.4%.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a> Individuals with AF are usually complex patients who frequently have multiple diseases, are elderly and have had multiple consultations in Internal Medicine and Neurology Units.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In the vast majority of patients with FA, chronic oral anticoagulation will be necessary to prevent the risk of stroke.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">2,5</span></a> Until now, vitamin K antagonists (VKA) have usually been used. Although effective, they have significant limitations that hinder their use in clinical practice.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">2,6</span></a> Direct-acting oral anticoagulants (DAOC) have proven to be at least as effective as VKA in the prevention of stroke and systemic embolism in patients with non-valvular AF (NVAF), and have a better safety profile.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">7,8</span></a> Furthermore, routinely monitoring their anticoagulant activity is not necessary as it can be administered at fixed doses.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> These differences could have a direct impact on the quality of life and patient satisfaction.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> In this context, studies are required that use validated tools to assess these aspects which can in turn assist in improving the treatment given to this population.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">10,11</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The <span class="elsevierStyleItalic">Anti-Clot Treatment Scale</span> (ACTS) is a satisfaction tool specifically designed to assess the burden and benefits of patients’ experience with oral anticoagulant therapy. Although it has been used in observational studies, patients with AF were not included in these studies. Nor has it been validated in the Spanish population.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> The main objective of the ALADIN study–validation of the ACTS questionnaire in patients with atrial fibrillation treated with oral anticoagulants in Spanish Internal Medicine and Neurology Units–was to validate the ACTS questionnaire for patients with AF who were being treated with oral anticoagulants in Spanish Internal Medicine and Neurology Units. Its secondary objectives included: comparing the burden and benefits with oral anticoagulants for patients who consulted Internal Medicine and Neurology Units; studying the burdens and benefits with oral anticoagulants depending on age, sex, CHAD<span class="elsevierStyleInf">2</span>, CHA<span class="elsevierStyleInf">2</span> DS<span class="elsevierStyleInf">2</span>-VASc and HAS-BLED, presence of multiple diseases, kidney function, polypharmacy, type of anticoagulant, degree of INR control (in patients with VKA) and a history of stroke; and describing the clinical profile of patients with AF treated with oral anticoagulants who consulted Spanish Internal Medicine and Neurology Units.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methodology</span><p id="par0020" class="elsevierStylePara elsevierViewall">Sixty-three Neurology and 41 Internal Medicine Units from 74 centres in Spain were included in this cross-sectional, observational study. The researchers consecutively included patients ≥18 years-old diagnosed with NVAF,<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">13,14</span></a> who consulted Neurology or Internal Medicine Units in Spain, have been in stable treatment with oral anticoagulants for ≥3 months and who provided a written informed consent. Individuals excluded included those with mitral stenosis or other significant valve disease that required specific treatment (prosthesis, valvuloplasty) and any kind of disorder that alter the patient's ability to understand/complete the questionnaires; hospitalised at the time of the evaluation; who were or had been participating in clinical trials with ACOD during the previous 6 months; or had a life expectancy of <span class="elsevierStyleMonospace"><</span>6 months. Patients were selected between September 2014 and March 2015. The study was approved by the Hospital Universitario de La Princesa's Ethics Committee in Madrid.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The study consisted of a single visit (to evaluate the test–retest reliability it was necessary to have a limited proportion of the sample complete the questionnaire a second time) without any specific therapeutic intervention. During the visit, information on sociodemographic and clinical variables (treatments, comorbidities) was gathered, as well as CHADS<span class="elsevierStyleInf">2</span>, CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VAS and HAS-BLED scales,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">14</span></a> the Charlson comorbidity index<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">15</span></a> and information regarding the presence of multiple diseases.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Patients completed the following questionnaires (if they needed help completing them, a nurse or doctor verbally explained to them what was being asked, resolving any doubts):<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0035" class="elsevierStylePara elsevierViewall">The ACTS questionnaire<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a>: measures the burden and benefits of patients being treated with oral anticoagulants. It was completed by patients during the first visit and by a selection of the sample in the retest visit, focusing on the last four weeks before the visit. It consists of 17 items and is a Likert-type scale with five possible answers from 1 (none) to 5 (a lot). The first 12 items assess the burden patients experience when taking anticoagulant therapy, question 13 generally assesses the negative impact of this treatment in their daily lives, items 14–16 assess the benefits that the treatment provides them with, and question 17 assesses the positive impact of the treatment on the patient's life (<a class="elsevierStyleCrossRef" href="#sec0040">Appendix I</a><span class="elsevierStyleSmallCaps">).</span> Burden: the higher the score, the lower the burden; benefit: the higher the score, the greater the benefit. The Spanish translation and linguistic validation of the ACTS questionnaire was carried out in 2012 by Oxford Outcomes upon request of the owner, Bayer. The methodology for translation and validation followed currently accepted standards.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">17–20</span></a></p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0040" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Self-Assessment of Treatment Questionnaire</span> (SAT-Q)<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a>: instrument that has 18 questions with a Likert-type scale with five possible answers regarding patient satisfaction with their medication treatment and which assesses their satisfaction with care in the health service. The higher the score, the greater the satisfaction.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0045" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">EuroQol-5 dimensions</span> (EQ-5D)<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a>: generic instrument for measuring the quality of life related to health. Provides three types of information: a descriptive profile of the individual's quality of life, a value for the individual's overall quality of life and a visual analogue scale from 0 to 100 (0 worst condition and 100 best health condition possible). Both the SAT-Q questionnaire and the EQ-5D have been validated for their use in Spanish.</p></li></ul></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Statistical analysis</span><p id="par0050" class="elsevierStylePara elsevierViewall">A descriptive analysis of the sociodemographic and clinical variables was carried out after the answers to the different scales were collected. In the case of quantitative variables, the central tendency and distribution were calculated (mean and standard deviation). For qualitative variables, the frequency distribution was calculated (absolute frequency and percentage). For the comparison of two means, the Student's <span class="elsevierStyleItalic">t</span> test or the Mann–Whitney <span class="elsevierStyleItalic">U</span> test was used depending on the sampling distribution. To compare percentages, the Chi-square test or Fisher test was used, depending on the sample size. All analysis was performed using the statistical package SPSS<span class="elsevierStyleSup">®</span> version 17.0.</p><p id="par0055" class="elsevierStylePara elsevierViewall">In response to the main objective of the study – validating the ACTS questionnaire – the following psychometric properties were specifically assessed using the classical test theory<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a>:<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">•</span><p id="par0060" class="elsevierStylePara elsevierViewall">Feasibility: assessed through the percentage of non-response for each item as well as the time taken to complete the ACTS questionnaire. The ceiling effect (percentage of patients with maximum score) and floor effect (percentage of patients with minimum score) were also assessed for each item and for the overall questionnaire.</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">•</span><p id="par0065" class="elsevierStylePara elsevierViewall">Test–retest reliability: intraclass correlation coefficient was calculated for total scores through the test and retest of the ACTS questionnaire in those patients who showed no clinically significant changes between the first and the second visit, which took place 7–15 days after the study interview. For this analysis the responses to the <span class="elsevierStyleItalic">Clinical Global Impression-Global Improvement</span> (CGI-GI) subscale were used as clinical stability criteria for the sample.</p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">•</span><p id="par0070" class="elsevierStylePara elsevierViewall">Reliability of internal consistency: Cronbach's alpha was calculated as an indicator of the internal consistency of the questionnaire in order to compare the results with those obtained from the original version,<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> as well as the discrimination indices (corrected item-total correlation) of the questionnaire items.</p></li><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">•</span><p id="par0075" class="elsevierStylePara elsevierViewall">Construct validity: the dimensionality of the test was analysed by exploratory factor analysis in order to verify whether the structure of the questionnaire replicated the structure obtained in the original version, which identified two dimensions: burden and benefits.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a></p></li><li class="elsevierStyleListItem" id="lsti0040"><span class="elsevierStyleLabel">•</span><p id="par0080" class="elsevierStylePara elsevierViewall">Convergent validity: Spearman's rho was calculated between the Burden and Benefits scale scores from the ACTS questionnaire and the total scores from the SAT-Q questionnaire, both related to the level of patient satisfaction with treatment. Spearman's rho was also calculated between scores on the ACTS questionnaire Burden and Benefits scales and for the EQ-5D-3L questionnaire total score.</p></li><li class="elsevierStyleListItem" id="lsti0045"><span class="elsevierStyleLabel">•</span><p id="par0085" class="elsevierStylePara elsevierViewall">Criterion validity: the patients’ mean scores from the ACTS questionnaire Burden and Benefits scales were compared according to age, service, type of anticoagulant, CHADS<span class="elsevierStyleInf">2</span> and CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc, multiple diseases and polypharmacy through the Mann–Whitney <span class="elsevierStyleItalic">U</span> test (for two groups) and Kruskal–Wallis test (for more than two groups), assuming that the scores did not meet the assumption of normality.</p></li></ul></p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Results</span><p id="par0090" class="elsevierStylePara elsevierViewall">In the study, 1454 patients were included and 117 were excluded, either because they did not meet inclusion/exclusion criteria or their variables had not been adequately collected; eventually 1337 (91.9%) patients were analysed (865 assessed from the Neurology Unit and 472 from the Internal Medicine Unit).</p><p id="par0095" class="elsevierStylePara elsevierViewall">Regarding the clinical characteristics of the sample, the mean age was 75.63<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.94 years, 55.8% were male, 70.30% had permanent AF, 60.80% history of ischaemic stroke, the average CHADS<span class="elsevierStyleInf">2</span> was 3.21<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.25, CHAD<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">S</span>-VASc was 4.78<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.51 and HAS-BLED was 2.01<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.85. 56.10% of patients were taking VKA and the remaining was being treated with ACOD. The average time on anticoagulant treatment was 36.65<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>48.47 months and the average number of daily tablets was 9.10<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.47. The Charlson comorbidity index was 2.02<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.57.</p><p id="par0100" class="elsevierStylePara elsevierViewall">With regard to the SAT-Q questionnaire, the overall score was 63.79<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17.75 (54.67<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>20.14 in the Effectiveness scale and 82.49<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>17.31 in the Comfort/Convenience scale). In regard to the EQ-5D-3L questionnaire, patients’ mean scores were 75.58<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>20.88 and 63.05<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>20.63 for total score and the analogue visual scale respectively.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The ACTS questionnaire mean scores at the time of the visit (test) showed a high level of patient satisfaction with their anticoagulant therapy (51.84<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.38 on the Burden scale and 11.87<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.45 on the Benefit scale). The ceiling and floor effect analysis is shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The average time patients took to complete the questionnaire was 8.99<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.06<span class="elsevierStyleHsp" style=""></span>min, and 63.70% of the patients needed help with the task.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Seventy-eight patients were considered valid to take the reliability analysis (test–retest) (they had not shown any clinically significant changes according to the CGI-GI subscale and completed the initial ACTS questionnaire and a second one at 7–15 days). A high patient satisfaction with anticoagulation therapy was observed. The scores obtained in the retest were slightly higher than those obtained in the test (Burden scale 53.01<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.06; Benefit scale 12.51<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>2.31). 43.40% of patients needed help to complete the questionnaire. There was a high concordance (average measures) between the test and retest scores: 0.921 intraclass correlation coefficients on the Burden scale and 0.793 for the Benefits scale (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace"><</span><span class="elsevierStyleHsp" style=""></span>0.001 in both cases).</p><p id="par0115" class="elsevierStylePara elsevierViewall">Regarding the reliability assessment (internal consistency), the total (Cronbach's alpha) was 0.95 for the Burden scale and 0.82 for the Benefits scale, values that are compatible with good internal consistency of the measures. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the discrimination indices of the ACTS questionnaire Burden and Benefits scales items (corrected item-total correlation). All values exceed the minimum value of 0.20 and were above 0.50 (indicator of the high internal consistency for both scales).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">For the study of the construct validity, given that in Bartlett's sphericity tests the statistical value was statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace"><</span><span class="elsevierStyleHsp" style=""></span>0.05), the null hypothesis was rejected so the significant correlations between variables could be confirmed and, consequently, the factorial model was relevant. The results also showed how the two components obtained explained the 54.98% of the variance of scores (39.47% for the Burden scale and 15.51% for the Benefit scale). All the values obtained in the rotated component matrix were above 0.60 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0125" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the data concerning convergent validity by comparing the Spearman rho between the ACTS scale and the SAT-Q and EQ-5D-3L questionnaire scores. All correlations with SAT-Q questionnaire were positive, moderate and statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace"><</span><span class="elsevierStyleHsp" style=""></span>0.01): high scores on the ACTS questionnaire (greater satisfaction) were associated with a high degree of satisfaction measured by the SAT-Q questionnaire. The correlation with the total score in the EQ-5D-3L questionnaire was positive, low and statistically significant, indicating that patient satisfaction also was associated with quality of life for them.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">The convergent/discriminant validity of the ACTS questionnaire items was also studied by calculating Spearman's rho between the items and the Burden and Benefit scales’ total score (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>). All the correlations of the items and the total score of the scale to which they belonged were positive, high and statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace"><</span><span class="elsevierStyleHsp" style=""></span>0.05).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0135" class="elsevierStylePara elsevierViewall">For the criterion validity, the patients’ mean scores on the ACTS questionnaire Burden and Benefit scales were compared according to different variables. No statistically significant differences between age groups or according to polypharmacy in any of the ACTS subscale were observed. However, significant differences were found according to: service (in the Burden scale, Neurology patients showed higher satisfaction with anticoagulation therapy), type of anticoagulant treatment (patient satisfaction was higher when they were being treated with ACOD), the CHADS<span class="elsevierStyleInf">2</span> scale (in the Burden scale, patients with <span class="elsevierStyleMonospace">></span>2 CHADS<span class="elsevierStyleInf">2</span> were more satisfied), the CHA <span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc scale (on the Burden scale, patients with <span class="elsevierStyleMonospace">></span>3 CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc were more satisfied) and the presence of multiple diseases (in the Burden scale, multi-disease patients showed less satisfaction with anticoagulation therapy) (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>).</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Discussion</span><p id="par0140" class="elsevierStylePara elsevierViewall">Most studies examining antithrombotic therapy in patients with AF have focused mainly on the analysis of both efficacy (prevention of thromboembolic complications) and safety (haemorrhaging). However, despite great clinical interest, there are limited studies that analyse the impact anticoagulant treatment has on AF patients’ quality of life and their levels of satisfaction and burden.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> Fortunately, it seems this type of analysis is being increasingly prioritised and there are certain specific recommendations in clinical practice guidelines.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> Consequently, having validated instruments that allow us to analyse patients undergoing this type of chronic treatment is essential.</p><p id="par0145" class="elsevierStylePara elsevierViewall">At present there are generic questionnaires on the quality of life, such as the SF-36, or questionnaires that assess satisfaction with treatment, such as the SAT-Q, or questionnaires specifically designed to analyse the quality of life in patients with AF, such as the AF-QoL or questionnaires that assess aspects of anticoagulated patients’ quality of life, such as the DASS or PACT-Q.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">10,21,22</span></a> Although useful, these questionnaires have significant limitations when assessing the patient with atrial fibrillation undergoing anticoagulant treatment, either because some are not specifically designed to analyse these aspects, or because–despite being specifically designed to study the impact of anticoagulant treatment–they are relatively complex. For example, the DASS scale, which analyses both negatives and anticoagulation aspects, has 25 items. The PACT-Q consists of 27 items grouped into four categories (Expectations of treatment, Convenience, Burden of disease and treatment, and Satisfaction with anticoagulant therapy).<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">10,21,22</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">The ACTS questionnaire is a satisfaction tool specifically designed to assess the burden and benefits of patients being treated with oral anticoagulants, consisting solely of 17 items. This scale has been validated in patients with deep vein thrombosis, but not in patients with atrial fibrillation, and in several languages (German, Italian, French, and English), but not in Spanish.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> Consequently, ALADIN is the first study that has validated the ACTS questionnaire in Spanish and specifically applied it to patients with atrial fibrillation undergoing anticoagulant treatment. There are other questionnaires, such as SAFUCA, that includes 25 items grouped into seven categories (Efficacy of medication, Convenience of medication, Anticoagulation monitoring, Interference of the medication with daily life, Undesirable effects, Medical follow-up and General satisfaction), and assess the satisfaction of medical care given.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">23</span></a> Therefore, the different questionnaires, far from overlapping, provide additional information.</p><p id="par0155" class="elsevierStylePara elsevierViewall">The sample size with which our validation was conducted (n<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleMonospace">=</span><span class="elsevierStyleHsp" style=""></span>1337) was very similar to that used in the questionnaire validation study conducted on a sample of patients with venous thromboembolism (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1336).<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> However, while in our study the validation was done only in Spanish, the previous validation study was done in several languages, which adds value as it increases the statistical power.</p><p id="par0160" class="elsevierStylePara elsevierViewall">Patients included in our study had a high comorbidity (mean age 76, 61% had a history of stroke, Charlson comorbidity index was 2.0) and a high risk of thromboembolic events (CHADS<span class="elsevierStyleInf">2</span> 3,2; CHAD<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc 4.8). This population accurately represents the type of AF patients who usually consult Neurology and Internal Medicine Units, and is quite similar to the population included in the ROCKET-AF or ENGAGE AF-TIMI 48 clinical trials.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">24,25</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">Regarding the results of the questionnaire obtained in our study, patients’ average score in the SAT-Q highlighted their high satisfaction with the medication used to treat them and the care provided to them by the health service, and the results of the EQ-5D-3L questionnaire highlighted a good quality of life related to health. The descriptive analysis of the ACTS questionnaire showed a high patient satisfaction with anticoagulation therapy. Significantly, the average time patients had been undergoing anticoagulant treatment for was 36.65 months and this might have influenced their high satisfaction levels. It is possible that if patients had been undergoing treatment for a shorter time, their degree of satisfaction would not have been so high.</p><p id="par0170" class="elsevierStylePara elsevierViewall">With regards to the validation of the ACTS questionnaire, and specifically feasibility, the results were very similar to those of the original validation, with low ceiling and floor effects on both scales.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> Another important aspect is that although the patients were elderly and had significant comorbidities, the average time of completion of the test could be considered acceptable (9<span class="elsevierStyleHsp" style=""></span>min), although a high percentage of patients needed help to complete it. It is very likely that in less complex patients, completion time would have been lower.</p><p id="par0175" class="elsevierStylePara elsevierViewall">The results of the analysis of reliability and discrimination indices highlighted the high internal consistency of the questionnaire. The test–retest reliability study showed that the ACTS questionnaire was a stable assessment instrument for those patients who did not show any clinically relevant changes between the first and second visit, with a high correlation between scores on the test and retest. In fact, the results largely replicated the questionnaire's original reliability.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> The observed factorial structure (construct validity) confirmed the presence of the two hypothesised dimensions (Burden and Benefits) with higher factor weightings than those obtained with the original validation of the test.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">Regarding the study of the association between the ACTS questionnaire and the SAT-Q and EQ-5D questionnaires, all correlations obtained were positive, statistically significant and generally higher than those obtained in the original validation study, where the degree of association between the ACTS questionnaire scores and the TSQM II satisfaction questionnaire was studied.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a></p><p id="par0185" class="elsevierStylePara elsevierViewall">The study of the ACTS questionnaire criterion validity showed how satisfaction with anticoagulant treatment was higher in patients treated with ACOD, and statistically significant differences were observed in the two scales (Burden and Benefits), which could be related to the advantages over ACOD on VKA,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> both objective advantages and those perceived by the patient. This corresponds with recent publications that show how–when compared with VKA–treatment with ACOD is associated with less anxiety and depression.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a></p><p id="par0190" class="elsevierStylePara elsevierViewall">Although the psychometric evaluation of the ACTS questionnaire was relatively extensive, the main limitation of this study is that this was done on a population of patients with AF who had certain clinical characteristics (patients consulting Neurology and Internal Medicine Units). Consequently, it would be interesting to perform further analysis on other subgroups of patients with AF. However, the quality of the analyses performed and the sample size used, which was similar to the previous validation study,<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a> but this time conducted only in Spanish, have helped reduce this potential limitation. Furthermore, the results show that there is a ceiling effect especially in the Burden items, so it is possible that the questionnaire does not discriminate well in a context of maximum values. Since the main objective of the study was to validate the ACTS questionnaire in Spanish, and not conduct analysis of subgroups, this information should be taken with caution. However, the subgroup analysis conducted in this study was one of the secondary objectives. Finally, approximately 64% of patients needed help to complete the questionnaire. Although the possibility assistance for completing forms exists for all questionnaires, it is best when patients do it themselves.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Conclusions</span><p id="par0195" class="elsevierStylePara elsevierViewall">This study provides fundamental information on the satisfaction levels of patients with atrial fibrillation being treated with oral anticoagulants, who consult Neurology and Internal Medicine Units in Spain. The patients completed ACTS, Self-Assessment of Treatment Questionnaire (SAT-Q) and EuroQol-5 dimensions (EQ-5D) questionnaires. ACTS is a satisfaction tool that specifically analyzes burdens (higher score, lesser burden) and benefits (higher score, higher benefit) with anticoagulant treatment. The psychometric properties of the questionnaire were evaluated according to the classical test theory.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">The average time to complete the questionnaire was 8.99<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6.06<span class="elsevierStyleHsp" style=""></span>min and 63.70% of patients needed assistance to complete it. There was a high concordance between test and retest scores. Total reliability (Cronbach's alpha) was 0.95 in the ACTS Burdens scale and 0.82 in the ACTS Benefits scale. The factorial model was pertinent. All correlations with the SAT-Q questionnaire were positive, moderate and statistically significant. With regard to the EQ-5D-3L questionnaire, correlations were positive, low and statistically significant. Los pacientes completaron los cuestionarios ACTS, <span class="elsevierStyleItalic">Self-Assessment of Treatment Questionnaire</span> (SAT-Q) y <span class="elsevierStyleItalic">EuroQol-5 dimensions</span> (5Q-5D). La escala ACTS es un instrumento de satisfacción específico para la evaluación de la carga (mayor puntuación, menor carga) y beneficios (mayor puntuación, mayor beneficio) con el tratamiento anticoagulante. Se evaluaron las propiedades psicométricas del cuestionario de acuerdo con la teoría clásica de los test.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">El tiempo medio en cumplimentar el cuestionario fue 8,99<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>6,06<span class="elsevierStyleHsp" style=""></span>min y el 63,70% de los pacientes precisaron ayuda para la cumplimentación del mismo. Existió una elevada concordancia entre las puntuaciones del test y el retest. La fiabilidad total (alfa de Cronbach) fue 0,95 en la escala Carga y 0,82 en la escala Beneficio. El modelo factorial resultó pertinente. Todas las correlaciones con el cuestionario SAT-Q fueron positivas, moderadas y estadísticamente significativas. Con respecto al cuestionario EQ-5D-3L, estas fueron positivas, bajas y estadísticamente significativas.
